Cluster Randomised Controlled Trials involve randomising groups of participants, rather than the individual participants themselves, whilst the outcomes are measured on the participants. Whilst there are a number of practical and methodological advantages to such a design, there are also statistical implications, both in terms of study design and sample size calculation, and in analysis. The methodology underpinning the cluster randomised design is now well-established in the statistical literature. Background: Migration and homing are essential to B-lymphocyte mediated immunity, and are driven by rapid, directed, and appropriate reorganisation of the actin cytoskeleton. Important observations have linked the cytoskeletal-rearrangements made by leukaemic B-lymphocytes of chronic lymphocytic leukaemia (CLL) to disease pathology. In particular, cytoskeletal alterations mediated by B-cell receptor (BCR) engagement or chemokine-binding are recognised to be central to the migration of CLL cells to lymphoid tissues, where they engage in the complex cellular and molecular interactions that underlie their survival, proliferation, and drug resistance. Dietary nitrate lowers the oxygen cost of submaximal exercise, but precise mechanistic insight into how this occurs is lacking. Research suggests that dietary nitrate may render oxidative ATP synthesis more efficient, but evidence is inconclusive at present. This thesis aimed to establish how nitrite (a reduced form of nitrate) affects the bioenergetics of cultured skeletal muscle cells. Comparison between the acute effects of nitrite and insulin, a hormonal regulator of muscle function that increases mitochondrial efficiency, was explored to assess possible mechanistic overlap. Calculation of real-time intracellular ATP synthesis rates from simultaneous oxygen consumption and medium acidification measurements revealed the …

Background: Three decades of clinical research have demonstrated that the risks of atherosclerotic cardiovascular disease (ACVD: coronary disease and ischaemic stroke), and the closely related type 2 diabetes (T2D) are established and influenced by events during childhood. This is at least partly because childhood obesity usually leads to adult adiposity; so, the currently high prevalence of adult cardiometabolic disease, means that obesity needs to be identified as early as possible, ideally in childhood. Screening for obesity largely relies on anthropometric measurements like body mass index (BMI), however this has many limitations including its inability to distinguish lean from fat mass. …

This thesis aimed to investigate in detail the processes underlying the control of recall accuracy. It is believed that recall accuracy control comprises processes occurring at memory search and post-retrieval. There is a large body of research concerning post-retrieval monitoring processes; however, less is known about how memory search is constrained and what factors affect this process.Chapter 2 developed and tested a new paradigm for measuring the accuracy of constrained search and monitoring processes simultaneously. A diagnosis of dementia should not mean the end of a person’s ability to participate in meaningful activities. However, accessibility for people living with dementia requires improvement across a range of settings. Anecdotal evidence and critical incident reporting suggests that air travel is not meeting the needs of people with dementia, although evidence-based research into air travel and dementia is extremely limited. Atrial fibrillation (AF) is the most common arrhythmia encountered in clinical practice accounting for approximately one-third of hospitalizations for cardiac rhythm disturbances and is associated with increased risk for stroke, heart failure and death. The epidemiology and clinical features of AF have primarily been characterized in whites of European-descent. The world literature on the epidemiology of AF among different ethnicities is very limited with most of the published reports of AF in non-white ethnicities focusing mainly on black (African-American) patients. This thesis explores how an understanding of an immaterial dimension of human experience was expressed and experienced in the context of nineteenth century engagement with science and technology. Drawing upon methods of media archaeology, it presents examples of visual technologies where apparatus and images representative of scientific and technological objectivity appear to have been appropriated and modified for the purpose of experiencing this dimension, an experience this thesis calls enchantment. The thesis suggests that some period specific behaviours associated with these visual technologies are sophisticated and subtle mechanisms that contribute to the affective and cognitive experience of enchantment. This practice-led research follows my exploration of the Sierra Negra astronomical observatories in Mexico, as well as other sites and temporalities where my practice has drifted: it flows from the heights of two volcanos, across Idaho during a total eclipse of the sun, and into the waters of the Usumacinta River. This thesis consists of an interdisciplinary research on an aesthetic particularism mentioned by anthropologist Claude Levi-Strauss as “The split representation in the Art of Asia and America” in his 1944 eponymous essay. According to the Centers for Disease Control and Prevention, more than 13.7 million children experience obesity nationally and Hispanic children display the highest occurrences of obesity for all racial/ethnic groups (Hales et al., 2020). Because parents have a significant influence on their children’s overall health, we were interested in examining whether parents’ attitudes and knowledge about obesity impacted their child’s body mass index (BMI) for their age. Objective: Little research has focused on the drinking issues at minority serving institutions. The purpose of this study was to examine the relationships among personal psychological distress, mental well-being, self-esteem, and alcohol-related problems. Participants: 360 undergraduate students were recruited at a public university in the Southwest US. Methods: A cross-sectional design survey was conducted. One-way ANOVA and multiple regression analyses were utilized. Results: Male students experienced more alcohol-related problems than female students. Statistically significant correlations were found among life satisfaction and loneliness, life satisfaction and self-esteem, life satisfaction and alcohol-related problems, and self-esteem and loneliness. Superficial cutaneous injuries occur on a huge scale across the globe; while most will resolve without intervention or any lasting damage, many will experience complications that arise through delayed healing or as a consequence of the skins protective barrier being breached. This thesis aimed to critically synthesise the relationships between cultural capital and perceptions of authenticity in literary tourism experiences. Whilst authenticity is a central theme in literary tourism research, few studies have explored authenticity and Bourdieusian cultural capital concurrently. Thus, this thesis investigates the two concepts and proposes new theory which elicits an in-depth understanding of individual perceptions of authenticity. The study adopted an interpretivist qualitative approach, using an ethnographic research strategy. Ethnographic fieldwork was conducted in Cornwall, UK. Teacher educators in the United States often struggle to develop preservice teachers' cultural and global competence. Background: As a normative theory, care ethics has become widely theorized and accepted. However, there remains a lack of clarity in relation to its use in practice, and a care ethics framework for practice. Background: A core aspect of midwifery philosophy is the optimisation of normal physiology; however, this has been challenged as a radical idea in the medicalisation of birth. Research has demonstrated the benefits of midwifery in improving outcomes for both mothers and babies. The understanding of midwifery benefits fails to reach wider sociocultural contexts as births becomes more medicalised. Midwifery research requires an action arm, to help translate theory to practice and mobilise midwives in solidarity with women towards action and change.
