Problem: Individuals with substance use disorder (SUD) have disproportionately higher rates of unintended pregnancy when compared to the general population, estimated to be 85% (Heil et al., 2011). Not only are poor maternal and fetal outcomes associated with unplanned pregnancies, but pregnancies in women with SUD are further complicated by additional risks and adverse outcomes (Black & Day, 2016).

Context: Addiction treatment centers and programs are primed with opportunity to offer family planning services, when contact with medical providers is increased for substance use treatment. In response to the opioid crisis and unprecedented rates of unintended pregnancy, this project …

The Hospital Consumer Assessment of Healthcare Providers and Systems (HCAHPS) survey given to patients after discharge from an inpatient unit is a validate tool used to determine the level of patient experience. Organizations across the country, including the Northern California Academic Medical Center, have implemented many initiatives aimed at improving these scores. HCAHPS scores for the hematology/oncology unit at this medical center remain low even after previous education efforts. The aim of this quality improvement project was to improve the patient experience, as evidenced by HCAHPS scores, with the implementation of an HCAHPS teaching program for nursing staff, focusing on …

Transitions Of Care: Raising Awareness And Improving Identification Of The Social Determinants Of Health, Stephanie Le Dec 2019

Transitions Of Care: Raising Awareness And Improving Identification Of The Social Determinants Of Health, Stephanie Le

Master's Projects and Capstones

Problem: Hospital readmission rates have steadily climbed in the United States and the cost of unplanned readmissions can be detrimental. It has been identified that greater discharge preparation and quality care coordination greatly impacts the patient’s plan of care and reduces the risk of unplanned 30-day readmissions. Transition programs help reduce the psychosocial barriers that prevent patients from being able to self-manage their conditions outside the acute setting and help patients effectively navigate the through the continuum of care.

Context: Research has shown that when the social determinants of health (SDOH) barriers are identified appropriately and early-on, this can decrease …

Exploring Relationships Among Environmental And Intrapersonal Variables And Evidence-Based Practice Implementation, Amanda N. Canada Dec 2019

Exploring Relationships Among Environmental And Intrapersonal Variables And Evidence-Based Practice Implementation, Amanda N. Canada

Nursing Theses and Dissertations

Evidence-based practice (EBP) is not consistently implemented within healthcare settings; although many organizations claim it is how they deliver care in the 21st century. In a review of the literature, multiple intrapersonal variables were found to impact the inconsistency with which EBP is applied and the success of its implementation within nursing practice. Organizational culture and perceived stress can influence the uptake of EBP beliefs and can affect EBP implementation. Clinicians who have more confidence in their EBP knowledge and skills are expected to implement best practices. Underpinned by Social Cognitive Theory, a model was developed to guide study …

The Impact Of A Competitive Healthcare Environment On Cost And Quality, Maureen E. Buckley Dec 2019

The Impact Of A Competitive Healthcare Environment On Cost And Quality, Maureen E. Buckley

Dissertations

A descriptive longitudinal (1988-1993) case study design, with the Resource Dependence Theory as the conceptual framework, was used to investigate the effect of a competitive health care market on the cost and quality of hospital care. The secluded geographic area in which the three study hospitals were located served to minimize the influence of additional competitors. The Resource Dependence theory emphasizes the adaptation of an organization to changes in the environment or competitive market.

Increasing Adult Annual Wellness Visits, Martha Caceres

Doctor of Nursing Practice

The purpose of this quality improvement project was to increase adult annual wellness visits in a high-volume family practice clinic. The significance of an adult annual wellness exam is health maintenance, disease prevention, early disease detection, and management of chronic diseases. Approximately 133 million Americans are diagnosed with at least 1 chronic disease, including cancer, diabetes, and heart disease, and 1 in 4 adults are diagnosed with multiple chronic diseases (Raghupathi & Raghupathi, 2018). In 2011, the Affordable Care Act mandated insurance companies to remove deductibles for preventative health services, relieving the financial burden of out-of-pocket expenses (Borsky et al., …

Faculty Publications - Teaching and Educational Leadership

Communities can adapt to residents' needs through innovative citizen-led initiatives. Extension can facilitate these innovation initiatives, but are Extension agents always receptive to such change? We conducted a study to examine the association between organizational change and personal factors and Extension family and consumer sciences agents' innovativeness regarding caregiving programming. Respondents rated their receptiveness to change and answered questions regarding psychosocial health factors. We found that years in current position, leadership self-efficacy, interoffice support, and social support were significant predictors of innovativeness. Results suggest that personal factors rather than organizational change factors may be the more crucial mechanisms for driving …

Change Effort Organizational Communication Effectiveness And Work Locus Of Control Influence On Change Fatigue In The Workplace, Kady B. Pietz Dec 2019

Change Effort Organizational Communication Effectiveness And Work Locus Of Control Influence On Change Fatigue In The Workplace, Kady B. Pietz

Dissertations

Approximately 70% of change management initiatives fail (Beer & Nohria, 2000; Kotter, 2007; Pasmore, 2011; Warner, 2018), and employee reactions to change are critical in forecasting successful change efforts (Connell & Waring, 2002). Predicting and implementing successful change serves as a critical component in addressing failed change. Change fatigue as an employee experience, among other factors, results in adverse outcomes for organizations through change. Addressing contributing factors of change fatigue to avoid the onset of and potentially harmful implications of change fatigue could also improve change management initiatives (Perel, 2015). To better understand how to successfully implement change in organizational …

Improving Human Rights Compliance In Supply Chains, Kishanthi Parella Dec 2019

Improving Human Rights Compliance In Supply Chains, Kishanthi Parella

Notre Dame Law Review

Corporations try to convince us that they are good global citizens: “brands take stands” by engaging in cause philanthropy; CEOs of prominent corporations tackle a variety of issues; and social values drive marketing strategies for goods and services. But despite this rhetoric, corporations regularly fall short in their conduct. This is especially true in supply chains where a number of human rights abuses frequently occur. One solution is for corporations to engage in meaningful human rights due diligence that involves monitoring human rights, reporting on social and environmental performance, undertaking impact assessments, and consulting with groups whose human rights they …

Quality Improvement Initiative To Increase Comprehensive Eye Exams In Patients With Diabetes, Julia Khoury Dec 2019

Quality Improvement Initiative To Increase Comprehensive Eye Exams In Patients With Diabetes, Julia Khoury

Doctor of Nursing Practice

Diabetic retinopathy is the most common microvascular complication associated with poorly controlled diabetes. Yet, only 60% of the diabetic population receives an annual eye examination by an eye specialist. Retinal imaging increases surveillance of retinopathy, however, it does not take the place of a comprehensive eye examination by an ophthalmologist or optometrist. The aim of this quality improvement project was to increase comprehensive eye examinations in patients with diabetes aged 18 to 75 years in a family practice that recently purchased a retinal imager. This quality improvement project occurred over a 10-week period. Project objectives included increasing comprehensive dilated eye …

How Ecommerce Is On The Rise, Kayla Smith Dec 2019

How Ecommerce Is On The Rise, Kayla Smith

Honor’s College Freeman Research Journal

In this paper I will observe how the development of new technologies (that have seen extreme changes) has led to a lot of changes in the way products are manufactured, services are offered, and money is transacted and the products delivered. Due to changes in consumer demands in the market, technology has been manipulated over the years, giving rise to multinational billion dollar organizations that keep adapting overtime to keep up with the rapidly changing consumer needs. In particular, with communication, the rise of the mobile phone network and the Internet have particularly revolutionized the way business is done entirely. …
