Background: Sensitivity and specificity are foundational metrics for cancer detection tools. However, most machine learning algorithms prioritize overall accuracy during optimization, which fails to align with clinical priorities of early detection. We aim to develop a feature selection machine learning algorithm while maximizing sensitivity at a given specificity.
Methods: We developed SMAGS-LASSO, a machine learning algorithm that combines our developed Sensitivity Maximization at a Given Specificity (SMAGS) framework with L1 regularization for feature selection. This approach simultaneously optimizes sensitivity at user-defined specificity thresholds while performing feature selection. IMPORTANCE: HIV preexposure prophylaxis (PrEP) is highly effective in HIV prevention; telemedicine is a promising avenue for delivering PrEP care.
OBJECTIVE: To explore telemedicine delivery of PrEP care (telePrEP) by the largest telePrEP company in the US.
DESIGN, SETTING, AND PARTICIPANTS: This retrospective, single-arm cohort study used health record data of patients receiving telePrEP from MISTR LLC, a for-profit telehealth company, from November 27, 2018, to March 5, 2025. Patient characteristics and AIDSVu data were assessed to determine levels of PrEP use nationally. Retention in care was assessed for persons initiating PrEP in 2020, 2021, and 2022. Objectives: Onconephrology is an expanding subspecialty focused on the management of cancer patients with renal injury. This study used a comprehensive bibliometric analysis to emphasize the need for cooperation between oncologists and nephrologists, exploring current trends and future research areas in onconephrology.
Methods: Relevant literature on onconephrology published between 1 January 2000 and 27 April 2024 was retrieved from the Science Citation Index Expanded of the Web of Science Core Collection, followed by manual screening. Bibliometric analyses were performed using CiteSpace, VOSviewer, and Bibliometrix software.

Fusobacterium nucleatum (Fn) is commonly enriched in colorectal cancer (CRC) and associated with poor outcomes, though its mechanisms remain unclear. Our study investigated how Fn affects the tumor microenvironment through single-cell transcriptomic analyses of 42 CRC patient tissues, comparing Fn-positive and Fn-negative tumors. We discovered that Fn impairs IgA plasma cell development and secretory IgA (sIgA) production by disrupting communication with tumor-associated macrophages. Additional experiments in germ-free mice, together with our re-analysis of a publicly available single-cell RNA-seq data set from a CRC mouse model with an intact gut microbiome–both models having been orally gavaged with Fn–jointly validated the causal …

Developing autonomous sensor systems that draw power from the ambient environment is a critical step for creating the Internet of Things. In this study, the authors built dozens of graphene-based solar cells, wire bonded them into standard packages, and characterized the current-voltage characteristics of each under illumination. Next, solar cells were connected in series to increase the output voltage. Three different sets of solar cells were used to charge three storage capacitors to the voltage levels required by our temperature sensor. The storage capacitors require only a few minutes to charge, yet power the sensor system for more than 24 …

Falls among older adults remain a leading cause of morbidity and mortality, especially in underserved populations. The Centers for Disease Control and Prevention (CDC) developed the Stopping Elderly Accidents, Deaths, and Injuries (STEADI) toolkit to help providers systematically identify and address fall risks. UNLABELLED: Metastasis is the main cause of prostate cancer-associated deaths, highlighting the urgent need to determine the mechanisms underlying prostate cancer progression. TROP2 (also known as tumor-associated calcium signal transducer 2) is an oncogenic transmembrane surface protein that is highly expressed in metastatic prostate cancer. Naturally occurring cleavage of TROP2 leads to a release of the TROP2 extracellular domain (TECD) into the extracellular environment. In this study, we identified an important functional role of TECD in prostate cancer metastasis. Latino families with emerging adults diagnosed with Autism Spectrum Disorder (ASD) face unique challenges during the transition to adulthood, influenced by cultural values, acculturation, and systemic barriers. Acculturation and acculturative stress are particularly relevant in this population, as they navigate cultural adaptation while maintaining traditional values like familismo (family) and respeto (respect). Despite extensive research on customer continuance intention, few studies have explored the mediating roles of cognitive and affective inertia in shaping continuance intention, particularly in emerging markets. We address this gap in an empirical study, grounded in the SOR (Stimulus-Organism-Response) framework, that investigates how trust, satisfaction, and switching barriers influence mobile network users' continuance intention, with cognitive and affective inertia as mediators. Using Partial Least Squares Structural Equation Modelling (PLS-SEM), we analyzed primary data from 407 mobile phone users in Bangladesh, an emerging economy. The findings indicate that trust, satisfaction, and switching barriers have a significant influence on continuance intention. …

As the global demand for renewable energy intensifies, piezoelectric energy harvesting from roadways has emerged as a promising avenue for sustainable power generation. This systematic literature review analyzes 61 peer-reviewed studies to assess the feasibility, performance, and potential of integrating piezoelectric systems into roadway infrastructure. While technology faces challenges, such as high installation costs, limited energy output, and a scarcity of thorough economic evaluations, findings suggest it holds considerable promise as a supplementary renewable energy source. Understanding the initial signature of noise-induced auditory damage remains a significant priority. Animal models suggest the cochlear base is particularly vulnerable to noise, raising the possibility that early-stage noise exposure could be linked to basal cochlear dysfunction, even when thresholds at 0.25-8 kHz are normal. To investigate this in humans, we conducted a meta-analysis following a systematic review, examining the association between noise exposure and hearing in frequencies from 9 to 20 kHz as a marker for basal cochlear dysfunction. The Mississippi Supreme Court in Loblolly Properties, LLC v. Le Papillon Homeowner’s Association, Inc. held that the nonjudicial foreclosure of a deed of trust does not extinguish restrictive covenants to pay HOA fees that were filed after the recording of the deed of trust. In doing so, the Court blatantly disregarded the fundamental understanding of lien priority in Mississippi, and also failed to explain how the policy behind lien priority would further be upheld in the future. This decision by the Supreme Court is incorrect and places Mississippi in a “super-minority” of jurisdictions on this issue. The overwhelming majority of …

Antipsychotic drugs are essential in the management of schizophrenia, bipolar disorder, and other psychiatric illnesses, but their use is closely linked to cardiometabolic side effects. Cardiometabolic side effects include weight gain, dyslipidemia, glucose intolerance, and increased cardiovascular morbidity and mortality. Mechanisms of cardiometabolic side effects involve disruption of the central nervous system’s ability to regulate appetite, peripheral metabolic effects, which are changes in metabolism outside the CNS, and receptor-level interactions, with second-generation antipsychotics generally posing a greater risk than first-generation agents. Background: The term sarcopenia was introduced to describe the age-related decline in muscle mass, but current definitions also include measures of muscle strength and function. Menopause increases sarcopenia risk and may exacerbate the adverse effects of physical inactivity. Exercise training is a potent stimulus to restore muscle health, and nutritional supplementation can further improve the outcomes. This multi-generational research project represents an academic partnership between Edith Cowan University (ECU) and Chung Wah Community Care (CWCC), conducted from February to August 2025. Through CWCC's established community participation programs, this collaboration provided both a rich platform for community engagement and an accessible entry point for an ECU Research Internship to collect and analyse in-depth data on the "homing" experiences of Chinese elderly immigrants in Australia.

The heat capacity, thermal diffusivity and thermal conductivity of (Hf,Nb,Ta,Ti,Zr)C-(Hf,Nb,Ta,Ti,Zr)B2 high entropy dual phase ceramics were evaluated from room temperature to 1800 °C and compared with the constituent high entropy single phase ceramics of the same nominal compositions. Thermal conductivity of the (Hf,Nb,Ta,Ti,Zr)C-(Hf,Nb,Ta,Ti,Zr)B2 dual phase ceramic increased from 17.7 W m−1 K−1 at room temperature to 54 W m−1 K−1 at 1800 °C. The addition of WC to the dual phase ceramic reduced its thermal conductivity. For the addition of 5 wt% WC, room temperature thermal conductivity was 16.8 W m−1 K−1 …

As the Laser powder bed fusion (LPBF) process advances toward industrial-scale production, the sustainability of metal powder feedstocks has become a focal point of research. The powder recycling approach, while environmentally and economically favorable, results in a series of physicochemical variations that could compromise the part quality. The resulting variation in powder characteristics with powder recycling could affect the powder layer density and melt pool dynamics, resulting in defect formation and hindering the structural integrity of LPBFed parts. An experimental continuous-wave (cw) pump-probe scheme is demonstrated by investigating the population and photoionization dynamics of an atomic system. In particular, 6Li atoms are initially prepared in optically pumped 22S1/2 and 22P3/2 states before being excited via multi-photon absorption from a tunable femtosecond laser. The subsequent cascade back to the ground state is analyzed by ionizing the atoms in the field of a cw optical dipole trap laser. Conventional spectroscopic methods, such as standard cold-target recoil ion momentum spectroscopy or velocity map imaging, cannot provide simultaneous momentum and time-resolved information on an event-by-event basis for the system investigated here. Detecting vulnerabilities in smart contracts is vital for the security and reliability of decentralized apps. To facilitate vulnerability detection, contract codes, including bug patterns, are represented as heterogeneous graphs with various nodes and edges, like control-flow and function-call graphs. Offline Learning from Observations (LfO) focuses on enabling agents to imitate expert behavior using datasets that contain only expert state trajectories and separate transition data with suboptimal actions. This setting is both practical and critical in real-world scenarios where direct environment interaction or access to expert action labels is costly, risky, or infeasible. This paper addresses the problem of learning avoidance behavior within the context of offline imitation learning. In contrast to conventional methodologies that prioritize the replication of expert or near-expert demonstrations, our work investigates a setting where expert (or desirable) data is absent, and the objective is to learn to eschew undesirable actions by leveraging demonstrations of such behavior (i.e., learning from negative examples).To address this challenge, we propose a novel training objective grounded in the maximum entropy principle. Although Federated Learning (FL) is promising for privacy-preserving collaborative model training, it suffers from low inference performance due to heterogeneous client data. Due to heterogeneous data across clients, FL training easily learns client-specific overfitting features. Existing FL methods adopt coarsegrained averaging, which can easily cause the global model to get stuck in local optima, leading to poor generalization. Due to the communication bottleneck in distributed and decentralized federated learning applications, algorithms using compressed communication have attracted significant attention. The Error Feedback (EF) is a widely-studied compression framework for convergence with biased compressors such as top-k sparsification. Although various improvements have been obtained in recent years, the theoretical guarantee for EF-type framework is still limited.
