Anti-retroviral therapy (ART) has decreased human immunodeficiency virus (HIV)-1-associated morbidity. However, despite ART, immune cells remain latently infected, leading to chronic inflammation and HIV-1-associated comorbidities. New strategies are needed to target viral proteins and inflammation. We found activation of Notch3 in renal cells of the HIV-1 transgenic mouse model (HIV-Tg26) and in patients with HIV-associated nephropathy. We hypothesized that targeting NOTCH3 activation constitutes an effective therapy for HIV-related chronic kidney disease. We generated HIV-Tg26 mice with Notch3 knocked out (Tg-N3KO). Compared to HIV-Tg26 mice at 3 months, Tg-N3KO mice showed a marked reduction in renal injury, skin lesions and mortality …

Patients diagnosed with metastatic basal cell carcinoma (BCC) have a poor prognosis. The current standard of care for adults with locally advanced or metastatic BCC who are not candidates for surgery or radiation therapy is treatment with hedgehog pathway inhibitors (HHIs). For patients who progress while on this therapy, further treatment options are limited. There is also a need for real-world clinical practice data on the clinical characteristics, management, disease progression, and survivorship of these patients. The ongoing CemiplimAb-rwlc Survivorship and Epidemiology (CASE) study is a phase IV, multicenter, prospective, noninterventional survivorship and epidemiology cohort study evaluating the effectiveness and …

Advancing Literary Interpretation Via Critical Reading Proficiency: A Sustainable Efl Framework For Generation Z, Wesam Khairy Morsi, Walid Abdallah Rezk
English Language and Literature
Abstract—This research investigates the transformative potential of literary analysis in advancing English proficiency among Egyptian EFL learners, particularly those in the Faculty of Arts and Humanities. The study focuses on the linguistic and analytical challenges encountered by recent public-school graduates in the Suez Governorate, where English instruction is limited to simplified texts and lacks critical engagement. Unlike their counterparts from language schools in Cairo, these students enter university without placement tests, relying solely on their high school grades, which results in underdeveloped critical reading and thinking skills. Sixty Egyptian students participated in this experimental quantitative research design. Drawing on the …
We Need To Talk: Modernizing Attorney-Client Jail Communications, Cynthia Alkon
We Need To Talk: Modernizing Attorney-Client Jail Communications, Cynthia Alkon
Faculty Scholarship
Attorney-client jail phone calls, video calls, and emails are all routinely recorded and monitored by jails, with numerous examples of jails and private telecommunications providers giving those recordings and emails to law enforcement and prosecutors. This widespread failure to protect the confidentiality of attorney-client communications prevents lawyers from being able to enjoy easy, quick, and regular communications with their clients. In practice, and despite the ever-expanding communication methods in the outside world, those held in jails in this country while their criminal cases are pending are effectively stuck in the nineteenth century when the only way to communicate was in …

Human pluripotent stem cell-derived β-cells (SC-β-cells) represent an alternative cell source for transplantation in diabetic patients. Although mitogens could in theory be used to expand β-cells, adult β-cells very rarely replicate. In contrast, newly formed β-cells, including SC-β-cells, display higher proliferative capacity and distinct transcriptional and functional profiles. Through bidirectional expression modulation and single-cell RNA-seq, we identified SPOCK2, an ECM protein, as an inhibitor of immature β-cell proliferation. Human β-cells lacking SPOCK2 presented elevated MMP2 expression and activity, leading to β-integrin-FAK-c-JUN pathway activation. Treatment with the MMP2 protein resulted in pronounced short- and long-term SC-β-cell expansion, significantly increasing glucose-stimulated insulin …

Background: The management of acute ischemic stroke (AIS) was revolutionized within the last 15 years with the introduction of mechanical thrombectomy (MT) to standard of care. Despite the success of mechanical thrombectomy (MT) in achieving high recanalization rates for large vessel occlusion, functional independence post-treatment remains suboptimal. The current limitations of MT prompt evaluation of the role of adjunctive pharmacologic neuroprotective therapies to prevent excitotoxicity, cellular apoptosis, and inflammation that cause irreversible neuronal damage during AIS. Magnesium (MgSO4) provides an attractive neuroprotectant profile, having many different effects, and is inexpensive, readily available, and has a long-established safety and tolerability profile …

Amygdala activation by emotional arousal during memory formation can prioritize events for long-term memory. Building upon our prior demonstration that brief electrical stimulation to the human amygdala reliably improved long-term recognition memory for images of neutral objects without eliciting an emotional response, our study aims to explore and describe individual differences and stimulation-related factors in amygdala-mediated memory modulation. Thirty-one patients undergoing intracranial monitoring for intractable epilepsy were shown neutral object images paired with direct amygdala stimulation during encoding with recognition memory tested immediately and one day later. Adding to our prior sample, we found an overall memory enhancement effect without …

Background and objectives: Corticobasal syndrome (CBS) is a complex of symptoms and signs comprising limb rigidity, bradykinesia, dystonia, myoclonus, apraxia, cortical sensory loss, and a variety of cognitive and language impairments. CBS is commonly seen in tauopathies. Striking asymmetry in clinical and imaging findings in CBS raises questions about potential triggers initiating neurodegeneration. The objective of this study was to investigate potential central or peripheral triggers preceding CBS symptoms.
Methods: In this retrospective observational study, we reviewed medical records of patients with CBS at our Parkinson's Disease Center and Movement Disorders Clinic, focusing on evidence of possible central or peripheral …

The child welfare system in the United States requires collaboration across various agencies in the provision of services. Although collaboration between mental health therapists and state agencies is fundamental when the goal is to provide quality care and achieve positive outcomes for families involved with Child Protective Services (CPS), the system is characterized as complicated and fragmented. Using Bronstein’s 2003 model of interprofessional collaboration to understand the dynamics of collaboration between mental health therapists and CPS workers in Texas, this qualitative descriptive research study described mental health therapists’ perceptions of collaborating with CPS workers in delivering therapy services to CPS-involved …
Improving Outcomes Of Depression Through Implementation Of Depression Screenings At A College Health Center, Shelby Leigh Hanigan
Improving Outcomes Of Depression Through Implementation Of Depression Screenings At A College Health Center, Shelby Leigh Hanigan
Electronic Theses and Dissertations
Depression is a common phenomenon. Depression and suicidality are a problem on university campuses. This quality improvement Doctor of Nursing project aimed to establish the importance of routine depression screenings within a college health clinic. The purpose of this project was to determine if implementing routine depression screenings at a university health clinic will or will not increase the number of counseling referrals when compared to the absence of depression screenings in place within an identified timeframe. This protocol screened for depression in college-aged students and suggested care to those students who screened positive. It also led an interprofessional team …

Advisor Satisfaction To Support Online Student Retention, Erica K. Whiddon
Electronic Theses and Dissertations
In this dissertation, the researcher explored the relationship between satisfaction with academic advising and online student retention. Online student retention is a critical issue in the context of increasing competition for students in higher education institutions. The specific problem was that online students have lower retention rates, and the reasons students typically select online degree programs are also the reasons they drop out of their degree programs. The purpose was to identify whether students’ reported satisfaction with their advisors mediated the relationship between the existence of external personal life factors and retention. A quantitative nonexperimental causal-comparative and correlational research design …
K–12 Educators’ Use Of Technology And Experiences With Technostress: A Case Study, Greg Blair Pittman
K–12 Educators’ Use Of Technology And Experiences With Technostress: A Case Study, Greg Blair Pittman
Electronic Theses and Dissertations
Abstract
This dissertation explored the increased use of technology in K–12 classrooms, which is causing educators to experience technostress. The purpose of the qualitative, descriptive, single case study was to explore the experiences of K–12 teachers with technology and technostress—techno-insecurity, techno-invasion, techno-complexity, techno-uncertainty, and techno-overload—in a West Texas private school district. The researcher conducted one-to-one interviews with seven K-12 educators and two senior leadership team members. In addition, open-ended online questionnaires were available for K–12 educators and senior leadership team members. Thematic analysis was used to locate and communicate the findings. The principal research question was: How do K–12 classroom …

Rationale & objective: Atrial fibrillation (AF) is common in patients with kidney failure on hemodialysis (HD), but few patients receive oral anticoagulant (OAC) treatment. Availability of direct-target OACs starting in 2010 may have induced greater OAC initiation, but this has not been systematically studied.
Study design: Retrospective cohort study.
Setting & participants: Using Medicare fee-for-service billing claims (2006-2020), we identified previously OAC-naïve HD patients newly-diagnosed with AF between January 1, 2007, and October 1, 2020.
Exposures: Calendar year; race/ethnicity.
Outcomes: OAC initiation within 90 days from AF diagnosis (any; specific agent).
Analytical approach: We estimated initiation risk ratios for each …

MYCN-amplification is a genetic hallmark of ~40% of high-risk neuroblastomas (NBs). Altered glycosylation is a common feature of adult cancer progression, but little is known about how genetic signatures such as MYCN-amplification alter glycosylation profiles. Herein, matrix-assisted laser desorption/ionization mass spectrometry imaging (MALDI-MSI) revealed increased core fucosylated glycan abundance within neuroblast-rich regions of human MYCN-amplified NB tumors. GDP-mannose 4,6-dehydratase (GMDS) is responsible for the first-committed and rate-limiting step of de novo GDP-fucose synthesis. High GMDS expression was found to be associated with poor patient survival, advanced stage disease, and MYCN-amplification in human NB tumors. Chromatin immunoprecipitation and promoter reporter assays …
