Department of Emergency Medicine

Regional spinal and epidural anesthesia are used commonly in operative procedures. While the most frequent complication, postdural puncture headache (PDPH), is a clinically diagnosed positional headache that is usually self-limited, subdural hemorrhage (SDH) is a potentially fatal complication that cannot be missed. We report a case of an otherwise healthy female who presented with persistent positional headache and was ultimately found to have a large subdural hematoma with midline shift requiring surgical evacuation.

Bridge structures are subjected to continuous degradation due to environmental effects and increase in traffic loading. Bridge monitoring is a key element of any maintenance strategy as it can provide early signs of structural deterioration. In recent years, the demand towards sensor-based bridge monitoring in lieu of visual inspection, has increased. However, sensor-based monitoring is costly and requires significant maintenance. Recent studies on bridge damage monitoring have focused on the instrumentation of a passing vehicle instead of the bridge. This dissertation presents some results on the solvability of second order elliptic equations with nonlinear boundary conditions at resonance, in which the nonlinear boundary conditions perturbation is not necessarily required to satisfy Landesman- Lazer conditions or the monotonicity assumption. The nonlinearity may be unbounded. The nonlinearity interacts in some sense with the Steklov spectrum. The proofs are based on a priori estimates for possible solutions to a homotopy on suitable trace spaces and topological degree arguments. The objective of this dissertation was to study the effects of nanoconfinement on the thermodynamic and kinetic parameters of phase transitions in selected compounds. The compounds were loaded into silica gel nanopores and filled them partially. A model was developed for estimating the fullness of the nanopores as well as the height of the compound layer deposited on the pore walls. A study of the phase transition in NH4Cl confined to the native silica gel nanopores revealed that the heat of transition increases with the increasing the layer height. Pancreatic beta cells secrete insulin to maintain normal blood glucose levels and allow cells of the body to use glucose. However, in diabetes, there is beta cell dysfunction and pancreatic beta cells undergo programmed cell death leading to inadequate insulin production and secretion. Thioredoxin-Interacting Protein (TXNIP) is a ubiquitously expressed protein, 46 kDa in size, which is an arrestin protein family member. Unique to this member of the arrestin family, however, is its ability to bind to, and reduce, thioredoxin. Drawing on the sociological theories of fundamental causes and social support, it was hypothesized that exercise during pregnancy would be positively associated with both socioeconomic status (SES) and social support for exercise and that the association between SES and exercise levels would be at least partially explained by attitudes towards exercise. Buprenorphine/naloxone (BNT) is a pharmacological treatment for opioid use disorders and is available by prescription in an office-based setting from physicians with specialized training. The purpose of this study was to identify the characteristics and behaviors among adults on BNT with the intent to identify the linear combination of variables that account for the differences based on the origins of opioid use. Within this thesis are two separate articles, each representing a distinct chapter. The first chapter is a redescription of the ‘toxochelyid’-grade marine turtle Ctenochelys acris Zangerl, 1953. This redescription, based on several nearly complete specimens from the early Campanian Mooreville Chalk of Alabama, reveals multiple previously undescribed autapomorphic characteristics of this species and contributes significantly to our understanding of ‘toxochelyid’ alpha taxonomy. These individuals play a central role in a religious institution that is in a state of decline in the 21st century United States. In addition, burnout, a state of compromised social psychological health related to work involving relationships, is a salient issue among clergy. Obesity is a state of chronic low-grade systemic inflammation that, along with type 2 diabetes (T2D), increases the risk of developing cardiovascular disease (CVD). Despite the wealth of information on the link between macrophages and cytokines in adipose tissue and peripheral insulin resistance, their role in the pathogenesis of diabetic cardiomyopathy and cardiac diastolic dysfunction is unclear. Dentinogenesis is a complex process of dentin formation. It involves differentiation of odontoblasts, the cells that make dentin, and the formation of a unique extracellular matrix, known as dentin. Dentin is ultimately produced through the interaction of collagen fibers, non-collagenous proteins and hydroxyapatite (HA) crystals. Odontoblasts are cranial neural crest (CNC) ectomesenchymal derived cells. The present study investigated the efficacy of a single administration of transcranial direct current stimulation (tDCS) to reduce binge eating symptoms, namely food consumption, food cravings, and binge urges, in individuals clinically diagnosed with BED and sub-threshold BED. The study used a within-subjects crossover design whereby each participant received both a real and a sham condition of tDCS in counterbalanced order. Results indicated that tDCS significantly reduced caloric intake of specifically preferred foods during an in-lab eating test, decreased food cravings induced by food photo images, and decreased binge desire on the same day as treatment, compared to sham stimulation. …

The conditions that embryos experience during development have substantial effects on offspring phenotypes and fitness. Although plastic responses to developmental environments are widespread, whether they are adaptive is unknown in most cases. Developmental plasticity is often thought to be adaptive in heterogeneous environments, but it can also enhance fitness when environments experienced by embryos provide cues for the conditions they will eventually experience later in life. The aim of this study is to review the state of art concerning 3D optical acquisition techniques and devices currently used in Orthodontics, and determine whether a commercial intra-oral scanner is as accurate as traditional impressions for orthodontic purposes.
