「敘事醫學倫理」之課程建構及教學經驗, 慧如 林, 心運 王, 春生 賴, 育志 林

目的:近年來,敘事醫學在培養醫療人員的同理心及提升臨床情境的理解能力上,已經逐漸在世界各國得到肯定。然而在台灣目前的醫學教育中,卻仍未有相關正式課程的經驗及成果報導。我們認為在醫學教育的早期階段引進敘事醫學,或許對於往後醫學生的醫學倫理教育會有助益。本文報導高雄醫學大學在98學年度所進行之「敘事醫學倫理」課程的經驗與結果。方法:本研究乃於2010年2月至6月間,在高雄醫學大學,由醫學人文教師與臨床醫師共同合作開設敘事醫學倫理課程。授課對象為醫學院醫學系與呼吸治療學系二年級學生。課程內容包括敘事知識及技巧的教學與實作,以及訪談真實病人故事的團體敘事活動。我們以學生對課程的問卷評值調查、學生的敘事寫作成果(包括學生個別敘事寫作及團體敘事報告)的分析,以及對教師之教學經驗的彙整,評估整體課程的效益及可行性。結果:學生能各自以不同的觀點及敘事手法,完成病人故事的團體敘事報導。學生回饋中也顯示,對於提升對自我主觀性的覺察,以及對他人所敘故事的開放性,此敘事課程確實有其效果。結論:我們對低年級醫學院學生所進行的敘事醫學課程,對於培養敘事技巧、觀點取替,以及主觀性覺察等能力等確實有效且可行。

背景及目的:為能培養出利用全人醫療概念來提供全民口腔醫療照顧的牙醫師,牙醫學系實習醫師會固定至內、外科實習,此訓練應算是醫學院醫、牙整合課程之一大延伸。本研究目的為,評估牙醫學系實習醫師在內、外科訓練成果。方法:台北榮民總醫院一般醫學內科訓練示範中心自2008年起,固定為國立陽明大學牙醫實習醫師做迷你臨床演練(mini-clinical evaluation exercise, mini-CEX)評量。針對評量結果,分析台北榮總施行牙醫實習醫師的mini-CEX早期成效。結果:自2008年1月至2010年7月間,本中心共進行196名牙醫實習醫師之mini-CEX評量。本研究發現此mini-CEX之內部一致信度在0.79到0.86間,且三年來mini-CEX評量之信度有愈來愈來好的趨勢。另外,本研究也發現隨著在內、外科受訓時問之加長,牙醫實習醫師的mini-CEX之加總平均分數有越來越高之趨勢(p value for trend 0.04),且mini-CEX表現進步之幅度以醫療面談及人道專業兩項目最顯著。另外本研究也發現,女性牙醫實習醫師在身體檢查及人道專業之表現,優於男性牙醫實習醫師。相反的,在組織效能方面,男性牙醫實習醫師之表現,優於女性牙醫實習醫師。最後,本研究也發現未到本中心受訓前就曾接受mini-CEX評量之學員,其在本中心之mini-CEX表現則優於其他從未接受過mini-CEX評量之牙醫實習醫師。結論:研究結果顯示,將來要積極規劃牙醫實習醫師,在每一內、外次專科實習期間皆安排mini-CEX的評量,累積多次觀察,可提供更可靠mini-CEX評量表現的結果。另外,評估mini-CEX應平均安排於各內、外次專科實習期間,以便能讓臨床教師及牙醫實習醫師有充裕的時問改進教與學的方法。

In this paper, we describe our solution for ICDM 2010 Contest Task 2 (Jams), where the task is to predict future where the next traffic jams will occur in morning rush hour, given data gathered during the initial phase of this peak period. Our solution, which is based on an ensemble approach, finished Second in the final evaluation.

Mainstreaming Evaluation: Four Case Studies Of Systematic Evaluation Integrated Into Organizational Culture And Practices, Amy M. Gullickson

Following a literature review, a researcher generated a descriptive theory of evaluation mainstreaming, the integration of systematic evaluation into the culture, systems, and job responsibilities of organizations. She then explored the validity and generalizability of the theory in the National Science Foundation’s grant-funded Advanced Technological Education Program using mixed methods research. Four centers were chosen based on quantitative survey responses which suggested that the organizations were likely to be mainstreaming evaluation. This dissertation is intended to evaluate the effects of economic and risk factors on three aspects of sexual and reproductive health: maternal mortality, prenatal care demand, and contraceptive use. The first study of this dissertation discusses the effects of risk factors and socioeconomic determinants on maternal mortality. This research uses a unique, nationwide panel of counties to analyze maternal mortality in Madagascar. Factors like health environment and access to health services were controlled for. The results indicate that female literacy and wages decrease maternal mortality. Other factors related to health infrastructure, and diseases can represent a burden to women's health. …

Energy consumption is an important issue in the design of wireless sensor networks (WSNs) which typically rely on portable energy sources like batteries for power. Recent advances in ambient energy harvesting technologies have made it possible for sensor nodes to be powered by ambient energy entirely without the use of batteries. However, since the energy harvesting process is stochastic, exact sleep-and-wakeup schedules cannot be determined in WSNs Powered solely using Ambient Energy Harvesters (WSN–HEAP). Geochemical, Isotopic, And U-Pb Geochronologic Investigations Of Intrusive Basement Rocks From The Wallowa And Olds Ferry Arc Terranes, Blue Mountains Province, Oregon-Idaho, Gene Alan Kurz

Late Paleozoic to Mesozoic arc terranes comprising the Blue Mountains Province of northeastern Oregon and western Idaho are poorly understood in terms of the timing and duration of recorded magmatism and deformation. Intrusive basement rocks of the Wallowa and Olds Ferry arc terranes are investigated using a combination of highprecision U-Pb geochronology, radiogenic isotope and trace element geochemistry, and structural geology to help constrain their tectonomagmatic evolution and to provide a better understanding for the accretionary history of the Blue Mountains Province.

Quality Of Life As Perceived By Rural Dwellers With A Serious Mental Illness, Deborah G. Townsend

The purpose of this qualitative study is to develop a theoretical model that describes perceptions about quality or life (QOL) and maintenance of mental wellness as held by rural dwellers with serious mental illness (SMI). The National Institute of Mental Health (2006), approximately 1 in 4 adults will have a diagnosable mental illness in any given year and within this population, 6 percent will have SMI. Furthermore, mental bealth providers are scarce in rural areas and isolation and poverty decrease the likelihood of individuals with SMI obtaining services.
