Yale Medicine Thesis Digital Library
THE FDA BLACK BOX WARNING SYSTEM: THE UTMOST IN DRUG AND PATIENT SAFETY? Andrew Georgi (Sponsored by Paul Barash, MD, and Loreta Grecu, MD). Department of Anesthesiology, Yale University School of Medicine, New Haven, CT. Of the 7856 pharmaceutical products under the purview of the Food and Drug Administration (FDA), over six hundred have a black box warning (BBW). As the FDAs highest level of warning included on drug package inserts, this notation is reserved for those drugs that pose significant risks leading to death or serious injury. However, the types of warnings that warrant this distinction have not been …

Green Practices For Surgical Units, Gifty Kwakye
Yale Medicine Thesis Digital Library
The study aimed to identify leading practices to promote environmentally friendly and efficient efforts in surgical healthcare. Despite widespread enthusiasm for going green in the U.S. economy, little information is available to inform the medical community on the effort. We explore safe and efficient strategies for hospitals and healthcare providers to protect the environment while delivering high-quality care. As part of the study design, we performed a systematic review of the literature using relevant Pubmed search terms and surveyed a panel of hospital managers and CEOs of healthcare organizations pursuing green initiatives. Recommendations were itemized and reviewed with each panelist …

Dartmouth Scholarship
Malaria, caused by the apicomplexan parasite Plasmodium, threatens 40% of the world's population. Transmission between vertebrate and insect hosts depends on the sexual stages of the life-cycle. The male gamete of Plasmodium parasite is the only developmental stage that possesses a flagellum. Very little is known about the identity or function of proteins in the parasite's flagellar biology. Here, we characterise a Plasmodium PF16 homologue using reverse genetics in the mouse malaria parasite Plasmodium berghei. PF16 is a conserved Armadillo-repeat protein that regulates flagellar structure and motility in organisms as diverse as green algae and mice. We show that …

CD23 has long been appreciated to be a natural, negative regulator of IgE synthesis. This understanding is due in part to animal models in which CD23 deficient or CD23 transgenic animals display exacerbated or reduced IgE levels respectively. Interestingly, CD23 is susceptible to proteolytic cleavage from the cell surface. When this occurs, CD23 loses its regulatory capability and the solubilized form can lead to pro-inflammatory events through its cytokinergic activity on macrophages. Thus, targeting this specific cleavage would be beneficial to the control of allergic disease by stabilizing CD23 at the cell surface. Inhibitor studies performed by our group as …

Background
Because they can generate comparable predictions, mathematical models are ideal tools for evaluating alternative drug or vaccine allocation strategies. To remain credible, however, results must be consistent. Authors of a recent assessment of possible influenza vaccination strategies conclude that older children, adolescents, and young adults are the optimal targets, no matter the objective, and argue for vaccinating them. Authors of two earlier studies concluded, respectively, that optimal targets depend on objectives and cautioned against changing policy. Which should we believe?

Section of Rheumatology, Department of Internal Medicine, Yale University School of Medicine, New Haven, CT. The specific aims of this study were to characterize: 1) the impact of the pediatric rheumatology workforce shortage from the perspective of pediatric rheumatologists and general pediatricians, and 2) the acceptance of telemedicine, including both video conferencing and store and forward technology, as a potential solution for the pediatric rheumatology workforce shortage. We hypothesized that physicians practicing in settings with decreased access to pediatric rheumatology would perceive …

Relationship Quality And Men's Oxidative Stress, Leslie A. Merriman
Relationship Quality And Men's Oxidative Stress, Leslie A. Merriman
Psychology ETDs
The association between oxidative stress and quality of romantic relationships was investigated in a sample of 98 college males. Given a postulated life history trade-off between current and future reproductive potential, men currently in higher quality romantic relationships may expend less general mating effort (i.e., less energy allocation to finding, attracting, and competing for new mates) than single men or men in lesser quality relationships. Reduced mating effort may allow greater allocation of energy to anti-oxidant defense systems, and increased resistance to oxidative damage by reactive oxygen species (ROS). Consistent with this prediction, men who reported being in higher quality …
Human Development In Poor Countries: Bangladesh In Comparative Perspective, Nafisa Halim
Human Development In Poor Countries: Bangladesh In Comparative Perspective, Nafisa Halim
Sociology ETDs
My dissertation addresses the developmental paradox' of rapid gains in human development in Bangladesh against the backdrop of a social scientific literature that portrays two of the country's endemic features—Islam and illiberal politics—as inveterate obstacles to their achievement. I use data from household surveys conducted by the Bangladesh Institute of Development Studies in collaboration with the World Bank in 1991-2 and 1998-9, and identify the distal determinants of the Bangladeshi mortality decline. I find female education has a linear effect and household income has a curvilinear effect on infant and child mortality. Why is the relationship between income and child …

Student Volunteer Opportunities In Hydrophilanthropy: The Steps To Organizing A Successful Project, Cathleen M. Fitzgerald
Journal of Contemporary Water Research and Education
University students who travel internationally become de facto ambassadors not only of their country but also of their universities and colleges. Therefore, it is important that all hydrophilantropy projects include awareness of cross-cultural differences and include community participation and involvement during the early planning stages. Schedules should not be so ambitious that the project cannot be completed in the allotted time period and multiple trips to the same community are essential to assess the effectiveness and sustainability of completed projects. Low-tech water supply technologies that are affordable, easily replicable, and require minimal or no outside monetary assistance are the keys …

PURPOSE OF REVIEW: To provide clinical investigators with an understanding of factors to consider when wishing to add cytokine and inflammatory marker measurements to their studies. RECENT FINDINGS: Inflammation involves complex and coordinated responses of the immune system to tissue damage. In the absence of tools to routinely assess inflammation within living tissues, measurements of humoral factors such as cytokines and other inflammatory mediators or markers can provide predictive clinical information and insights into disease mechanisms. Historically, enzyme-linked immunosorbent assays (ELISAs) became the gold standard, yet this approach of measuring a single protein in each sample limits the amount of …

Concurrent Sexual Partnerships Do Not Explain The Hiv Epidemics In Africa: A Systematic Review Of The Evidence, Larry Sawers, Eileen Stillwaggon
Concurrent Sexual Partnerships Do Not Explain The Hiv Epidemics In Africa: A Systematic Review Of The Evidence, Larry Sawers, Eileen Stillwaggon
Economics Faculty Publications
The notion that concurrent sexual partnerships are especially common in sub-Saharan Africa and explain the region’s high HIV prevalence is accepted by many as conventional wisdom. In this paper, we evaluate the quantitative and qualitative evidence offered by the principal proponents of the concurrency hypothesis and analyze the mathematical model they use to establish the plausibility of the hypothesis.
We find that research seeking to establish a statistical correlation between concurrency and HIV prevalence either finds no correlation or has important limitations. Furthermore, in order to simulate rapid spread of HIV, mathematical models require unrealistic assumptions about frequency of sexual …
