Background/aim: Frailty is a clinical syndrome that affects individuals physically and psychosocially. However, the association between lumbar spinal stenosis (LSS) and frailty remains unclear. This study aimed to compare pain, balance, disability, fear of falling, and quality of life between patients with LSS with and without frailty.

This Doctor of Nursing Practice (DNP) project was a staff education initiative to address polypharmacy and increase medication adherence among older adults in a primary care setting. Polypharmacy is a prevalent and significant safety concern in older adults, increasing the risk for adverse drug events, medication nonadherence, hospitalizations, and decreased quality of life. The purpose of the project was to implement and evaluate a structured staff education focused on evidence-based strategies for medication review, deprescribing, and the integration of smartphone-based adherence tools. Objectives: This study aimed to investigate the association of two intronic polymorphisms of the ACE2 gene (rs2285666 and rs4646188) with coronary artery disease (CAD) in Iraqi patients with dyslipidaemia. Methods: This case-control study was conducted from March to September 2023 at the Iraqi Center of Heart Disease, Baghdad, Iraq, and included patients with CAD and dyslipidaemia. Genotyping was performed using TaqMan probes and real-time qPCR. Results: A total of 170 participants were included in this study. A 58-year-old female with a 15-year history of rheumatoid arthritis (RA) and a history of treatment consisting of methotrexate, hydroxychloroquine, leflunomide, and chronic prednisolone therapy, presented with fever, productive cough, dyspnea, and left leg cellulitis. She developed septic shock complicated by tension pneumothorax requiring emergency thoracostomy and non-ST-elevation myocardial infarction (NSTEMI). Pleural fluid culture grew Cupriavidus pauculus, a rare environmental gram-negative bacillus, alongside Burkholderia cepacia and Candida parapsilosis, indicating polymicrobial infection which was most likely predisposed by patient's immunosuppressed state from RA and prolonged steroid use. Treatment consisted of trimethoprim/sulfamethoxazole, meropenem, fluconazole, and cardiopulmonary support, and culminated in …

Background/aim: Extremity arterial injuries (EAIs) present a significant clinical challenge due to the risk of limb ischemia, amputation, and mortality. This retrospective cohort study aimed to delineate the clinical course, treatment strategies, and patient outcomes following trauma-induced EAIs, and to identify independent predictors of adverse outcomes.
Materials and methods: Retrospectively analyzed were data from 168 consecutive patients with traumatic EAIs who underwent computed tomography angiography at a tertiary care center between 2019 and 2025. In casino games using a standard deck of cards, a wash shuffle is sometimes performed prior to the rest of the card shuffling procedure. Unlike other methods of shuffling, the wash shuffle has not yet been well studied. To this end, we first develop a mechanistic model of the wash shuffle based on our observation of how cards tend to move when a wash shuffle is being performed. A lack of physician engagement in healthcare reform initiatives is a persistent problem that can negatively affect hospital performance and patient care outcomes. Hospital leaders and healthcare administrators are concerned with low levels of physician engagement because of its association with reduced organizational effectiveness and quality of care. Making decisions regarding data processing and analysis are crucial steps toward extracting insights from data in clinical trials. Trial registries like clinicaltrials.gov promote transparency about these decisions and encourage making them in advance. However, clinical studies often face decisions with multiple reasonable options outside the bounds of preregistration, such as when studies conduct post hoc analyses, deviate from preregistered plans, or simply were not preregistered. Additionally, even a priori decisions often have multiple reasonable options from which to choose. Methods that maximize transparency and minimize bias in such situations are needed. Primary hyperoxaluria type 1 (PH1) is a rare genetic disorder caused by hepatic oxalate overproduction due to alanine-glyoxylate aminotransferase (AGXT) deficiency. Therapeutic strategies targeting glycolate oxidase (GO) and lactate dehydrogenase A (LDHA), key enzymes in glyoxylate metabolism, have shown promise in reducing oxalate burden. However, recently approved siRNA therapies remain limited by high cost, unfavorable pharmacokinetics, and limited global accessibility. We report the development of compound 2, a dual GO/LDHA inhibitor (K i = 390 and 40 nM, respectively) that also promotes hydrophobic tag-mediated autophagic degradation of LDHA. Background/aim: The Oral Frailty Index-8 (OFI-8) is a reliable screening for oral frailty. In our study, while presenting the Turkish reliability and validity study of the OFI-8, we also aimed to establish a cut-off value for oral frailty in Turkish population.

Background/aim: Anti-interleukin-1 agents have known beneficial effects in the treatment of colchicine-resistant familial Mediterranean fever (cr-FMF); however, studies to date have tended to have small sample sizes and to be based on pooled data. Background/aim: The changes in proximal femoral blood flow occurring after reconstructive surgery for Crowe types III– IV developmental dysplasia of the hip (DDH) and their impact on healing remain unclear. Background/aim: The external oblique intercostal plane block (EOIB) is a regional anesthesia technique that may be utilized for the management of postoperative pain following laparoscopic radical nephrectomy. This study aims to evaluate the efficacy of EOIB after laparoscopic radical nephrectomy by comparing 24 h tramadol consumption between groups.
Materials and methods: Initially, 66 patients were enrolled in the study, and after exclusions, 60 were analyzed and divided into two groups: the EOIB group and the control group. Postoperative tramadol consumption, pain scores at rest and during movement, and dermatomal coverage of the block were recorded.

Background/aim: Breast cancer represents the foremost cause of cancer-related mortality among women worldwide and is characterized by a markedly high metastatic potential, contributing substantially to its clinical severity and poor prognosis. This study aimed to evaluate the interrelationship among silent information regulator (SIRT1), Forkhead box O3a (FoxO3a), and microRNA-34a (miR- 34a), the latter of which promotes apoptosis by suppressing the proliferation, migration, and invasion of breast cancer cells. Background/aim: Obesity impairs male fertility through metabolic dysfunction, oxidative stress, and disruption of the hypothalamic– pituitary–testicular (HPT) axis. Adropin (ADR), a peptide hormone whose circulating levels are reduced in obesity, plays emerging roles in metabolic homeostasis; however, its involvement in reproductive endocrine regulation remains unclear. The present study was conducted in healthy, nonobese male rats and aimed to investigate the neuroendocrine and testicular effects of exogenous ADR administration, focusing on circulating reproductive hormones, hypothalamic regulatory peptides, and testicular antioxidant pathways.

A 66-year-old man with type 2 diabetes mellitus and known gastroparesis presented to the emergency department with fatigue, confusion, and hypoglycemia. During hospitalization, he developed worsening abdominal pain and distention. CT revealed marked gastric distention, extensive portal venous gas, and mesenteric air, raising concern for emphysematous gastritis, gastric outlet obstruction, or volvulus. Despite these alarming radiographic findings, the patient remained hemodynamically stable, with no peritoneal signs. Surgical consultation was obtained; however, given his clinical stability and history of gastroparesis, conservative management was pursued. Treatment included nasogastric decompression, IV fluids, and prokinetic therapy with metoclopramide and erythromycin. Within 48 hours, the …

Therapeutic proteins face a critical pharmacokinetic challenge: rapid clearance from circulation limits their clinical efficacy. Albumin-binding domains (ABDs) offer an elegant solution by enabling therapeutic proteins to “hitchhike” on serum albumin’s favorable 19-day half-life through FcRn-mediated recycling. Clinical validation through approved therapeutics like ozoralizumab demonstrates the success of this approach, with preclinical studies showing fusion to an ABD extended half-life to 18 days. This review provides an analysis of ABD-fusion protein design, integrating structural biology, computational prediction, and rational engineering principles.
