Adjunctive Probiotic Use During Antibiotic Exposure In Very Preterm Infants: A Narrative And Translational Review, Mustafa Törehan Aslan, Mustafa Altındiş, Fahri Ovalı
Journal of Pediatric Infectious Diseases
Infants born before 32 weeks of gestation are highly susceptible to late-onset sepsis because of immune immaturity, impaired intestinal barrier function, and frequent broad-spectrum antibiotic exposure. Whether probiotics should be continued or initiated during antibiotic therapy remains unresolved and is often conflated with routine prophylaxis in stable infants. This SANRA-guided structured narrative and translational review synthesized clinical, microbiome, resistome, mechanistic, and safety evidence identified in PubMed/MEDLINE, Scopus, and Web of Science through July 1, 2026, across three contexts: stable antibiotic overlap, suspected sepsis, and confirmed or unstable sepsis. Pooled prophylactic analyses suggest a small, heterogeneous reduction in late-onset infection (risk …

Journal of Pediatric Infectious Diseases
Objective: This study investigated the age-dependent clinical characteristics of Kawasaki disease (KD) in pediatric patients and assessed the influence of age on disease presentation and outcomes. Methods: A retrospective analysis was conducted of 480 children diagnosed with KD who were admitted to Hangzhou First People's Hospital between 2013 and 2022. Subjects were stratified into two cohorts based on age: <2 years and 2-14 years). Demographic data, clinical manifestations, laboratory parameters, comorbidities, and coronary artery abnormalities were comprehensively evaluated across the age groups. Results: Time from symptom onset to diagnosis was significantly prolonged in the older cohort (z=-2.70, P=0.007). The incidence of cervical lymphadenopathy was markedly higher in older children (χ²=15.12, P<0.001), while peripheral extremity changes were significantly less frequent (χ²=5.79, P=0.016). The younger cohort exhibited significantly elevated platelet counts, alanine aminotransferase, and NT-proBNP levels compared to older patients (z=-3.57, P<0.001; z=-2.39, P=0.02; z=-3.59, P<0.001, respectively). Coronary artery dilatation and aneurysm formation occurred more frequently in younger patients (z=7.70, P=0.006; z=14.63, P<0.001). However, coronary artery recovery rates following intravenous immunoglobulin therapy showed no significant inter-group differences (P>0.05). Conclusions: The clinical presentation of KD demonstrates significant age-dependent variation. Clinicians should maintain a high index of suspicion for KD in older children presenting with fever and cervical lymphadenopathy. In younger patients with KD, close monitoring of hepatic function, platelet …2>

Al-Mustaqbal Journal of Pharmaceutical and Medical Sciences
Background: Chronic kidney disease (CKD) is a progressive disorder frequently complicated by anemia and a persistent inflammatory state, both of which significantly contribute to morbidity and mortality. Reduced renal production of erythropoietin (EPO) is the primary cause of anemia in CKD. Emerging evidence suggests that EPO may exert anti-inflammatory and metabolic effects beyond its hematopoietic role. This study aimed to compare serum EPO levels and inflammatory biomarkers in CKD patients treated with EPO versus untreated CKD controls.
Material and Methods: This cross-sectional analytical study was conducted on 60 adult patients with CKD undergoing regular hemodialysis. Participants were divided into two …

Computational Design, Molecular Docking, Sar, And Admet Analysis Of Novel Jak1 Inhibitors As A Possible Treatment For Rheumatoid Arthritis, Sarah Ahmed Waheed
Al-Mustaqbal Journal of Pharmaceutical and Medical Sciences
Rheumatoid arthritis is an inflammatory autoimmune disease affecting the joints which eventually leads to systemic comorbidities. Its pathogenesis was attributed to disturbances in the Janus Kinase/Signal Transducer and Activator of Transcription signaling pathway. In this research, an innovative lead compound C6 was created and docked toward Janus Kinase1 (JAK1). The docking process was confirmed by re-docking of the co-crystallized ligand MI1 (Tofacitinib). C6 showed a predicted docking score of -9.17 kcal/mol, with enhancement of 1.26 kcal/mol over Tofacitinib. Evaluation of the interactions showed a framework of five hydrogen bonds within the binding site involving an important contact with Glu966 as …
Designing High-Resilience Hybrid Metamaterials: A Systems Engineering Specification For Elemental Hybridization, Solitonic Stabilization, And Non-Destructive Process Verification In Artificial Crystal Lattices, Christopher L. Eckes
Defensive Publications Series
Traditional artificial crystal lattices grown via high-mass commercial methods exhibit acute failure modes under extreme operational environments. These loops manifest as high-frequency acoustic micro-cracking along crystal grain boundaries during cyclic load profiles, or structural delamination driven by thermal expansion differentials across thin-film interfaces.
This specification introduces a formalized systems engineering framework for the elemental hybridization of lab-grown crystal substrates—specifically Thin-Film Lithium Niobate (LiNbO₃) and 3D Self-Assembled Inverse Opal Lattices. By strategically infiltrating these substrates with low-weight-fraction Graphene Oxide (GO) nanosheets and Boron Nitride Nano-Tubes (BNNTs), we create hybrid, non-linear metamaterials that isolate, dampen, and redirect destructive kinetic and thermal wave …

Acquired brain injury (ABI) is the leading cause of adulthood injury-related death and disability worldwide. Despite ABI’s prevalence, little is known about the extent to which social workers are prepared for brain injury working and specifically pre-qualifying brain injury curricula. This study employed a nine-item survey regarding brain injury social work knowledge. The survey asked about curricula inclusion within social work pre-qualification via multiple education providers to understand the extent to which brain injury training is incorporated within UK courses. This included a series of open-ended questions. Data from 36 survey respondents identified that 1) there was an acknowledged gap …

Abiotic stress conditions impair plant metabolism partly by inducing the excessive production of reactive oxygen species (ROS). Melatonin, a multifunctional indoleamine, has emerged as an important regulator of plant responses to oxidative stress. This review highlights recent findings on the interactions among phytomelatonin, ROS homeostasis, antioxidant metabolism, photosynthetic protection, stress priming, and abiotic stress tolerance. We discuss how melatonin modulates the antioxidative response under diverse stress conditions and evaluate its effects on photosynthetic performance, ROS signaling, and membrane stability. Special attention is given to priming studies showing that melatonin pretreatment can improve stress tolerance. Emerging evidence from plant tissue culture …

Drought Priming Improves Waterlogging Tolerance In Alfalfa, But Less Effectively Than Waterlogging Priming, Gürkan Demi̇rkol, Ni̇lüfer Kara
Turkish Journal of Botany
Waterlogging severely restricts growth and productivity in alfalfa (Medicago sativa L.), necessitating effective strategies to improve stress tolerance. This study investigated whether drought priming or waterlogging priming could enhance tolerance to subsequent waterlogging stress in the alfalfa cultivar “Bilensoy-80”. Plants were subjected to drought priming, waterlogging priming, or no priming prior to progressive waterlogging stress for 0, 4, 8, 12, 16, and 20 days. Waterlogging priming consistently alleviated growth inhibition, maintained chlorophyll content, and reduced oxidative damage under prolonged stress. Drought priming was associated with partial cross-tolerance, particularly through improved root growth, plant water status, and oxidative stress responses, although …

Red Light Accelerates Adventitious Root Primordium Initiation And Differentiation In Phoebe Bournei Cuttings By Modulating Auxin Signaling And Hormonal Balance, Yuewang Niu, Yuanyang Bi, Chenyu Gong, Shipin Chen
Turkish Journal of Botany
Adventitious root formation in stem cuttings of Phoebe bournei, an economically and ecologically significant woody species, is a major bottleneck in clonal propagation. To elucidate the impact of spectral quality on early adventitious root development, we characterized the morphological, hormonal, and transcriptomic dynamics of primordium initiation and differentiation under white, red, and blue light. Under red light, cuttings exhibited the earliest and most pronounced formation of white filamentous structures at lenticels, indicative of root primordium initiation, as early as 20 days after cutting, whereas white and blue light treatments showed delayed responses. Paraffin section analysis confirmed the induced origin of …

Death: The Great Antagonist In Poe’S “The Cask Of Amontillado”, Majed S. Al-Lehaibi
Journal of Arts and Social Sciences
Poe’s story “The Cask of Amontillado” has been read as a story of a premeditated crime, of revenge and the protagonist’s ultimate pride in his well-executed crime. Readers are repulsed by the sheer evil of Montresor, the main character of the story. However, this paper argues that this reading barely scratches the surface of the depth and complexity of this story. It argues that the story is told in the form of a confession. The narrator, an elderly Montresor, is full of remorse, and in telling the story, he is seeking absolution. In its deeper meaning, “The Cask of Amontillado” …

Sex-specific penetrance in autosomal-dominant Mendelian conditions is largely understudied. The neurodevelopmental disorder Pilarowski-Bjornsson syndrome (PILBOS) was initially described in females. Here, we describe the clinical and genetic characteristics of the largest PILBOS cohort to date, showing that both sexes can exhibit PILBOS features, although males are overrepresented. A mouse model carrying a human-derived Chd1 missense variant (Chd1R616Q/+) displays female-restricted phenotypes, including growth deficiency, anxiety, and hypotonia. Orchiectomy unmasks a growth-deficiency phenotype in male Chd1R616Q/+ mice, while testosterone rescues the phenotype in females, implicating androgens in phenotype modulation. In the gnomAD and UK Biobank databases, rare missense variants …

Centralized cloud-based approaches to anomaly detection are hindered by their inherent latency and privacy issues while independent or stand-alone approaches to edge-based anomaly detection do not have enough data to effectively detect anomalies. This paper presents a federated learning framework to collaboratively develop an anomaly detection model from multiple edge nodes, without sharing the raw sensor data, so that data sovereignty is preserved. Two major contributions …

We study a time-inhomogeneous nonlinear SDE with drift and diffusion governed by state-dependent variable exponents. This framework generalizes models like the geometric Brownian motion (GBM) and the constant elasticity of variance (CEV), offering flexibility to capture complex dynamics while posing analytical challenges. Using a fixed-point approach, we prove existence and uniqueness, analyze higher-order moments, derive asymptotic estimates, and assess stability. Finally, we illustrate an application where Poisson’s equation admits a probabilistic representation via a timehomogeneous nonlinear SDE with state-dependent variable exponents.

Introduction: Despite the widespread use of non-invasive ventilation (NIV) in pediatric intensive care units (PICUs), little has been published regarding enteral nutrition (EN) in this setting. This pre-planned secondary analysis of the Association of Enteral Nutrition and Dexmedetomidine Usage for Critically Ill Children Requiring Non-Invasive Ventilation study aimed to describe current feeding practices during NIV in a large sample, including differences in timing, energy and protein delivery across NIV modes and interfaces. Methods: Secondary analysis of a retrospective study of 1763 patients acutely supported with NIV over one year in fourteen PICUs in the United States. Results: 1065 subjects (60.4%) …

Considering that neonatal pain is a relevant matter given its implications and consequences, many neonates, particularly those undergoing critical care, are subjected to analgo-sedation therapies, which commonly include treatment with opioids and benzodiazepines. These drugs, however, can induce tolerance and dependency, leading to the development of the so-called iatrogenic withdrawal syndrome (IWS) which is observed following discontinuation of these therapeutic agents particularly when utilized for a prolonged period of time (>= 5 days). IWS consists of signs and symptoms manifested especially in young children, such as term and premature newborns in the neonatal intensive care unit (NICU), who are …

The increasing urgency of climate change, biodiversity loss, and socio-economic transition has generated growing interest in tourism approaches that move beyond sustainability towards more transformative forms of development. While sustainable tourism has become the dominant paradigm within tourism planning and policy, it has been criticised for its emphasis on impact mitigation and its limited capacity to address systemic challenges. In response, regenerative tourism has emerged as an alternative framework that seeks to restore and enhance ecological, social, cultural, and economic systems through tourism activity.
This paper explores the potential of regenerative tourism as a framework for transformative destination management. Drawing …
