Lung cancer is the leading cause of cancer death in the United States and the world, with more than 1.3 million deaths worldwide per year. However, because of a lack of effective tools to diagnose Lung Cancer, more than half of all cases are diagnosed at an advanced stage, when surgical resection is unlikely to be feasible. The field of nursing currently faces a shortage of both nurses and nursing faculty. Yet, little is known about why clinical nurses opt to become nursing faculty. Qualitative investigations into this phenomenon are particularly rare. This study addressed these gaps in the context of career choice of faculty associate degree nursing (ADN) programs. The qualitative analysis was guided by Astin's (1984) psychosocial model of career choice. The central research question was, "What factors influence the career choice of nurse educators currently teaching in Kentucky associated degree nursing programs?" This multilayered study is an appraisal of some of the ongoing post-conflict reconstruction efforts in Sierra Leone in light of the causes of the conflict, which lasted from 1991-2002. It begins by providing a critical examination of the historical antecedents leading up to the war. The problems of colonialism which brought together different nationalities under one political umbrella by force of arm, along with other motley of issues such as corrupt leadership and inter-ethnic violence led to the outbreak of war in 1991. The creation of liminal spaces has been used for centuries cross-culturally to create sacred or taboo meanings in rituals, people, places, or objects. Liminality is constructed by the overlapping of cultural categories and "ruptures" an object or event's connotations, imbuing it with new and often transcendent or spiritually charged meaning. The photograph is an example of liminal space, since it freezes even the living in a state of corpse-like stasis. Alternative school settings for students who are identified as "disruptive or dangerous" are playing an increasingly prominent role in the world of public education, yet many gaps in the research literature are abound. This dissertation study is an effort to contribute to an understanding of the students placed in these alternative schools, identify longitudinal predictors of placement, and assess the risk of subsequent involvement of the juvenile justice system. The pulvinar is the largest nucleus of the human dorsal thalamus and is affected in a variety of brain disorders, such as schizophrenia. The experiments described in this dissertation elucidate key features of tecto-pulvino-cortical pathways as a first step toward understanding their role in coding visual stimuli and coordinating appropriate responsive actions. The tree shrew is used as the animal model because the visual structures of the tree shrew brain display many of the features of the primate brain, and the tectopulvinar pathways are particularly enhanced in this species. Described as important long-term consequences of trauma exposure, disruptions in emotional processes are regarded as central features of posttraumatic stress disorder (PTSD); however, the nature of these posttraumatic emotional disruptions remains poorly understood. The present study attempted to further understanding of these emotional disruptions by using experience sampling methodologies and electronic diaries to assess experiences of emotion, stress, and PTSD symptoms as they occurred in the day-to-day lives of midlife women exposed to intimate partner violence. Three sets of hypotheses were advanced and were tested using multilevel modeling approaches, with results demonstrating partial support for hypotheses overall. First, although past-month …

This thesis is divided into two parts below. Part I Adriamycin (ADR) can produce nephrotoxicity in rodents. The underlying mechanism may relate to ADR induced oxidative stress. In this study, we used transgenic mice (NMT3), which over-expressed the antioxidant protein metallothionein (MT) in podocytes, to study MT's protective potential on ADR nephrotoxicity. Urine and kidney samples were collected from control and transgenic mice at multiple time points after ADR injection, whose results showed that MT transgene alleviated ADR damage by reducing albuminuria, decreasing podocyte loss and protecting podocyte ultra-structure. The purpose of this study was to assess the effects of short-term high-intensity resistance training on time to exhaustion at maximal aerobic output. It was hypothesized that 2 weeks of training will induce improvements in performance at maximal aerobic output. Eighteen subjects participated in this study. Subjects were assigned to a control (n=8) or resistance training (n=10) groups. The resistance training group participated in 5 sessions of strength training over a period of 2 weeks on the leg press device. Both groups participated in pre and post intervention testing. After the intervention strength in the training group significantly increased (p=.00). …

Tobacco smoke is a strong and independent risk factor for several chronic systemic diseases and also increases susceptibility to a multitude of bacterial infections, including periodontal infections. Periodontitis is a chronic inflammatory disease of the supporting tissues of the periodontium caused by its chief etiological agent, Porphyromonas gingivalis . Smokers are more prone to persistent infections by P. gingivalis , and harbor higher numbers of P. gingivalis than non-smokers. However, smokers show reduced clinical signs of inflammation compared to non-smokers, making diagnosis of periodontal disease in smokers problematic. Evidence suggests that cortical minicolumns are reduced in size and increased in number in individuals with autism spectrum disorder (ASD), especially in the dorsolateral prefrontal cortex (DLPFC). More specifically minicolumns in individuals with ASD are narrower and contain less peripheral, neuropil space; this may cause an increase in the ratio of cortical excitation to inhibition and adversely affect the functional distinctiveness of minicolumnar activation. A lack of cortical inhibition may cause signal/sensory amplification which can impair functioning, raise physiological stress, and adversely affect social interaction in patients with ASD. Osteoarthritis (OA) of the knee is associated with decline in functional capacity and ultimately leads to Total Knee Arthroplasty (TKA) in many of these patients. Exercise regimens prior to surgery may potentially enhance pre and post TKA functional performance. However, assessment of such performance should involve biomechanical factors that characterize the mechanisms with which tasks are performed, and not just the quantity of task performed. Introduction: Osteoarthritis (OA) is a clinical condition affecting over 27 million Americans. There is no known cure for OA other than replacing the diseased joint with a joint prosthesis, total knee arthroplasty (TKA). Purpose: This study compared pre and post-surgical functional ability, exercise self efficacy and outcome expectations among total knee arthroplasty (TKA) patients who did and did not receive a prehabilitation exercise program. The Kosair Children's Hospital, located in Louisville, Kentucky, is the only children's hospital in the state. Recently, the hospital's emergency department has developed concerns with patient satisfaction. Patients have experienced excessive amounts of time to get through the emergency department resulting in low satisfaction. Hence, this research assesses the patient's time in the emergency department through the use of a computer simulation. This study investigated teachers' inclusion of racially diverse literature within their classroom libraries, how the representations within the literature compared with the students in the classrooms, the inclusivity of students' book selections, and teacher thoughts concerning selection of literature. Individual classrooms were found to fall within categories of challenging, progressing against, or continuing the selective tradition, the tendency of teachers to favor literature that primarily features White, middle-class, male authors and subjects over works by and about women and other ethnicities or social classes (Luke, Cooke, & Luke, 1986). Mucin 1 (MUC1) is a plasma membrane-bound glycoprotein that plays a protective role in corneal epithelial cells. Two full-length splice variants of MUCl: MUCl/B and MUCl/A, that differ by the inclusion of 27 bp from intron 1 and a SNP in MUCl/A, but have identical C-terminal cytoplasmic domain (CD) sequences, were identified in human conjunctival tissue. I tested the hypothesis that MUC1 splice variants are key immunoregulators that act on the ocular surface to protect the ocular surface from inflammation and that their expression correlates with dry eye status. Representations Of Homelessness In Four Canadian Newspapers: Regulation, Control, And Social Order, Barbara Schneider, Kerry Chamberlain, Darrin Hodgetts
The Journal of Sociology & Social Welfare
This article reports on a content analysis of homelessness representations in four Canadian newspapers: two city broadsheets, one city tabloid, and one national newspaper. Clear differences between the papers emerged showing that in general coverage of homelessness in Calgary was much more positive than coverage in Vancouver. It conveyed a stronger sense of crisis or urgency and a stronger sense of optimism that the problem should and can be solved. Experts dominate public discourse about homelessness, with people who experience homelessness themselves marginalized as speakers. Despite these differences, the four papers present a unified narrative of homelessness in which readers …

The Economics Of Being Young And Poor: How Homeless Youth Survive In Neo-Liberal Times, Jeff Karabanow, Jean Hughes, Jann Ticknor, Sean Kidd, Dorothy Patterson

Based upon in-depth interviews with 34 youth in Halifax and seven service providers in St. John's, Montreal, Hamilton, Toronto, Winnipeg, and Calgary, the findings of this study suggest that labor occurs within a particular street context and street culture. Formal and informal work can be inter-related, and despite the hardships they experience, young people who are homeless or who are at-risk of homelessness can respond to their circumstances with ingenuity, resilience and hope. Often street-involved and homeless young people are straddling formal and informal work economies while mediating layers of external and internal motivations and tensions. The reality is that …

Housing For People With Serious Mental Illness: Approaches, Evidence, And Transformative Change, Geoffrey Nelson
The Journal of Sociology & Social Welfare
The evolution of housing approaches for people with serious mental illness is described and analyzed. A distinction is made between three different approaches to housing: (a) custodial, (b) supportive, and (c) supported. Research evidence is reviewed that suggests the promise of supported housing, but more research is needed that compares supported housing with different supportive housing approaches. It is argued that the current move to a supported housing approach represents a fundamental shift or transformative change in mental health policy and practice. Strategies to facilitate this shift are discussed.

Fiberoptic intubation skills (FOI) are critical in reducing the anesthesia related morbidity and mortality in clinical settings. The purpose of the study was to prove that the simulator can train a novice to achieve the expert level in a relatively short time. The performance of the pre- and post-training of novice group and expert group was computer or video recorded. Three statistical methods were applied for data analysis. Complement, an early recognition system of innate immunity that senses local tissue damage and infection, cross-talks with and regulates other signaling systems, including Toll-like receptor (TLR) pathways. In the context of periodontitis, destructive inflammation and disease promotion are associated with extensive and synergistic activation of TLRs and complement within the chronically inflamed periodontium. The virulence of the periodontal pathogen Porphyromonas gingivalis is dependent, at least in part, upon its ability to use sophisticated stealth and sabotage tactics to undermine innate immunity. The Role Of Genetic And Environmental Oxidative Stress Factors In Prostate Cancer., Nicole A. Lavender

Prostate cancer (PCA) development may be influenced by genetic variations within oxidative stress response (OSR) related mechanisms, such as antioxidation (e.g., carcinogen metabolism/detoxification), DNA repair, and apoptotic regulation. Excessive oxidative stress can produce DNA base changes, damage tumor suppressors, enhance proto-oncogene expression, and induce malignant transformation of cells. Persistent oxidative stress may even trigger apoptosis. Environmental reactive oxygen species (ROS) exposure attributable to lifestyle factors may exacerbate this situation by increasing oxidative stress. Therefore, it is likely that genetic variation resulting in compromised ROS capacity combined with increased environmental ROS exposure may increase PCA risk and disease aggressiveness. The Oldham County Learning Institute (OCLI), a 3-day professional development experience for teachers, was provided during four sessions spanning two school years. The OCLI training focused on three interrelated strands: thinking strategies – what to do, gradual release of responsibility – how to do it, and building a classroom community of learners – in what context. This study investigated the impact of OCLI on teachers’ instructional practices with regard to implementation of the three strands.
