Delayed onset muscle soreness (DOMS) refers to skeletal muscle pain that follows strenuous and unaccustomed physical activity. The intensity of discomfort peaks at 24 to 48 hours, and subsides within 5 to 7 days post-exercise. It appears that DOMS occurs most frequently following unaccustomed eccentric exercise. However, it is not clear whether sensitivity to pain is uniform over the surface of the muscle, or whether some locations become more sensitive following eccentric exercise. The aim of this study was to investigate the immediate effects of upper extremity loading exercises on conduction in the median and ulnar nerves in the wrists of hemiplegic patients. Seventeen hemiplegic patients without past history of carpal tunnel syndrome or other confounding diseases and seventeen healthy and physically-matched control subjects were recruited. Nerve conduction tests of the median and ulnar nerves were performed in both arms of patients and healthy controls before and after a set of 30-minute bilateral loading exercise. Spinal arteriovenous malformations is a rare disease. The initial signs are usually in the forms of nonspecific sensory symptoms such as pains and parasthesia; therefore, it is difficult to conclusively diagnose this disease. Here, we report the case of a 30-year-old woman who suffered from a sudden weakness in the 4 limbs 15 years ago. The symptoms subsided after supportive care; she was discharged from a local hospital without further follow ups. Approximately 5 years ago, she had a sudden attack of pains in her left shoulder. High-resolution ultrasonography is a useful tool for the localization of non-palpable soft-tissue foreign bodies. A 21-year-old female underwent a routine health examination, during which chest radiography revealed a radio-opaque linear foreign body. The foreign body was located within left posterior chest wall, 4 to 5 cm lateral to the T8 spinous process. The shape indicated a needle. The possible presence of a needle in the chest wall meant that an operation to remove it was imperative. Muscle spasticity is a common problem that affects patients who have suffered spinal cord injury (SCI). It can affect the motor skills, activities of daily living, and quality of life of these patients. The modified Ashworth scale (MAS) is a widely used clinical rating scale for spasticity, but its validity and reliability have been questioned. Much attention has been devoted to the evaluation of spasticity by using electrophysiological approaches. Increased excitation of motor neurons, exaggerated stretch reflex, decreased presynaptic inhibition, and reduced reciprocal inhibition have been found to be associated with spasticity. Early damage involving the structures of visual perception in the brain will give rise to visual perceptual impairment. Lymphedema, the most significant complication of treatment for breast cancer, can result in substantial psychological and physical morbidity. However, no long-term follow-up lymphedema studies yet exist for Taiwanese breast cancer survivors. This study therefore focused on examining frequency and risk factors of lymphedema secondary to breast cancer in Taiwanese patients. Five hundred and seventy patients who received surgical treatment between 1994 and 2000 at Changhua Christian Hospital were retrospectively investigated in October 2002. The incidence of self-reported edema was 28 percent (161/570), and the incidence of lymphedema with circumference difference greater than 2cm was 8.1 percent (46/570). Eighty-four percent of …

Orthostatic hypotension (OH) is a common clinical problem for patients with spinal cord injury (SCI) at the cervical or high thoracic level when they move from the supine position to the upright position. Patients with severe SCI frequently suffer from symptoms of OH during head-up tilt, ranging from slight lightheadedness to full syncope. Tilt-table training is commonly used in clinical physiotherapy to overcome OH syncope, but this also relies on carefully manual control of the tilt angle and training time by the therapist during training. The purpose of this study was to design an insole-style testing piece for a quick and easy dynamic foot pressure test. Current commercial foot pressure systems are costly and complicated to operate. Neurotransmitter dopamine has relation with neuro-psychiatric disease, it also has effect on motor function for healthy individuals and also determines the outcome of rehabilitation for neuro-psychiatric disease patients. Finger Tapping Test (FTT), a neuro-psychological test, is thought to be associated with dopamine. Therefore, the goal of this study is to investigate the association relationship between dopamine genes polymorphism (DRD2 TaqI) and FTT. We report a case of an adult patient with post-traumatic hydrocephalus who was treated using a ventriculoperitoneal (VP) shunt with a fixed pressure setting valve. Slit ventricle syndrome was found two months later and an antisiphon device (ASD) was added. A fixed pressure valve with ASD was found not to be optimal in terms of improving his neurological symptoms and a programmable valve system was therefore implanted with a pressure setting of 140 mmH2O in the beginning. The valve then was gradually programmed to reduce down to 70 mmH2O over two months. This case demonstrates that selection of a shunting …

Plugged ducts are a common problem during lactation. They often occur when the breast is not emptied properly, or when there is local pressure on the breast. Plugged ducts usually occur three to four weeks postpartum. Symptoms include a sensitive lump with well-defined margins in the breast. There may be pain and tenderness in the affected area, although this is not necessarily accompanied by systemic symptoms such as high-grade fever. The bicipitoradial bursa is located between the distal biceps tendon and the radial tuberosity. The bursa functions as a buffer to reduce the friction between tendon and bone during pronation of the forearm. Bicipitoradial bursitis most frequently results from repetitive mechanical microtrauma by supination and pronation movements of the forearm, and it may cause painful swelling at the antecubital fossa. However, only a few cases of bicipitoradial bursitis have been reported to be diagnosed by ultrasound. Bladder compliance expresses the relationship between bladder volume and detrusor pressure and is generally regarded as a measure of bladder stiffness or distensibility. Based on the varied selection criteria during the urodynamic study, differences in bladder compliance often exist. Segmental zoster paresis is a rare complication of cutaneous herpes zoster. Clinical diagnosis of segmental zoster paresis depends upon a history of painful vesicular eruptions in a dermatomal distribution, followed by muscle weakness in the related myotomes. We report the case of a 45-year-old woman with opticospinal type multiple sclerosis with recent cutaneous herpes zoster and right shoulder girdle weakness. Differential diagnosis included multiple sclerosis relapse, zoster myelitis, segmental zoster shoulder paresis, cervical radiculopathy, and shoulder joint or soft-tissue disorders. Intravenous pyelography (IVP) is a common tool for evaluating the upper urinary tract in spinal cord injury patients. To obtain a proper image, bowel preparation is required. Among the regimens for bowel preparation, polyethylene glycol (PEG) electrolyte lavage solution has a good cleansing quality and fewer side effects. Nonetheless, adverse effects of the regimen, though rare, have been reported. Background/Purpose: Disturbances in sleep and health-related quality of life (HRQL) are common in hemodialysis patients. The aim of this study was to investigate the effects of high voltage galvanic stimulation (HVGS) on blood chemistry, sleeping quality and HRQL among hemodialysis patients.Methods: This is an experimental one way repeated measure design. This study investigated whether a stroke patient with left hemianopsia, which was confirmed by the perimetric test, may also have hemineglect. The relationship between hemineglect and hemianopia in patients with right hemispheric stroke was evaluated. Eleven subjects with left unilateral neglect underwent three testing sessions separated in time. The three test sessions, including cancellation and static perimetric tests, were performed at approximately 3 months, 4 months, and 7 months post the onset of the stroke. The main causes of cervical spine injury vary among different age groups. Elderly people have a tendency to sustain injuries due to falls, which is a low energy mechanism of injury. Here, we report a case of a 76-year-old woman who fell down from sitting and suffered severe dislocation of the C5 on the C6 vertebra and locking of the bilateral facet joints. She showed spontaneous reduction of the dislocation on a magnetic resonance image taken 3 h after the injury.
