Introduction: Burnout among physicians is a worldwide burden. While many causes of physician stress have been reported, we have found few quantitative studies of associations between burnout and participation in hobbies and interests outside of medicine. Our objective was to determine if health care professional burnout/disengagement could be mitigated by incorporating leisure interests and to characterize which specific interests, if any, are most significantly related.
Methods: We conducted an online survey of 2,563 US-based physicians and 512 residents/fellows and queried their participation in a list of 117 individual hobbies, which we then further categorized into three perceived levels …

Problem: Each year, thousands of patients sustain falls while hospitalized, resulting in an increase in serious injuries or death. In a Neuroscience Step Down Unit (NSDU), fall rates in 2022 were higher than the NDNQI benchmark. Despite a thorough fall prevention policy, nurses inconsistently implemented preventative interventions, resulting in increased falls.
Context: The NSDU is a 15-bed inpatient unit providing intermediate-level care for patients with neurological and neurosurgical diagnoses. The unit is part of a 459-bed academic medical center and safety net hospital providing tertiary and quaternary care in southern California.

Practice Problem: Falls among geriatrics continue to be a significant concern for healthcare organizations and one of the costliest healthcare problems attributed to increased morbidity and mortality rates. Falls rates in residential dwellings such as nursing homes and community living centers (CLCs) continue to rise despite fall prevention efforts.

The objective of this study was to use a Balanced Centricity (BC) approach to describe how personal values related to experienced quality of care are defined for residents, family and nursing staff in nursing homes. Capturing the values from a multi-stakeholder perspective, a qualitative approach was conducted in which the stakeholders’ (e.g., residents, family members of residents and nursing staff; a triad) experiences, opinions and values were explored. This literature review focuses on the complexities and inequalities of informal caregiving in the UK and was inspired by the story of the following individual: EL is a 68-year-old Caucasian lady who attended Movement Disorder Clinic and was diagnosed with Parkinson's Disease Dementia following many years of symptoms. The diagnosis came as a big relief to EL and her daughter (SL) who were eager to get treatment started as soon as possible. EL lives alone with SL and solely relies on her for care and support. Mokken Scale Analysis (MSA) can offer a more flexible approach to common Item Response Theories (IRT) as a non-parametric method for evaluating educational tests with ordinal data. This study applied MSA on a Vocabulary Levels Test (VLT) originally developed with IRT methods among 234 English as a Foreign Language (EFL) students to test the assumptions of MSA, including unidimensionality, monotonicity, local independence, and invariant item ordering (IIO) with mokken package in R. MSA assumptions successfully met with the VLT validated with validating the VLT being a unidimensional test with Automated Item Selection Procedures (AISP) and Genetic Algorithm (GA) methods utilized. …

Later-Life divorce, occurring at age 50 or older, also referred to as gray divorce, has become increasingly common as the population ages (Brown & Lin, 2012; 2022). With gray divorce comes experiences that may differ from those faced by younger divorcees. The research demonstrates that divorce in laterlife brings unique challenges, especially for those who divorce after a long-term marriage (20 or more years, Sommerville, 2017). BACKGROUND AND OBJECTIVE: Pituitary tumor in patients induces adverse alterations in the brain, accompanied by cognitive deficits. Dysfunction of glymphatic waste clearance results in accumulation of neurotoxic products within the brain, leading to cognitive impairment. However, the status of glymphatic function in the brain with pituitary tumor is unknown. Using magnetic resonance imaging (MRI) and an advanced mathematical modeling, we investigated the changes of glymphatic transport in the rats carrying spontaneous pituitary tumor.

Purpose: The present study explored current speech-language pathology service provision for aging individuals with intellectual disabilities in the United States, including areas and domains of services, factors restricting service provision, and reported knowledge and confidence of individuals within the field of speech-language pathology in providing services, to identify possible gaps of service and needed improvement.

Scientific advancements have led to significant improvements in cancer treatment and increased survival rates. However, cancer survivors often experience long-term effects, including cognitive impairments known as cancer-related cognitive impairment (CRCI). CRCI is a common and distressing side effect that affects affected individuals’ quality of life. The discipline of speech-language pathology (SLP) is well-positioned to assess and treat CRCI. This paper focuses on the establishment of an Alzheimer’s and dementia library outreach program in Nebraska, initiated by the Nebraska Chapter of the Alzheimer’s Association. The program aims to address the lack of community resources and support for individuals affected by dementia-related diseases in Nebraska. Extensive research, including interviews with successful library outreach programs globally, informed the program's foundation. A survey distributed to Nebraska libraries had a stellar response rate, with 57 libraries expressing interest in collaborating. Significance and Background: The skin is the largest organ of our body, and skin injury prevention begins with accurately inspecting skin. Current practices stress on looking for any signs of redness which often signify that the skin is possibly injured or compromised. However, darker skin tones do not appear red when injured. Assessing for redness is a practice that is suited better for lighter skin tones.

A basic knowledge of mathematics is required for many social and economic opportunities. Thus, understanding the variables related to children’s math learning is of utmost importance. This study explores the role of cognitive reflection on children’s mathematics learning. Cognitive reflection is the tendency to reflect on and override an inaccurate gut response. Only recently has a measure of cognitive reflection been available for use with school-aged children. FOXO transcription factors perform diverse functions that have effects on significant biological processes like stem cell homeostasis, cell cycle, apoptosis, and cell fate determination. This research focuses on FOXO4 that is differentially localized in U87MG glioblastoma cells where FOXO4 is targeted to the nucleus. This research is aimed at investigating the mechanism and localization of FOXO4 in U87MG cell lines. Unmarried women over the age of 60 continue to experience disproportionate rates of adult poverty in the United States, while families headed by single mothers experience the highest poverty rates. This study explores the long-term impact of single motherhood on financial wellness through the perspective of Hispanic/Latina women ages 60 and over who have experienced single motherhood in Massachusetts. This thesis investigates the associations between household social, economic, and physical environment conditions and the trajectory of self-reported functional limitations over time among middle-aged and older adults in China. Mitochondrial disorders are complex diseases that involve multiple systems and result from mutations and deletions in the mitochondrial genome. Leigh syndrome (LS) is a rare and devastating primary mitochondrial disorder characterized by progressive neuromuscular degeneration. Despite its severe consequences, the clinical genotype pathophysiological underlying LS remains elusive, posing significant challenges for the development of effective therapies. The current study aims to advance our understanding of the pathology and phenotype correlation of LS by evaluating mitochondrial dynamics and bioenergetics in patient-derived human induced pluripotent stem cells (hiPSCs) and their differentiated derivatives, which serve as a reliable model of the disease. 香港的老年人口增長迅速,從1988年至2018年間,老年人口由46萬增加到127萬(政府經濟顧問辦公室, 2019)。隨著人口老化,長期護理服務需求也相對增加。資助安老院舍的平均等待時間長達20個月(香港政府, 2021)。在2017至2018年間,有40,000人在輪候資助安老院舍,其中6,600人在等候期間去世(香港政府, 2018)。安老服務業一直面臨人力短缺的問題,2017年的相關職位空缺率為15-20%(香港政府, 2017)。2013年的研究統計發現超過一半的安老服務前線員工超過50歲(勞工及福利局, 2019)。隨著退休潮的來臨,人力短缺的問題會進一步加劇。因此,科技與安老服務的結合被視爲減輕人手壓力的方向。結合科技和院舍服務不僅可以減輕工作量,還能提高服務質量和效率。社交媒體和視訊會議平台可以減少在訊息傳遞上的時間和人力成本。樂齡科技產品,諸如可穿戴設備和遠程監控傳感器,可以讓護理人員更快捷和系統化地掌握服務對象的狀態,從而減少親自監測的次數(Chen, 2020)。因此,自2017年起,香港政府與各部門積極推廣樂齡科技,並於2018年設立樂齡及康復創科應用基金,幫助合資格的安老及康復服務單位購置、租借和試用科技產品。香港政府於2022年更將此計劃擴展至其他非資助的私營或自負盈虧的安老院舍及殘疾人士院舍。在過去三年的新冠疫情影響下,大部分安老院舍因為防疫工作量的增加而面臨更嚴重的人手短缺,不少院舍開始將科技融入服務,以應對疫情衍生的需求和改善現有服務質素。然而,香港院舍對於如何應用社交媒體、會議平台和樂齡科技產品的研究甚少。在此背景下,本研究旨在瞭解院舍應用相關科技的情況,並提供應對有關障礙的政策建議。
