Background: Annually in the United States approximately 750,000 children with special health care needs transition from pediatric to adult health care. Attention deficit hyperactivity disorder (ADHD) is one of the most prevalent neurodevelopmental disorders and nearly two thirds of children who have symptoms of ADHD in childhood have symptoms that persist to adulthood (Inman et al., 2017). The Child Development Center at a large medical center in northern New Jersey, prior to this project, did not have a transition process or procedure in place to aid providers to successfully prepare adolescent patients with ADHD for transition to adult care. Research on the intersection of copyright law, research activities, and innovation in Latin America reveals significant deficiencies in the legal framework and public discourse surrounding copyright limitations and exceptions (L&Es) for research purposes. The problem is more visible with modern data assembling and analyzing techniques, such as through artificial intelligence (AI) systems. This study highlights the scarcity of explicit provisions for research-related exceptions in the region by examining copyright legislation, case law, and stakeholder perspectives in six Latin American countries. Chapter 1 discusses the development of antagonists against the dopamine 4 receptor (D4R) that is expressed in the motor, associative, and limbic subdivisions of the basal ganglia network and is involved in Parkinson’s disease (PD). Levodopa is the firstline drug for the management of PD symptoms but its long-term use often leads to development of levodopa-induced dyskinesia (LID). D4R antagonists have been shown to decrease the abnormal involuntary movement scores in mouse models of LID. Military spouses in Okinawa experienced denial of care at military treatment facilities and off base Japanese healthcare facilities during the COVID-19 pandemic. Over two years of travel restrictions further contributed to difficulties accessing healthcare while stationed on the island with their active-duty spouse. This study examined the implications of a priority-based access system through the TRICARE program implemented due to limited resources during COVID-19 and its impact on military spouses in a scarce medical resource environment. F. tularensis employs various defense mechanisms to survive and persist under diverse environmental and host-induced stress conditions. Background: Social Health Programming (SHP) is a holistic intervention that can empower communities to address modifiable risk factors for poor cardiovascular health, including suboptimal nutrition or psychosocial stressors. SHP participation has been shown to increase dietary intake of fruits and vegetables, decrease perceived stress, and improve cardiovascular endurance. However, it is unknown how SHP participation during adolescence affects carotenoid serum levels, hair cortisol concentrations (biological marker of chronic stress), or vascular reactivity index (VRI; measure of subclinical cardiovascular health).

However, limited access to the viral reservoir has significantly hindered its effectiveness in fully eradicating HIV. Therefore, to improve HIV therapy, here we propose a C-C chemokine receptor type 5 (CCR5)-targeted theranostic lipid Based Nanoparticles (LBNPs) to noninvasively track and specifically deliver the antiretroviral drugs (ARVs) to the viral reservoir. Children born preterm are at an elevated likelihood for autism (ELA) and intellectual disabilities (ID). Event-related Potentials (ERPs), such as the P1 and the Mismatch Negativity (MMN), can be used to measure underlying neurophysiology, which may aid in the identification of these important neurodevelopmental and cognitive outcomes. Specifically, enhanced P1 and MMN discrimination, measured via the difference between two alternating sounds, is associated with autism while reduced neurophysiological habituation is associated with a lower IQ. One important cell signaling pathway is the PI3K pathway because it influences many key cellular functions such as proliferation and apoptosis. Among the enzymes that are involved in this pathway, the SH2-containing inositol-5’-phosphatase (SHIP) plays an important role by maintaining the balance of PI(3,4,5)P3 and PI(3,4)P2. SHIP activation promotes a higher rate of hydrolysis of PI(3,4,5)P3 and thus reduces the downstream activation of AKT, which influences many cell processes. The purpose of this thesis project was to 1) examine trajectories of multidimensional cancer-related fatigue (CRF) in men with prostate cancer during and after radiation treatment completion, and 2) identify biobehavioral correlates (demographic, clinical, co-occurring symptoms, and biologic [inflammatory cytokines]) for the different CRF dimensions.

Data for this secondary analysis were obtained from a descriptive, longitudinal IRB-approved study investigating men with non-metastatic prostate cancer receiving radiation therapy (RT). Data were obtained from 68 participant medical records, self-report questionnaires, and blood samples. This dissertation seeks to understand the sociocultural contexts surrounding the do-it-yourself (DIY) use of transcranial direct current stimulation (tDCS) devices. TDCS is a non-invasive method of brain stimulation which has a history of use in (1) human brain mapping for basic neuroscience studies, (2) neuropsychological rehabilitation and the treatment of psychological disorders in clinical populations, and (3) cognitive enhancement in healthy people. The earned income tax credit (EITC) is currently the largest means-tested antipoverty program in the United States that assists low-income working families surviving along the edges of poverty. Within Emergency Medicine, older adults represent a distinct population with unique needs and challenges. There is a scarcity of epidemiologic data on the requirements of Ireland’s older population for, and their utilisation of, emergency care networks. We sought to assess this deficit utilising data from the recently published European Geriatric Emergency Departments Registry Study (EGERS) dataset.

INTRODUCTION: The utility of pancreaticoduodenectomy (PD) for high-grade traumatic injuries remains unclear and data surrounding its use are limited. We hypothesized that PD does not result in improved outcomes when compared with non-PD surgical management of grade IV-V pancreaticoduodenal injuries.

METHODS: This is a retrospective, multicenter analysis from 35 level 1 trauma centers from January 2010 to December 2020. Included patients were ≥15 years of age with the American Association for the Surgery of Trauma grade IV-V duodenal and/or pancreatic injuries. The study compared operative repair strategy: PD versus non-PD.
