The discovery of the human genome has unveiled new fields of genomics, transcriptomics, and proteomics, which has produced paradigm shifts on how to study disease mechanisms, wherein a current central focus is the understanding of how gene signatures and gene networks interact within cells. These gene function studies require manipulating genes either through activation or inhibition, which can be achieved by temporarily permeabilizing the cell membrane through transfection to deliver cDNA or RNAi. Background: A major health problem that frequently accompanies old age is long-term pain, but pain must be acknowledged by older adults and health care providers. Interventions are needed to alleviate pain and suffering while holistically providing health care that promotes wellbeing. The intervention project, Reflective STRENGTHGiving Dialogues© (STRENGTH) was implemented to increase health and wellbeing among community dwelling older adults living with long-term musculoskeletal pain at home.

The central focus of this dissertation is to examine the inextricable link between persons, their social worlds, and their environments. I do this through an ethnographic study of senior members of non-biologically based kinship groups with an affiliation to place. Critical to this examination is the city of Detroit itself, as members of these groups ultimately collectively identify as Detroiters through space and time. Introduction

Serious Streptococcus pneumoniae infections, encompassing pneumonia, bacteremia, and meningitis, are a major cause of mortality. However, literature regarding mortality is often limited to invasive pneumococcal disease, excluding pneumonia. This study sought to identify predictors of mortality among adults with serious pneumococcal disease, including pneumonia and invasive pneumococcal disease.

Methods

This was a nested case-control study of unvaccinated older Veterans with positive S. pneumoniae cultures (blood, cerebrospinal fluid, respiratory) admitted to Veterans Affairs medical centers nationally between 2002 and 2011. Patients vaccinated against pneumococcal disease were excluded. Using multivariable logistic regression, predictors of 30-day mortality were identified, including patient demographics, …

Assessment of level of consciousness using the Glasgow Coma Scale (GCS) is a tool requiring knowledge that is important in detecting early deterioration in a patient’s level of consciousness. Critical thinking used with the skill and knowledge in assessing the GCS is the foundation of all nursing practice. This study aims to explore the knowledge and competence in assessing the GCS among staff nurses working in the Emergency and Outpatient Departments. The bathing checklist program was implemented for five months in a home health care setting with clients who received occupational therapy and home health aide services. This program's main intention was to help clients become more independent with bathing by the occupational therapy discharge utilizing the home health aide to achieve this goal. The bathing checklist complied of 18 steps of bathing and an educational sheet to inform the client and caregivers of the program. The relative income hypothesis predicts poorer health in societies with greater income inequality, yet the psychological mechanisms that explain this association are not clear to date. This study tests the hypothesis that perceived age discrimination acts as a mediator in the inequality-health nexus for people who categorize themselves as old. It is expected that the detrimental mediating effect of perceived age discrimination does not occur for those who categorize themselves as young, since their low status is only temporary until they move to the higher status middle-aged group. What is wisdom and does it come with age as many people assume, or is it a relatively rare quality even among the older population? How do people develop wisdom throughout life and what might be its benefits in old age? Empirical evidence suggests that wisdom in old age is positively related to subjective well-being and less fear of death, even in the face of physical disability or the nearing of death (Ardelt, Landes, Gerlach, & Fox, 2013). Introduction: Fear of Alzheimer’s disease (FAD), or Anticipatory Dementia, is a healthy adult’s misinterpretation of everyday memory failures as indicators of developing dementia. The current study investigated the construct of FAD and aimed to contextualize FAD within the Health Belief Model through development of a new scale, the Anticipatory Dementia Index (ADI). The study also assessed the relationship between FAD and metacognitive monitoring and metacognitive control.

Resident-to resident bullying has attracted some attention in the popular press and is well-known to many who work with seniors in long-term care facilities. However, this is very little empirical literature that has address the topic of "senior bullying". The aim of the proposed qualitative study is to better understand the phenomenon of resident-to-resident bullying from the perspective of staff who work in long-term care facilities. Staff members (n=45) responded to a combination of open- and close-ended interview questions regarding their observations of senior-to-senior bullying. Despite an aging U. S. workforce, age discrimination at work remains an issue. Researchers have found that beliefs about the aging process affect workers' performance and attitudes. There is little research available examining this phenomenon from the perception of older workers. Although wisdom has always played a prominent role in philosophy and religion, contemporary empirical wisdom research started around 1980, when several research teams tried to determine what wisdom is and how it can be measured. Two different approaches emerged, divided into implicit and explicit theories of wisdom. The implicit approach asked lay people to name characteristics of wise individuals that were then summarized into several dimensions, while the explicit approach referred to experts and classical wisdom texts to define the essential elements of wisdom. Homeodynamic space (HDS) shrinks as vulnerability increases with aging and repeated damage to the cells. HDS is lost in alcoholic pancreatitis patients due to overconsumption of alcohol, smoking, and high fat diets. Etiologically relevant animal models for study of chronic pancreatitis (CP) are needed. In order to begin filling this gap a central purpose of this dissertation research was to examine relationships between the alcohol and high fat diet (AHF) and pancreatitis with attention to hypersensitivity and anxiety-like behaviors. The AHF diet induced pancreatitis described here etiologically mimics human risk factors of AHF consumption for advancement to alcoholic CP.

Purpose of the study: This article focuses on Chinese visiting grandparents who come to the US to help their adult children who are in graduate school care for the grandchild/ren. The aims were to determine: 1) what motivates them to come to the US, 2) what are the challenges and supports of life in the US, and 3) the implications of their experience in the US as caregivers for their well-being.

Alcohol use disorder (AUD) is a problem for older veterans. This study examines whether positive age stereotypes may be associated with lower prevalence of alcohol use disorder in this group. Prevalence estimates of demographic and military characteristics were analyzed by Alcohol Use Disorder status and age group. The cohort consisted of participants in the National Health and Resilience in Veterans Study. The Alcohol Use Disorder Identification Test, that assesses AUD DSM5 symptoms was used to measure AUD prevalence, number of symptoms, an alcohol consumption subscore, and an alcohol consequences subscore. In the present study, we tested whether the volatile metabolome was altered by mutations of the Alzheimer’s disease (AD)-implicated amyloid precursor protein gene (APP) and comprehensively examined urinary volatiles that may potentially serve as candidate biomarkers of AD. Establishing additional biomarkers in screening populations for AD will provide enhanced diagnostic specificity and will be critical in evaluating disease-modifying therapies. Having strong evidence of gross changes in the volatile metabolome of one line of APP mice, we utilized three unique mouse lines which over-express human mutations of the APP gene and their respective non-transgenic litter-mates (NTg). Introduction: Stepping On is a 7-week comprehensive fall program aimed at reducing falls for the community-dwelling elderly. Participants are educated on balance and strengthening exercises, and the influence medications, visions, inclement weather, etc. has on safety and falls.

Purpose: The main purpose of this study is to utilize the Activities-specific Balance Confidence scale (ABC) and survey results to determine if attendance of Stepping On improves balance confidence.
