Inpatient Hospital Performance Is Associated With Post-Discharge Sepsis Mortality., Nicholas M. Mohr, Alexis M. Zebrowski, David F. Gaieski, David G. Buckler, Brendan G. Carr

BACKGROUND: Post-discharge deaths are common in patients hospitalized for sepsis, but the drivers of post-discharge deaths are unclear. The objective of this study was to test the hypothesis that hospitals with high risk-adjusted inpatient sepsis mortality also have high post-discharge mortality, readmissions, and discharge to nursing homes.
METHODS: Retrospective cohort study of age-qualifying Medicare beneficiaries with sepsis hospitalization between January 2013 and December 2014. Hospital survivors were followed for 180-days post-discharge, and mortality, readmissions, and new admission to skilled nursing facility were measured. Inpatient hospital-specific sepsis risk-adjusted mortality ratio (observed: expected) was the primary exposure.

BACKGROUND: Patient-Centered Medical Home (PCMH) adoption is an important strategy to help improve primary care quality within Health Resources and Service Administration (HRSA) community health centers (CHC), but evidence of its effect thus far remains mixed. A limitation of previous evaluations has been the inability to account for the proportion of CHC delivery sites that are designated medical homes.

Background Diet and inflammation have both been studied in relation to type 2 diabetes mellitus (T2DM). The aim of this cross-sectional study was to examine the association between the Dietary Inflammatory Index (DII®) and T2DM. Methods Subjects were adults enrolled in the baseline study of the Xinjiang multi-ethnic natural population cohort and health follow-up study from January to May 2019. The study involved 5,105 subjects (58.7% men) between 35 and 74 years of age. The DII score was calculated from a data obtained via a food frequency questionnaire consisting of 127 food items. COVID-19 is a novel virus with lethal effects on human health, causing respiratory failure and infiltration in the lungs. The present study attempted to examine global publication trends in research associated with COVID-19 and sought the fitness of Lotka's Law, data downloaded from the WHO database (https://www.who.int/). The search term "2019nCoV, COVID -19" was used to retrieve articles published from December 2019 to March 19, 2020. Two thousand two hundred eighteen authors have contributed a total of 2250 articles. In the early days of the COVID-19 pandemic, experts warned of the unique and devastating havoc that the novel illness could wreak on people experiencing homelessness, an already vulnerable population. While reports thus far suggest that neither the prevalence nor mortality of COVID-19 among people who are homeless has been as severe as feared, the pandemic has brought about opportunities to enact innovative and long overdue approaches to the issue of homelessness. The sacoglossan sea slug, Elysia papillosa is associated with two species of the siphonaceous green alga, Penicillus, which co-occur in mixed macrophyte beds at Tarpon Springs, FL, USA. Field collections revealed that Elysia papillosa is frequently collected from the alga, P. capitatus, but is also present in much lower abundance on P. lamourouxii. Past studies have shown that some species of sacoglossans consume algal species that differ from the algae from which they are collected so, in order to determine whether E. papillosa was consuming either or both species of Penicillus, total DNA was extracted from individual slugs collected from …

Using transverse cross-sectional transects, a survey of 31 km of the non-tidal Potomac River was conducted from White’s Ferry, Virginia to Brunswick, Maryland, USA, between June and September in 2013 through 2015 to assess a recurring benthic cyanobacteria bloom. Abundant benthic cyanobacteria blooms were detected during the 2014 and 2015 sampling seasons and the primary taxon was identified morphologically and molecularly as Planktothrix cf. isothrix. When present, P. cf. isothrix blooms were concentrated from river center to the Maryland shoreline. This pattern was correlated with significantly greater benthic chlorophyll-a and phycocyanin concentrations. In an apparent response to …

Introduction: Surgical site infections (SSIs) account for 14-16% of nosocomial infections and are one of the major causes of increased morbidity, hospital stay, cost of care, and even mortality. Hypothermia as a risk factor for SSI is debated but there is lack of conclusive evidence. The present study explores the association of hypothermia with SSI.
Methodology: This is a prospective cohort study conducted on adult patients who underwent elective laparotomy. The type of exercise is a relevant resistance training-variable that might be manipulated in order to induce significant increases in muscle strength. The aim of this study was to analyze the influence of multi-joint vs single-joint resistance exercises on maximal strength. International Journal of Exercise Science 13(3): 1638-1649, 2020. The aims of this study were: (i) to analyze the relationship between the performance of different measures of muscular strength, and (ii) to identify which measurements present a greater relationship with an overall strength score. Sixty older women (aged 69 ± 6 years) were submitted to muscular strength measurements from isotonic, isokinetic, and isometric tests. An overall-strength score was generated with z-scores of the values obtained in all tests. Interquartile intervals were created for each measure and the overall-strength score. Pearson’s r (0.463-0.951, p < 0.05) and Cronbach’s α (0.500-0.966) suggested that subjects had relatively similar strength performance compared to their peers in the different tests. Greater associations were observed between tests for similar tasks. In addition, strong-magnitude associations were revealed between all the tests and the overall-strength score (r = 0.710-0.806; α = 0.760-0.846). Factor …

This study examined the phenomenon of transient hypoglycemia and metabolic responses to pre-exercise carbohydrate (CHO) maltodextrin ingestion in cycling and running on the same individuals. Eleven active males cycled or ran for 30 min at 80% maximal heart rate (HRmax) after ingestion of either 1g/kg body mass maltodextrin (CHO-Cycle and CHO-Run respectively) or placebo (PL-Cycle and PL-Run) solutions. Education plays an important role in the dissemination of neoliberal narratives. The neoliberal approach to education focuses on human capital and subordinates people to the pure logic of the market. It shapes educational processes in a considerable way, including Human Rights Education (HRE) and Peace Education (PE). The conscious perception and unmasking of the prevailing neoliberal paradigm should therefore be a high priority in a critical approach to HRE and PE. Background: Presidents with no possibility of re-election overvalue far-future rewards and succumb to terminal logic behavior (TLB), responding to end-of-tenure legacy concerns despite political context. Government authorities perceiving the outgoing government is losing power at the end of term behave under the logic of strategic defection (SD), dissociating from the outgoing government once it is perceived powerless. Probiotic Bacillus coagulans GBI-30, 6086 (BC30) has been shown to increase protein digestion in an in vitro model of the stomach and small intestine. Once active in the small intestine after germination, BC30 aids the digestion of carbohydrates and proteins. The extent to which BC30 administration may impact protein digestion and amino acid appearance in humans after protein ingestion is currently unknown. This study examined the impact of adding BC30 to a 25-g dose of milk protein concentrate on post-prandial changes in blood amino acids concentrations.

In 2018, the United Nations General Assembly convened the first-ever high-level meeting (HLM) on tuberculosis (TB). Since that time news on the world’s most lethal infectious disease is not good—the 2019 WHO TB report shows 1.2 million people died from TB, a number that has fallen just 11% since 2015, less than one-third of the way towards the End TB Strategy milestone of a 35% reduction (to about 850 million deaths) by 2020. Purpose: Breast cancer survivors are at increased risk of cancer recurrence, second malignancies, and other comorbid conditions. This study examined if use of a convenient, commercially available, $65 per month app that gives breast cancer survivors access to a health and wellness coach is more effective than a self-guided toolkit and one-time health education session at achieving the following goals: 1) improving adherence to a plant-based diet, 2) increasing physical activity, 3) assisting with weight loss and reduction in body mass index, 4) reducing elevated depression and fatigue scores, and 5) leading to sustained adherence to lifestyle and wellness plan …
