Autonomic Dysfunction As The Initial Presentation Of Vitamin B12 Deficiency: A Casereport, Hun-Pin Lee, Huey-Wen Liang, Tung-Chou Li

Background: Vitamin B12 deficiency may be associated with variable neurological involvement, and myelopathy is the most common disorder associated with this deficiency. However, initial presentation with autonomic dysfunction is very rare.Case: The present report describes the case of a 76-year-old man who initially presented with recurrent syncope, followed by impaired vibration and proprioception, and then progressive weakness of the limbs. Result: The findings of a hematological study, blood biochemistry, gastric biopsy by panendoscopy, and electrophysiological examinations all supported the diagnosis of chronic atrophic gastritis-related vitamin B12 deficiency associated with macrocytic anemia, myelopathy, peripheral neuropathy, and autonomic dysfunction. Ultrasound imaging provides a unique and complete representation of tongue structure in almost all planes of interest and is rapid enough to record real-time tongue movement during speech. Recording tongue movement with ultrasound is non-invasive, and has not been shown to have any biologic effects in humans at diagnostic intensities. As a result, ultrasound has been widely used in speech studies to measure tongue functions in recent years. Papillary thyroid carcinoma is a type of well-differentiated thyroid cancer. This type of cancer is often diagnosed in patients found with palpable neck mass or lymph node enlargement. The clinical characteristics of papillary thyroid carcinoma are slow disease progression, less tendency for distant metastasis, and better prognosis than other types of thyroid cancer. The diagnosis of thyroid cancer is primarily based on the identification of pathological tissue obtained through echo-guide fine needle aspiration biopsy. Objective: The aim of this study was to analyze the effectiveness of a new air cell cushion (Elaston cushion) made of gelatinous elastomers, a newly developed material with a novel structure on reducing pressure and temperature.Methods: We compared the pressure and thermal responses of the Elastone cushion to three other available cushions (foam, gel-filled and air-filled villous) and four regular wooden or spongy chair seats. The skin temperature at the buttocks was also continuously measured for eight-hours sitting. A 38-year-old man with uncontrolled type 2 diabetes mellitus complained of a mild painful swelling over the right posterior thigh that had been present for 3 days. He was initially diagnosed as having a right hamstring strain. After a 7-day treatment with non-steroidal anti-inflammatory drugs, the symptoms progressed to a prominent painful swelling over the right posterior thigh. Leukocytosis and high inflammation indices were noted. A 30-year-old woman with a history of 9th to 11th thoracic vertebral compression fracture developed complete thoracic spinal cord injury immediately after percutaneous vertebroplasty. The patient did not have any sign of neurological recovery after two operations for spinal decompression. The postoperative chest X ray and thoracic computed tomography scan revealed diffuse asymptomatic pulmonary embolism, but the patient did not have dyspnea or chest pain.Percutaneous vertebroplasty has been generally considered a minimally invasive and effective procedure for treatment of painful compression fracture. Neuromyelitis optica is a rare demyelinating disease. Radicular pain as the initial presentation of monophasic neuromyelitis optica has not been previously reported. We present a case of monophasic neuromyelitis optica in a 69-year-old female who suffered 1 year (from 2004/6 to 2005/5) from back pain that radiated to the lower extremities. Fifth lumbar radiculopathy was the first diagnosis. Despite treatment with laser discectomy on 2005/2/15, her pain, lower extremity weakness, and bilateral visual acuity worsened. Three weeks after surgery, she came to our hospital for help and was admitted for evaluation. The diagnosis of neuromyelitis optica was confirmed later. Prednisolone …

The paper investigates the influence of technical and allocative efficiency on export performance of Slovenian individual firms. The main contribution of the paper to the literature is the test on whether technical and allocative efficiencies, measured by data envelopment analysis (DEA) and stochastic frontier function (SFA), significantly influence the export orientation of individual firms. Our evidence suggests that firms which are more efficient than their competitors in industry are more export oriented and that technical efficiency is more significant for firms’ export orientation than allocative efficiency. This paper investigates the relative costs and benefits associated with introducing a more active monetary and a different exchange rate regime in the Republic of Macedonia. In this finding, the econometrics result show that introducing a more active monetary policy and a different strategy of the exchange rate targeting in order to promote rapid economic growth could easy disturb macroeconomic stability (after having achieved it at a substantial cost) without any significant economic benefits. Dental caries is a highly prevalent disease caused by colonisation of tooth surfaces by cariogenic bacteria, such as Streptococcus sobrinus and Streptococcus salivarius. Reducing initial adherence of such bacteria to teeth may delay onset of caries. Many foods, such as milk, can inhibit microbial adherence. In this investigation, the effect of untreated (UT) and enzyme-treated (ET) dairy powders on adherence of S. sobrinus and S. salivarius to hydroxylapatite (HA), an analogue of tooth enamel, was examined. In May 2009, the landlord of a rent-stabilized building in Williamsburg, Brooklyn openly declared his intention to force out the current tenants if they did not leave on their own. Located on North 8th Street right off trendy Bedford Avenue, the property was prime real estate in Williamsburg. The only obstacles that stood between the landlord and the potential for him to reap large profits were the rent-stabilized status of the building and all the tenants who paid the regulated rent, a fraction of the market rate. We report the first measurement of the transverse momentum dependence of double-spin asymmetries in semi-inclusive production of pions in deep-inelastic scattering off the longitudinally polarized proton. Data have been obtained using a polarized electron beam of 5.7 GeV with the CLAS detector at the Jefferson Lab (JLab). Modulations of single spin asymmetries over the azimuthal angle between lepton scattering and hadron production planes φ have been measured over a wide kinematic range in Bjorken x and virtual photon squared four-momentum Q². A significant nonzero sin2φ single spin asymmetry was observed for the first time indicating strong spin-orbit …
