The Extension for Community Healthcare Outcomes (ECHO) Model was developed by the University of New Mexico Health Sciences Center as a platform to deliver complex specialty medical care to underserved populations through an innovative educational model of team-based interdisciplinary development. Using state-of-the-art telehealth technology, best practice protocols, and case-based learning, ECHO trains and supports primary care providers to develop knowledge and self-efficacy on a variety of diseases. As a result, they can deliver best practice care for complex health conditions in communities where specialty care is unavailable. Although oral coinfections (e.g., periodontal disease) are highly prevalent in human immunodeficiency virus type 1-positive (HIV-1+) patients and appear to positively correlate with viral load levels, the potential for oral bacteria to induce HIV-1 reactivation in latently infected cells has received little attention. We showed that HIV-1 long terminal repeat (LTR) promoter activation can be induced by periodontopathogens in monocytes/macrophages; nevertheless, the mechanisms involved in this response remain undetermined. Since Toll-like receptor 2 (TLR2), TLR4, and TLR9 activation have been involved in HIV-1 recrudescence, we sought to determine the role of these TLRs in HIV-1 reactivation induced by …

Herpes simplex virus type 1 (HSV-1) glycoprotein K (gK) and the UL20 protein (UL20p) are strictly required for virus-induced cell fusion, and mutations within either the gK or UL20 gene cause extensive cell fusion (syncytium formation). We have shown that gK forms a functional protein complex with UL20p, which is required for all gK and UL20p-associated functions in the HSV-1 life cycle. Objective This study examines whether men-who-have-sex-with-men (MSM) and transgender (TG) persons’ attitudes, beliefs, and risk perceptions toward human immunodeficiency virus (HIV) vaccine research have been altered as a result of the negative findings from a phase 2B HIV vaccine study. Design We conducted a cross-sectional survey among MSM and TG persons (N = 176) recruited from community settings in Atlanta from 2007 to 2008. The first group was recruited during an active phase 2B HIV vaccine trial in which a candidate vaccine was being evaluated (the “Step Study”), and the second group was recruited after product futility was widely reported …

以病人模擬器及標準化病人進行針對日常例行醫療個案之跨領域團隊合作調練, 金洲 黃, 加璋 黃, 仁峰 梁, 鈴茹 黃, 雅琳 程, 幸榮 林, 肇文 陳

背景:良好的團隊合作是成功施行臨床醫療的基礎。團隊合作訓練最早起源於航空業,之後被應用在麻醉、急診及重症醫學等跨領域的臨床醫療上,一般內容包括上課、討論、及演練。本研究的目地,是希望能為日常例行醫療建立一套新的結合病人模擬器和標準化病人的跨領域團隊合作訓練課程。研究方法:自2010年4月起,我們開始針對日常例行醫療個案舉辦一系列的跨領域團隊合作訓練課程,內容包括醫學模擬操作、回饋和討論,將病人模擬器和標準化病人結合應用在課程之中;參加對象包括醫師、護理師、藥師、放射師、檢驗師及呼吸治療師等職系的老師和學員。在每次的課程結束之後,所有的參與者都需填寫評估問春。結果:總共51位參與者參加本訓練課程,問春的回收率達到100%。有47位(92.1%)參與者對課程感到滿意;教師和學生對課程的整體滿意度相近(P=0.437),但教師認為此課程可幫助了解團隊合作重要性的滿意度高於學生(P=0.027),教師對於學生在演練中表現的滿意度也高於學生自己的滿意度(P=0.002);在不同職系間的整體滿意度並無差異(P=0.287)。結論:病人模擬器和標準化病人可成功的應用於日常例行醫療個案之跨領域團隊合作訓練課程。這是一種新的教學訓練模式,值得推廣。

Histoplasma capsulatum is the most common cause of fungal respiratory infections and can lead to progressive disseminated infections, particularly in immunocompromised patients. Infection occurs upon inhalation of the aerosolized spores, known as conidia. Once inside the host, conidia are phagocytosed by alveolar macrophages. The conidia subsequently germinate and produce a budding yeast-like form that colonizes host macrophages and can disseminate throughout host organs and tissues. Even though conidia are the predominant infectious particle for H. capsulatum and are the first cell type encountered by the host during infection, very little is known at a molecular level about conidia or about …

Background: The human mitochondrial genome includes only 13 coding genes while nuclear-encoded genes account for 99% of proteins responsible for mitochondrial morphology, redox regulation, and energetics. Mitochondrial pathogenesis occurs in HIV patients and genetically, mitochondrial DNA haplogroups with presumed functional differences have been associated with differential AIDS progression.
Methodology/Principal Findings: Here we explore whether single nucleotide polymorphisms (SNPs) within 904 of the estimated 1,500 genes that specify nuclear-encoded mitochondrial proteins (NEMPs) influence AIDS progression among HIV-1 infected patients. OBJECTIVE: To estimate the relationship between maternal methadone dose and the incidence of neonatal abstinence syndrome (NAS).
STUDY DESIGN: We performed a retrospective cohort study of pregnant women treated with methadone for opiate addiction who delivered live-born neonates between 1996 and 2006. Four dose groups, on the basis of total daily methadone dose, were compared (160 mg/d). The primary outcome was treatment for NAS. Symptoms of NAS were objectively measured with the Finnegan scoring system, and treatment was initiated for a score>24 during the prior 24 hours.

Objective: This study aims to provide a rich analysis of particular women’s medical education experiences.
Design: One-on-one interviews and self-administered questionnaires Participants: 25 practicing women physicians who had graduated from U.S. medical schools.
Results: The author identified the following themes: 1) societal gender role assumptions significantly impact women physicians’ experiences as medical students, in practice, and as primary care givers. 2) Marginalities in women’s health education exist in all levels of medical training. OBJECTIVE: To evaluate the clinical characteristics and certain risk factors that may be associated with fatal outcome in patients with H1N1 influenza. METHODS: This retrospective study was conducted between October and December 2009 in the Department of Infectious Diseases and Clinical Microbiology, SB Diskapi Yildirim Beyazit Training and Research Hospital, Ankara, Turkey. Data regarding the epidemiological and clinical characteristics of 74 hospitalized cases of confirmed pandemic H1N1 influenza were reviewed. Intranasal application of vesicular stomatitis virus (VSV) induces acute encephalitis characterized by a pronounced myeloid and T cell infiltrate. The role of distinct phagocytic populations on VSV encephalitis was therefore examined in this study. Ablation of peripheral macrophages did not impair VSV encephalitis or viral clearance from the brain, whereas, depletion of splenic marginal dendritic cells impaired this response and enhanced morbidity/mortality. Selective depletion of brain perivascular macrophages also suppressed this response without altering viral clearance. Thus, two anatomically distinct phagocytic populations regulate VSV encephalitis in a non-redundant fashion although neither population is essential for viral clearance in the CNS. …

The Qualitative Report
It is well known that the pathways to homelessness for young people are embedded in often ongoing negative childhood experiences. Many of these experiences are rooted in multiple and intersecting problems including, but not limited to: family conflict, abuse, addictions, and mental health issues. The authors draw upon qualitative interviews conducted with 15 homeless male and female youth between the ages of 16 and 24 in a suburban area of Southern Ontario, Canada. We describe these young people's perceptions of family experiences and find support for Elliott Currie's (2004) proposition that a broader ethos of individualism and intolerant parenting underpins …

Osteomyelitis in newborn infants is a rare infection. Lower extremity joints are commonly affected. Most of the cases have a haematogenous spread. Aerobes are the common group of organism involved, of which Staphylococcus aureus is the commonest. Klebsiella osteomyelitis has been reported as a cause of Osteomyelitis. However, to the best of our knowledge, this is the first case report of Klebsiella pneumoniae associated osteomyelitis in an infant from Pakistan.

iardia lamblia and Cryptosporidium parvum are both waterborne pathogens associated with diarrhoea in developing countries. In a recent study based at the Aga Khan University in Karachi, 334 adults aged 16–83 years (178 patients with chronic diarrhoea and 156 diarrhoea-free volunteers who acted as controls) were checked for infection with these parasites, using stool microscopy and/or PCR. Overall, 21 (6.3%) and 29 (8.7%) of the subjects were found positive for G. lamblia by microscopy and PCR, respectively, while the corresponding values for C. parvum were 13 (3.9%) and 14 (4.2%). HIV-1 integration is mediated by the HIV-1 integrase protein, which joins 3'-ends of viral DNA to host cell DNA. To complete the integration process, HIV-1 DNA has to be joined to host cell DNA also at the 5'-ends. This process is called post-integration repair (PIR). Integration and PIR involve a number of cellular co-factors. These proteins exhibit different degrees of involvement in integration and/or PIR. Some are required for efficient integration or PIR. With the exponential growth of Facebook users worldwide, this platform for social network online has become a powerful tool to connect individuals and share information with each other. This study explores the phenomenal trend of utilizing a Facebook application called Causes to help users organize into online communities for a specific cause and mobilize their resources for disaster relief during the Haiti earthquake disaster.
