Racecadotril For The Treatment Of Severe Acute Watery Diarrhoea In Children Admitted To The Kenyatta National Hospital - A Randomised Double Blinded Placebo Controlled Trial, Jaspreet Singh Gharial

Master of Medicine in Paediatrics and Child Health

Background: Diarrhoeal disease is the second most common cause of death in children under 5 years of age in Kenya. Treatment with Oral Rehydration Solution (ORS), elemental zinc and continued feeding (as recommended by the World Health Organisation) led to a significant reduction in mortality in the 1970s and 1980s, which has slowed since then. Adjunctive treatments have however, not gained much popularity either due to modest improvements in morbidity and mortality or unacceptable adverse effect profiles. Racecadotril, an enkephalinase inhibitor, has been used for over 2 decades in parts of Europe but is only recently gaining recognition in other …

Hospital-Acquired Malnutrition In Children In A Tertiary Care Hospital, Del-Rossi Sean Quadros Jan 2014

Hospital-Acquired Malnutrition In Children In A Tertiary Care Hospital, Del-Rossi Sean Quadros

Master of Medicine in Paediatrics and Child Health

Introduction: Hospital-acquired Malnutrition occurs as a result of reduction in food intake, increased dietary loss and/or increased calorie requirements as a result of disease-induced high catabolic state. A child's nutritional status often deteriorates after admission to the hospital resulting in longer duration of hospital stay and increased risk of complications, which also increases treatment cost. Hospital-acquired Malnutrition is usually assessed using anthropometric measurements and/or serum pre-albumin. Objectives: This study sought to investigate the incidence of Hospital-acquired Malnutrition using anthropometric measurements and to determine diagnostic utility of serum pre-albumin in predicting weight change. A secondary objective was to identify factors associated …

Cura Personalis: A Healthcare Delivery Quandary At The End Of Life, George P. Smith Ii Jan 2014

Cura Personalis: A Healthcare Delivery Quandary At The End Of Life, George P. Smith Ii

Scholarly Articles

Holistic Medicine traces its provenance to the foundational value or chrism of the Society of Jesus of cura personalis which directs respect be given to all individuals and to their souls — especially whenever medical healing is required. Today, the notion of best patient care should include not merely attention to somatic issues of refractory pain management but, equally, to non-somatic or existential suffering. It is at the end-stage of life that palliative — as opposed to curative — care must be provided. When a condition is seen as medically futile, this Article advocates palliative or deep sedation — when …

Background: Race and ethnicity are significant factors in predicting survival time of breast cancer patients. In this study, we applied advanced statistical methods to predict the survival of White non-Hispanic female breast cancer patients, who were diagnosed between the years 1973 and 2009 in the United States (U.S.). Materials and Methods: Demographic data from the Surveillance Epidemiology and End Results (SEER) database were used for the purpose of this study. Nine states were randomly selected from 12 U.S. cancer registries. A stratified random sampling method was used to select 2,000 female breast cancer patients from these nine states. We compared …

Sediment, Nutrient, And Bacterial Runoff From Biosolids And Mineral Fertilizer Applied To A Mixed Cool- And Native Warm-Season Grassland In The Ozark Mountains, Cody B . Wallace, Michael G. Burton, Steven G. Hefner, Thomas A. Dewitt Jan 2014

Dewitt

College of Agriculture

Rainfall simulations were conducted within mixed (cool- and native warm-season) grasslands in the sloping, rocky soils typical of the Ozark Mountains region to estimate nutrient and bacteria levels in runoff from biosolids and mineral fertilizer (MF). The ability of narrow (1 m) vegetated filter strips (VFS) to reduce losses was evaluated. Experiment 1 included an untreated control (C); 37 kg plant available nitrogen (PAN) ha−1 from biosolids applied to the upslope half of the plot with the downslope half serving as a VFS (LBF); 74 kg PAN ha−1 from biosolids, with VFS (HBF); and a uniform biosolids application at the …

Trauma Exposure, Posttraumatic Stress, And Depression In A Community Sample Of First-Time Mothers, Mickey Sperlich Jan 2014

Trauma Exposure, Posttraumatic Stress, And Depression In A Community Sample Of First-Time Mothers, Mickey Sperlich

Wayne State University Dissertations

The adverse effects of posttraumatic stress and depression have separately been well-documented in the perinatal mental health literature. However, few studies have considered the comorbidity between trauma, posttraumatic stress and depression. This dissertation study brings attention to this comorbidity and explores implications of recent changes to diagnostic criteria for posttraumatic stress disorder related to the ability to predict postpartum depression and impairments in mother/infant bonding. Following a conceptual framework which outlines the effects of violence and trauma on adverse childbearing outcomes, hypotheses were that many women with depression in pregnancy would endorse trauma and would be at risk for subthreshold …

Migratory Connectivity And Carry-Over Effects In Northwest Atlantic Loggerhead Turtles (Caretta Caretta, L.), Simona Ceriani Jan 2014

Migratory Connectivity And Carry-Over Effects In Northwest Atlantic Loggerhead Turtles (Caretta Caretta, L.), Simona Ceriani

Electronic Theses and Dissertations

Migration is a widespread and complex phenomenon in nature that has fascinated humans for centuries. Connectivity among populations influences their demographics, genetic structure and response to environmental change. Here, I used the loggerhead turtle (Caretta caretta, L.) as a study organism to address questions related to migratory connectivity and carry-over effects using satellite telemetry, stable isotope analysis and GIS interpolation methods. Telemetry identified foraging areas previously overlooked for loggerheads nesting in Florida. Next, I validated and evaluated the efficacy of intrinsic markers as a complementary and low cost tool to assign loggerhead foraging regions in the Northwest Atlantic Ocean (NWA), …

Food Miles: Environmental Protection Or Veiled Protectionism?, Meredith Kolsky Lewis, Andrew D. Mitchell Jan 2014

Food Miles: Environmental Protection Or Veiled Protectionism?, Meredith Kolsky Lewis, Andrew D. Mitchell

Michigan Journal of International Law

Eat local. Such a small phrase yet such a loaded proposition. Buying food from nearby sources has become a popular objective. This aim is associated with helping farmers in one’s country or region; observing the seasonality of one’s location; eating fresher foods; striving for food security; and protecting the environment. One of the unmistakable messages of the “locavore” movement is that importing food—particularly food that comes from far away—causes environmental harm. The theory is that transporting food long distances results in the release of high levels of greenhouse gases (GHGs) into the atmosphere and is thus a dangerous contributor to …

Nitrogen Fertilization Has A Stronger Effect On Soil Nitrogen-Fixing Bacterial Communities Than Elevated Atmospheric Co2, Sean T. Berthrong, Chris M. Yeager, Laverne Gallegos-Graves, Blaire Steven, Stephanie A. Eichorst, Robert B. Jackson, Cheryl R. Kuske

Scholarship and Professional Work - LAS

Biological nitrogen fixation is the primary supply of N to most ecosystems, yet there is considerable uncertainty about how N-fixing bacteria will respond to global change factors such as increasing atmospheric CO2 and N deposition. Using the nifH gene as a molecular marker, we studied how the community structure of N-fixing soil bacteria from temperate pine, aspen, and sweet gum stands and a brackish tidal marsh responded to multiyear elevated CO2 conditions. We also examined how N availability, specifically, N fertilization, interacted with elevated CO2 to affect these communities in the temperate pine forest. Based on data …

Variation Of Some Seed Mineral Contents In Open Pollinated Faba Bean (Vicia Faba L.) Landraces From Turkey, Faheem Shahzad Baloch, Tolga Karaköy, Ahmet Demi̇rbaş, Faruk Toklu, Hakan Özkan, Rüştü Hati̇poğlu

Turkish Journal of Agriculture and Forestry

The first step towards the biofortification of edible portions of crop species with improved nutritional value is to understand the genetic diversity available to breeders in germplasm collections. A faba bean germplasm (129 landraces and 4 cultivars) from diverse geographic regions of Turkey was assessed for micro- and macroelement contents of seeds. The results showed high diversity in open-pollinated faba bean germplasm for contents of N (27.5-93.3 g kg^{-1}), P (1.24-4.89 g kg^{-1}), K (4.5-19.3 g kg^{-1}), Fe (29.7-96.3 mg kg^{-1}), Mn (15.5-29.2 mg kg^{-1}), Cu (10.3-33.0 mg kg^{-1}), and Zn (10.4-49.3 mg kg^{-1}). Meanwhile, the ranges of mineral elements …

Limiting Liberty To Prevent Obesity: Justifiability Of Strong Hard Paternalism In Public Health Regulation, Thaddeus Mason Pope Jan 2014

Limiting Liberty To Prevent Obesity: Justifiability Of Strong Hard Paternalism In Public Health Regulation, Thaddeus Mason Pope

Faculty Scholarship

Because of the largely self-regarding nature of obesity, many current and proposed public health regulatory measures are paternalistic. That is, these measures interfere with a person’s liberty with the primary goal of improving that person’s own welfare.

Paternalistic public health measures may be effective in reducing obesity. They may even be the only sufficiently effective type of regulation. But many commentators argue that paternalistic public health measures are not politically viable enough to get enacted. After all, paternalism is repugnant in our individualistic culture. It is "wrong" for the government to limit our liberty for our own good.

In this …

The Millennium Development Goals (MDGs) harnessed the power of numbers to focus the world's attention on some of the most pressing development challenges. Numbers are perceived to be professional, rational, credible, and neutral. The MDGs embody both the promise and the peril of development approaches that view measurable outcomes as the most certain path to progress for all. They focus on quantifiable progress in access to basic social services and goods, using only well-known indicators, metrics, and established data sets for monitoring. Further, they focus on aggregate progress, failing-except in relation to a specific gender equality goal-to capture the dimension …

Synthetic drug-like molecules that directly modulate the activity of key clock proteins offer the potential to directly modulate the endogenous circadian rhythm and treat diseases associated with clock dysfunction. Here we demonstrate that synthetic ligands targeting a key component of the mammalian clock, the nuclear receptors REV-ERBα and β, regulate sleep architecture and emotional behaviour in mice. REV-ERB agonists induce wakefulness and reduce REM and slow-wave sleep. Interestingly, REV-ERB agonists also reduce anxiety-like behaviour. These data are consistent with increased anxiety-like behaviour of REV-ERBβ-null mice, in which REV-ERB agonists have no effect. These results indicate that pharmacological targeting of REV-ERB …

An Economic Perspective On Subprime Lending, Michael H. Anderson Jan 2014

An Economic Perspective On Subprime Lending, Michael H. Anderson

Chicago-Kent Law Review

This article aims to provide a concise economic overview of several interesting subprime financing mechanisms, which are becoming increasingly common on the U.S. financial landscape. In particular, rent-to-own, payday lending, pawn broking, and (vehicle) title loans are considered. Generally speaking, a common thread with these loans is their relatively small size and short duration as well as the absence of a credit check or any of the traditional processes for determining credit-worthiness. Due to the ready availability of these loans, they appeal to low-income consumers, including the “working poor,” and to those who have suffered financial setbacks. Because the natural …
