We are in the midst of a global ecological crisis. At times, the scale of the suffering involved can be hard to fully comprehend. The whole planetary ecosystem feels out of kilter. The practice problem addressed was the lack of standardized orientation on the electronic health record (EHR) system for rotating students, which impacted documentation quality and workflow efficiency. Prior to the intervention, students received no formal training on the clinic’s EHR system, resulting in documentation errors, workflow disruptions, and dependence on administrative staff. Exploiting Chloride Conservative Tendencies As Contaminant Surrogates In Groundwater Transport Modeling In A Typical Hydrogeological Environment Of Northern New Jersey, Toritseju Oyen, Duke Ophori

This study investigates the transport of chloride, a conservative tracer and surrogate for contaminants, in the fractured Brunswick aquifer of northern New Jersey using a dual-porosity MODFLOW-MT3DMS model. Focusing on the First Watchung Mountain region—a microcosm of northern New Jersey’s hydrogeological environment encompassing Montclair State University and adjoining communities, the numerical model simulates groundwater flow and solute transport in a hydrogeologically complex, urbanized setting. Results indicate that chloride migrates through the fractured aquifer via both local flow systems (e.g., Third River) and regional flow systems (Passaic River) within decades. Advancements in robotics and computer vision are transforming how infrastructure is monitored and maintained. This paper presents a novel, fully automated pipeline for pavement condition assessment that integrates real-time image analysis with PCI (Pavement Condition Index) computation, which is specifically designed for deployment on mobile and robotic platforms. The rapid expansion of artificial intelligence is driving a sharp increase in U.S. electricity demand, with AI-driven data centers emerging as a central contributor through 2030. This paper asks how U.S. AI data centers can meet projected electricity needs while simultaneously reducing carbon emissions. Using a supply-chain and systems perspective, it analyzes the full energy chain of AI data centers: upstream electricity supply and grid deliverability, midstream facility design and grid interaction, and downstream operational practices, waste management, and disclosure. Principals play a crucial role in implementing MTSS/RtI (Multi-Tiered Systems of Support/Response to Intervention), yet little is known about their understanding of the RtI component. This quantitative survey examined Florida middle school principals’ actual and perceived knowledge of MTSS/RtI, as well as how training and demographic factors—such as special education certification, education level, Title I status, administrative experience during rollout years (2006–2010), district training hours, and college coursework—relate to that knowledge. Data from 54 principals were collected through perception scales, training background, and knowledge-based questions. CONTEXT: In animal models, Roux-en-Y gastric bypass (RYGB) is associated with increased Roux limb intestinal glucose uptake that may contribute to early metabolic benefits, though prospective clinical studies are lacking. OBJECTIVE: The present study aimed to test the hypothesis that Roux limb glucose uptake would increase relative to baseline in a cohort of patients undergoing RYGB. METHODS: RYGB patients underwent preoperative baseline, and 3- and 6-month positron emission tomography/computed tomography postoperative imaging. Poor nutrition is a major contributor to death in the United States, and residents of public housing face disproportionate challenges in maintaining adequate nutrition. Public housing is the nation’s oldest housing subsidy program, providing more than one million units for millions of residents, many of whom are women, minorities, and unmarried individuals with incomes below the poverty line. The purpose of this generic qualitative study was to explore public housing professionals’ perspectives on using a mobile food unit located within housing communities to help reduce food insecurity. Diabetes mellitus (DM) affects 12% of the United States population at an annual cost of $412.9 billion. In the local clinic, 20% of the patient population has a diagnosis of DM. Despite the importance of diabetes self-management education and support (DSMES) in managing DM, patients are often not educated on this important strategy. Leadership identified that patients were reporting a lack of knowledge regarding DSMES practices and that the nurses lacked the necessary knowledge and awareness to educate patients in DSMES. Background: Traditional East Asian medicine (TM) is widely used in Korea and other East Asian countries. However, the effects of TM treatment on acute ischemic stroke (AIS) outcomes remain unclear, as previous studies lacked a sufficient sample size, a consecutive series design, or a prospective outcome capture approach. We aimed to investigate whether combining TM with conventional Western medicine (CM) treatments (C+TM) leads to better outcomes after AIS, relative to CM treatment alone.

Tumor-Informed Minimal Residual Disease Testing In Select Solid Tumors And Hematologic Malignancies: A Narrative Review, Humza Mallick, Marcus Yoakam, Varun Karri, Samir Dalia

Tumor-informed circulating tumor DNA assays detect patient-specific cancer mutations in plasma and hold promise for detecting minimal residual disease (MRD) after or during definitive therapy. Recent large studies across solid tumors and hematologic malignancies suggest that tumor-informed MRD (TI-MRD) is strongly prognostic for relapse and survival. We reviewed disease evidence for these claims, as well as the potential utility in guiding therapeutic decisions. TI-MRD assays consistently achieved high analytical sensitivity. In solid tumors (colorectal cancer [CRC], non-small-cell lung cancer, breast cancer, and bladder cancer), multiple studies demonstrated that post-treatment MRD positivity conferred markedly worse recurrence-free and overall survival. Introduction: Intraoperative anaesthesia handoffs represent a risk point in the care of surgical patients. Although often necessary to prevent fatigue, improve vigilance and optimise operational efficiency, critical information can be lost, potentially leading to postoperative complications. Structured handoffs can increase the transfer of knowledge during intraoperative anaesthesia handoffs, improving their quality. We therefore propose to test the primary hypothesis that a semi-structured intraoperative anaesthesia handoff cognitive aid reduces the number of serious 30-day complications in surgical patients.

Guillain-Barré syndrome is an acute polyradiculoneuropathy in which preceding infections often elicit the production of antibodies that target peripheral nerve antigens, principally gangliosides. Anti-ganglioside antibodies are thought to play a key role in the clinical diversity of the disease and can be helpful in clinical practice. Extensive research into clinical associations of individual anti-ganglioside antibody specificities has been performed. Recent research has highlighted glycolipid complexes, glycolipid combinations that may alter antibody binding, as targets. Background: Primary hypertension in childhood tracks into adulthood and is associated with increased cardiovascular risk. Studies conducted in individuals aged < 18 years, an age group without many of the confounding comorbid cardiovascular disease risk factors in adults, provide an opportunity to explore early cardiovascular target-organ injury.

Methods: Youths (n=132, mean age, 15.8 years) were stratified by blood pressure (BP) as low-BP, mid-BP, and high-BP and by left ventricular mass index as low-and high left ventricular mass index. Systemic circulating RNA, microRNA, and methylation profiles in peripheral blood mononuclear cells and deep proteome profiles in serum were determined using high-throughput sequencing techniques. In vitro cell culture experiments assessed angiotensin II- and microRNA-mediated Vash1 (vasohibin-1 protein) regulation and Vash1-mediated hypertrophic response.

A range of leaching protocols have been used to measure the soluble fraction of aerosol trace elements worldwide, and therefore these measurements may not be directly comparable. This work presents the first large-scale international laboratory intercomparison study for aerosol trace element leaching protocols. Eight widely-used protocols are compared using 33 samples that were subdivided and distributed to all participants. Protocols used ultrapure water, ammonium acetate, or acetic acid (the so-called "Berger leach") as leaching solutions, although none of the protocols were identical to any other.
