Although present in medical and historical texts for millennia, urinary incontinence remains a somewhat taboo topic, with both affected individuals and remedies for the condition--such as adult diapers--subject to ridicule, embarrassment, status loss, discrimination, and even exile. Reinforcing this discriminatory behavior, the adult diaper--a spur from the baby diaper invention--tracks a parallel path to incontinence. With diapers and adult incontinence rooted in a misunderstood cross-pollination with infant incontinence, individuals must struggle against known stereotypes and stigmas liable to label them as incompetent, impotent, or unclean. The stigma of incontinence is thus aligned with the diaper, reinforcing social ignorance and discriminating …

Photoacoustic (PA) imaging is a rapidly emerging non-invasive soft tissue imaging modality which has the potential to detect tissue abnormality at early stage. Photoacoustic images map the spatially varying optical absorption property of tissue. In multiwavelength photoacoustic imaging, the soft tissue is imaged with different wavelengths, tuned to the absorption peaks of the specific light absorbing tissue constituents or chromophores to obtain images with different contrasts of the same tissue sample. From those images, spatially varying concentration of the chromophores can be recovered. In this study, we retrospectively analyzed the outcomes of adults with bicuspid aortic valve (BAV) disease who underwent aortic valve reconstructive surgery (AVRS), consisting of replacement of the diseased BAV with 2 or 3 pericardial leaflets plus fixation of the sinotubular junction for accurate and constant leaflet coaptation. From December 2007 through April 2013, 135 consecutive patients (mean age, 49.2 ± 13.1 yr; 73.3% men) with symptomatic BAV disease underwent AVRS. Raphe was observed in 84 patients (62.2%), and the remaining 51 patients had pure BAV without raphe. A total of 122 patients (90.4%) underwent 3-leaflet reconstruction, and 13 (9.6%) …

The brachiocephalic artery is an alternative cannulation site in the repair of ascending aortic lesions that require circulatory arrest. We evaluate the effectiveness and safety of this technique.
Proximal aortic surgery was performed in 32 patients from 2006 through 2012 via brachiocephalic artery cannulation and circulatory arrest. Twenty-four (75%) of the patients were men. The mean age was 48.69 ± 9.43 years (range, 30–68 yr). Twelve had type I dissection, 2 had type II dissection, and 18 had true aneurysms of the ascending aorta. All operations were performed through a median sternotomy. The arterial cannula was inserted through an 8-mm …

Nonprofit Executive Pay As An Agency Problem: Evidence From U.S. Colleges And Universities, Brian D. Galle, David I. Walker

We analyze the determinants of the compensation of private college and university presidents from 1999 through 2007. We find that the fraction of institutional revenue derived from current donations is negatively associated with compensation and that presidents of religiously-affiliated institutions receive lower levels of compensation. Looking at the determinants of contributions, we find a negative association between presidential pay and subsequent donations. We interpret these results as consistent with the hypotheses that donors to nonprofits are sensitive to executive pay and that stakeholder outrage plays a role in constraining that pay. The recent financial crisis has jeopardized the retirement savings of twenty-seven million Americans who depend on public pension funds, leading to cuts in benefits, increased employee contributions, job losses, and the rollback of legal rights like collective bargaining. This Article examines ways in which public pension funds invest against the economic interests of their own participants and beneficiaries, and the legal implications of these investments. In particular, the Article focuses on the use of public pensions to fund privatization of public employee jobs. Scholars and activists interested in racial justice have long been opposed to surrogacy arrangements, wherein a couple commissions a woman to become pregnant, give birth to a baby, and surrender the baby to the couple to raise as its own. Their fear has been that surrogacy arrangements will magnify racial inequalities inasmuch as wealthy white people will look to poor women of color to carry and give birth to the white babies that the couples covet. However, perhaps critical thinkers about race should reconsider their contempt for surrogacy following the Supreme Court’s recent decision in United States v. Windsor. In …

There are several normative theories of jurisprudence supporting our critique of the scored society, which complement the social theory and political economy presented in our 2014 article on that topic in the Washington Law Review. This response to Professor Tal Zarsky clarifies our antidiscrimination argument while showing that is only one of many bases for the critique of scoring practices. The concerns raised by Big Data may exceed the capacity of extant legal doctrines. Addressing the potential injustice may require the hard work of legal reform.

This chapter is based on data collected as part of a larger qualitative empirical study based on face-to-face interviews with artists, scientists, engineers, their lawyers, agents and business partners. Broadly, the project involves the collecting and analysis of these interviews to understand how and why the interviewees create and innovate and to make sense of the intersection between intellectual property law and creative and innovative activity from the ground up. The widespread use of Mattick and Clarke’s (1998) Social Interaction Anxiety Scale (SIAS) and Social Phobia Scale (SPS) led two independent groups of researchers to develop short-forms of these measures (Fergus, Valentiner, McGrath, Gier-Lonsway, & Kim, 2012; Peters, Sunderland, Andrews, Rapee, & Mattick, 2012). PURPOSE: Accurate characterization of contrast reagent (CR) longitudinal relaxivity in whole blood is required to predict arterial signal intensity in contrast-enhanced MR angiography (CE-MRA). This study measured the longitudinal relaxation rate constants (R1 ) over a concentration range for non-protein-binding and protein-binding CRs in ex vivo whole blood and plasma at 1.5 and 3.0 Tesla (T) under physiologic arterial conditions.
METHODS: Relaxivities of gadoteridol, gadobutrol, gadobenate, and gadofosveset were measured for [CR] from 0 to 18 mM [mmol(CR)/L(blood)]: the latter being the upper limit of what may be expected in CE-MRA.

When a heterosexual man is charged with murdering a transgender woman with whom he has been sexually intimate, one defense strategy is to assert what has been called the trans panic defense. The defendant claiming this defense will say that the discovery that the victim was biologically male provoked him into a heat of passion causing him to lose self-control. If the jury finds that the defendant was actually and reasonably provoked, it can acquit him of murder and find him guilty of the lesser offense of voluntary manslaughter. While bicyclists and other active travelers obtain health benefits from increased physical activity, they also risk an uptake of traffic-related air pollution. But pollution exposure for urban bicyclists is not well understood due to a lack of direct measurements and insufficient analysis of the determinants of exposure. This knowledge gap impedes pollution-conscious transportation planning, design, and health impact assessment. An Environmental Value Engineering (EVE) analysis was used to conduct an environmental life cycle assessment of transportation alternatives consisting of high-speed passenger rail and interstate passenger car transportation. This analysis was formulated on the EMERGY inputs of environment, fuel energy, goods, and services (labor). EMERGY input tables were calculated based on subsystem inputs for each mode of transportation. Areas of commonality were identified for analysis exclusion while aspects that showed differentiation were included as viable inputs into the calculation of the total EMERGY for each system. Electronic dance music (EDM) events are strongly associated with polydrug use, or the use of several substances at one time (Fernandez-Calderon et al., 2011), which has contributed to overall negative stereotypes of the EDM scene. Nitrous oxide use has been documented in EDM research, but mainly in qualitative designs (Hunt, Evans, Moloney, and Bailey, 2009). Qualitative research suggested participants pair nitrous oxide inhalation with use of ecstasy, or MDMA, for feeling an enhanced effect. The purpose of this study was to investigate the effects of large-group, explicit, story grammar vocabulary instruction during shared storybook reading on the narrative retell skills of preschoolers. Two preschool classrooms in rural, southeastern Illinois participated in the study. The study examined narrative retell ability via the Test of Narrative Retell-Preschool. Scores were compared for the experimental group, who received instruction from the investigator, to the control group, who received instruction from their regular classroom teacher. This is the first known study to examine the relationship between Native identity and psychological well-being, which is defined by Ryff in positive psychology as existential strengths. It is also the first known study to investigate the relationship between Native identity and blood quantum through quantitative measures. Background: Alcohol consumption among HIV-infected patients may accelerate HIV disease progression or reduce antiretroviral therapy adherence. Self-reported alcohol use is frequently under-reported due to social desirability and recall bias. The aim of this study was to compare self-reported alcohol consumption to phosphatidylethanol (PEth), a biomarker of alcohol consumption, and to estimate the correlation between multiple measures of self-reported alcohol consumption with PEth.
Methods: The Uganda AIDS Rural Treatment Outcomes (UARTO) cohort is located in southwestern Uganda and follows patients on ART to measure treatment outcomes. In the central nervous system, cystine import in exchange for glutamate through system xc- is critical for the production of the antioxidant glutathione by astrocytes, as well as the maintenance of extracellular glutamate. The Staphylococcus aureus two-component regulatory system, GraRS, is involved in resistance to killing by distinct host defense cationic antimicrobial peptides (HD-CAPs). It is believed to regulate downstream target genes such as mprF and dltABCD to modify the S. aureus surface charge. However, the detailed mechanism(s) by which the histidine kinase, GraS, senses specific HD-CAPs is not well defined. Post-myocardial infarction ventricular septal defect is a devastating complication of ST-elevation myocardial infarction. Although surgical intervention is considered the gold standard for treatment, it carries high morbidity and mortality rates. We present 2 cases that illustrate the application of percutaneous closure of a post-myocardial infarction ventricular septal defect: the first in a patient who had undergone prior surgical closure and then developed a new shunt, and the second as a bridge to definitive surgery in a critically ill patient.

The objective of this project is to design a framework that could be used to evaluate the effectiveness and efficiency of non-emergency transportation services (NEMT) for better livability.
