Background and objectives: Emergency medical services (EMS) clinicians play a critical role in early identification and management of neurologic emergencies yet may encounter sparse neurology-specific education. With interprofessional collaboration, we developed a multimodal, active-learning, and experiential curriculum for teaching prehospital care in a neurologically underserved setting. We evaluated curriculum feasibility and impact on prehospital neurologic assessment. Introduction: Attention-Deficit/Hyperactivity Disorder (ADHD) is a neurodevelopmental disorder that affects 5 to 8% of youth and 2.5 to 4% of adults. Pharmacological treatments are among the most effective when treating ADHD, yet adherence to medication is still sub-optimal. We reviewed the literature on clinical and socio demographic factors that may influence adherence and compared it to our clinical data.

Background: Composite heel defects involving the Achilles tendon and overlying soft tissue are rare and technically demanding. Successful management requires restoration of tendon continuity and durable coverage. We reviewed the literature and presented a representative case to illustrate current strategies.

Methods: A literature review of 28 studies that reported outcomes of Achilles tendon reconstruction with concomitant soft tissue coverage was performed. Tendon restoration techniques(allografts, autografts, vascularized grafts) and flap types (muscle and fasciocutaneous) were analyzed for functional recovery, complication rates, and donor-site morbidity. Background and objectives: During the COVID-19 pandemic, we tested the efficacy of a one-week online security prime (SP) writing intervention in reducing distress among 254 adults (60+ years and/or having an underlying health condition). The efficacy of writing interventions can depend on several factors. Attachment orientations, characterized by dimensions of anxiety and avoidance, reflect individuals' tendencies in how they seek proximity to and rely on others for support, and influence how people experience and regulate their emotions.

Objective: To evaluate whether antenatal betamethasone affects childhood respiratory impairment.

Methods: This was a prospective follow-up study of children aged 6 years and older from parents in the ALPS (Antenatal Late Preterm Steroids) trial randomized to betamethasone or placebo from 34 0/7 to 36 6/7 weeks of gestation. Background: The ARIES-HM3 trial demonstrated the safety and effectiveness of aspirin elimination from the antithrombotic regimen after HeartMate 3 (HM3) left ventricular assist device (LVAD) implantation. We explored the interaction of atrial fibrillation, diabetes mellitus, and obesity (AF/DM/Ob) with aspirin elimination on hemocompatibility-related adverse events at 1-year postimplant.

Methods: This prospective, double-blind, placebo-controlled trial randomized patients with an HM3 LVAD implant to receive aspirin (100 mg/d) or placebo, in addition to a vitamin K antagonist. Background: The management of head and neck cancer relies on multidisciplinary expertise; however, access to tumor boards remains variable. Large language models (LLMs) may support guideline-based decision-making, although performance in complex oncologic scenarios is not well defined.

Methods: Fourteen synthetic cases based on real tumor board encounters were evaluated. Five blinded comparator arms produced recommendations: a human expert, Non-RAG-GPT-4, Non-RAG-GPT-5, RAG-GPT-4, and RAG-GPT-5. Eight head and neck oncologic surgeons scored each recommendation for appropriateness, clarity, specificity, and feasibility using 5-point Likert scales. Paired permutation testing and inter-rater reliability were assessed.

Background: The pullback pressure gradient (PPG) is a novel physiological metric that quantifies coronary artery disease patterns as focal or diffuse on a scale from 0 to 1. This study assessed the relationship between PPG and residual angina at 1 year.

Methods: PPG Global is a prospective, investigator-initiated, single-arm, multicenter study that enrolled patients with at least 1 lesion with a fractional flow reserve ≤0.80 intended to be treated with percutaneous coronary intervention. Aims: Existing ST-segment elevation myocardial infarction (STEMI) alert pathways that rely on traditional STEMI criteria perform suboptimally. We aimed to evaluate the diagnostic performance of an artificial intelligence (AI) model to detect acute occlusion myocardial infarction (OMI) from the routine 12-lead electrocardiogram (ECG) and, specifically, its potential to reduce false-positive activations.

Methods and results: Consecutive adults managed via the STEMI pathway were included from a tertiary academic medical centre between January 2022 and December 2023. Given the morbidity and mortality associated with colorectal cancer (CRC), novel biomarkers are clearly warranted, especially for early detection. An antibody-based screen of 2000 proteins was utilized to identify stool proteins that were elevated in patients with CRC, compared with healthy controls (HCs). Thirty-seven lead candidates were selected for ELISA validation in three independent cohorts comprised of CRC patients, advanced adenoma patients, and HCs, drawn from two different ethnicities. Pancreatic ductal adenocarcinoma (PDAC) is a fatal malignancy. Current conventional chemotherapeutics are inadequate in controlling the disease; hence, there is an urgent need for precision medicine. Ex vivo models that replicate the tumor and its microenvironment can advance precision medicine in PDAC. Patient‐derived organoids (PDOs) offer a promising solution by retaining the functional features of the tumor, allowing for individualized study of cancer biology and drug response. Background: Tuberous sclerosis complex (TSC) is an autosomal dominant genetic disorder characterized by the formation of hamartomas in the brain, kidney, and heart, along with other complex clinical manifestations, including TSC-associated neuropsychiatric disorder (TAND). Development of genotype-phenotype correlations within TSC can aid clinicians in providing prognostic data and improve clinical management. We present here a multigenerational family who has a pathogenic variant in TSC2 displaying a severe renal phenotype.

Background: People with dementia are often hospitalized from the emergency department (ED) for conditions that could potentially be treated in the outpatient setting. In scenarios of clinical equipoise-where either admission or discharge may be appropriate-little is known about how emergency physicians make disposition decisions.

Methods: We conducted a qualitative study with attending emergency physicians at a single academic medical center. This study introduces an induction heating approach for producing activated carbon from biochar obtained by thermal degradation of pine pruning waste from timber-related industries, using CO2 and iron-based catalysts as activation agents. Activation experiments were performed using a 10 kW high-frequency induction system (28–40 kHz) under continuous CO₂ flow (15 mL min−1), with electric current intensities of 200, 300, 400, and 500 A (corresponding to 700–1100°C) and durations of 30, 60, and 90 min, employing 45 g of iron-based catalysts (Fe–Ni–Ti–C, 85:6:3:6 wt.%) per 3 g biochar (15:1 w/w). Accurate prediction of the critical heat flux (CHF) is a key pillar for ensuring both the safety and economic performance of nuclear reactors. While machine learning (ML)-based methodologies have demonstrated considerable success in predicting CHF, most existing studies have been confined to simplified geometries, such as single tubes. Biodiesel is a promising, sustainable alternative to fossil fuels such as petrol and diesel. Currently, biodiesel can be produced from edible plant oils and non-edible sources and wastes. Notably, fish waste oil is a sustainable resource for transesterification reactions to produce biodiesel. This research proposes a general process design methodology to investigate the potential of biodiesel production from fish waste oil as a pathway for waste-to-energy. This study presents a Judicial Intersectionality method that incorporates knowledge from arts, humanities, natural sciences, and social sciences. It aims to examine how historical legal, political, and social injustices against Indigenous Australians have persisted in various spheres. When considering the Stolen Generations period, attention is directed on addressing the existing insufficient copyright rights and enhancing future legal results for Indigenous artists. Many employees lament that their inability to stop ruminating about work during nonwork time is one of their most significant work-related challenges, yet limited research has articulated actionable strategies to address this issue on a day-to-day basis. Integrating goal shielding theory with rumination theory, our model introduces nonwork goal reflection as a deliberate behavioral intervention employees can engage in to curtail work-related rumination, in a way that facilitates well-being during nonwork time. Expanding Public Health Insurance To Parents: Effect On Parents' And Children’S Well-Being, Michelle Marcus, Xuan Zhang

Expanding public health insurance to parents may not only improve parental outcomes but also have spillover effects on their children. This paper leverages the natural experiment of the Affordable Care Act (ACA) to estimate the causal effects of expanding public health insurance to low-income parents on their well-being and that of their children. Using a difference-in-differences model and data from the 2010–2017 National Health Interview Surveys, we observe significant improvements in health care access, increased health care utilization, reduced financial burdens, and slight improvements in health status among low-income parents. Standard realized volatility (RV) measures estimate the latent volatility of an asset price using high frequency data with no reference to how or where the estimate will subsequently be used. This paper presents methods for “tailoring” the estimate of volatility to the application in which it will be used. For example, if the volatility measure will be used in a specific parametric forecasting model, it may be possible to exploit that knowledge to construct a better measure of volatility. The SMU Trade Dashboard (https://economics.smu.edu.sg/soetrade/tr ade-dashboard), developed by the SMU Center for Research on International Trade, consists of Tariff Tracker and Trade Simulation functions. The Tariff Tracker allows users to track the tariff applicable to an HS 6-digit product or an ISIC 2-digit sector bymarkets and country origins. The Trade Simulation presents quantitative simulation results of the effects of Trump II tariffs (and the associated tariff responses by its trading partners) on trade flows, welfare, and wages. This companion paper documents the methodologies underlying the tariff data collection protocol and the structuralsimulation designs.

Besides mortality levels, policymakers and providers of financial and health services would also be interested in disability prevalence and its potential future trajectories. This paper presents the first study on high-dimensional regression coefficient tests with high-frequency financial data. These tests allow the number of regressors to be larger than the number of observations within each estimation block and can grow to infinity in asymptotics. In this paper, the sum-type test and max-type test have been proposed, where the former is suitable for the dense alternative (many small betas) and the latter is suitable for the sparse alternative (a very small number of large betas). We study a multi-period joint capacity allocation and job assignment problem. The goal is to simultaneously allocate resources across J different supply nodes and assign jobs from I different demand origins to these J supply nodes, so as to maximize the reward for matching or minimize the cost of failure to match. We consider three features: (i) supply is replenishable after some random time, (ii) demand is random, and (iii) demand can wait and needs not be fully fulfilled immediately. Background: The role of naltrexone in improving acute alcoholic pancreatitis (AAP) outcomes in alcohol use disorder (AUD) patients, remains unclear. Aim: We aimed to assess the temporal trends in AAP incidence and the impact of naltrexone use on clinical outcomes in AUD patients. Methods: We conducted a retrospective cohort study using de-identified patient data from the TriNetX database, including all adults (≥ 18 years) diagnosed with AUD. AAP patients were then identified and stratified by naltrexone exposure. The incidence and prevalence of AAP in AUD patients were assessed pre-(March 2015–2019) and post-COVID-19(March 2020–2025). This qualitative study used semi-structured interviews to examine the experiences of formerly incarcerated deaf people in the carceral system. The examination of lived experiences in this research offers a context to the belief that the carceral system is structurally unqualified to address the significant, but nuanced needs of deaf people. This study counters the neglect of the intersectional experiences of incarcerated deaf people that exists in research literature by providing them the opportunity to share their stories in their own words through in-depth semi-structured interviews. Yoga can support mental health, recovery, and provide positive coping strategies. Where language inequities impede access for Deaf individuals, interpreters are thought to facilitate communication. However, the percentage of interpreters trained in safe, effective, or non-triggering yoga interpretation is unknown. Using background and frequency data, this précis of a lengthy descriptive project highlights barriers to yoga programming for the Deaf community. A central finding is only 33% (n = 7) of interpreters had training to ensure emotional and physical safety to yoga participants. While results are assessed cautiously, lack of interpreter training may add to health and recovery programming inequities.

A longitudinal study investigated mental health diagnostic trends and patterns among Deaf and Hearing individuals using a comprehensive dataset from Alabama Department of Mental Health spanning 2004-2024 (Deaf=4,547; Hearing=2,056,188). Chi-square analysis revealed significant proportional differences in diagnoses between the groups. Deaf individuals showed significantly higher proportions of psychotic (12.93% vs. 8.33% Hearing), mood (20.76% vs. 18.84% Hearing), and personality (0.58% vs. 0.43% Hearing) disorders. These findings suggest that increased access to specialized and competent mental health services for individuals who are Deaf, particularly following the increase of staffing patterns for the Office of Deaf Services in 2015,contributed to more accurate …

Robust and efficient deep LiDAR odometry models are crucial for accurate localization and 3D reconstruction, but typically require extensive and diverse training data to adapt to diverse environments, leading to inefficiencies.
