There is a physician shortage in eastern Kentucky because older physicians are retiring, and younger physicians are choosing to practice in more urban, affluent areas. Eastern Kentucky is plagued with persistent poverty and lacks cultural amenities. These facts put together myths regarding rural practice create a shortage.

My research consisted of interviewing twenty physicians currently practicing at Highlands Regional Medical Center in Prestonsburg, Kentucky. I created a survey instrument that asked questions of physicians regarding demographics characteristics, medical school attended, and what motivated them to practice at Highlands. Public funding of substance abuse treatment (SAT) in Kentucky dates back to the 1950s when legislators sought to curb the problem of alcoholism through legislative acts. The definition of substance abuse expanded through the years to include other substances such as cocaine, marijuana and opiates.

Employment after SAT is a critically important outcome for policymakers to consider when allocating funds because it assists in social re-integration, helps to prevent relapse and promotes economic self-sufficiency. Because of this, employment is an important factor to consider when assessing the impact SAT has on its clients (participants).

Head Start, a nationwide early childhood program, promotes the development and use of locally-designed program models, such as partnerships with child care centers, to expand enrollment opportunities and improve service delivery to eligible children and families. Partnerships are considered to be an efficient solution with positive benefits accruing to the Head Start grantee, partnering child care center, and participating families. It is not known, however, whether partnerships differ from standard, direct-managed centers in terms of promoting school readiness among children, the goal of Head Start. Sanctions, Countermeasures, And The Iranian Nuclear Issue, N. J. Calamita

The international community's response to Iran's nuclear development program highlights the sometimes complex legal relationship between the UN system of collective security and the rights of states to take unilateral countermeasures under the law of state responsibility. It also raises a number of important questions about (a) the discretion afforded to states in the interpretation and implementation of Security Council resolutions, (b) the availability of countermeasures for the violation of multilateral obligations, and (c) the exclusivity of the Chapter VII framework for collective security.

Nafta Cross-Border Trucking: Mexico Retaliates After Congress Stops Mexican Trucks At The Border, Chad Macdonald

The North American Free Trade Agreement (NAFTA) entitles Mexican tractor-trailers to enter the U.S. to deliver cargo from Mexico. In spite of NAFTA, the U.S. has only allowed Mexican trucks to operate in the U.S. during a controversial demonstration project that granted U.S. operating licenses to a select number of Mexican trucks during the Bush administration. The dispute over NAFTA's cross-border trucking provisions climaxed on March 16, 2009, when Mexico imposed $2.4 billion in retaliatory tariffs on U.S. imports in response to U.S. noncompliance.

Following the implementation of the State Children’s Health Insurance Program (SCHIP), rural health researchers noted that this public insurance expansion had the potential to dramatically improve health insurance coverage for rural children.1 At the time, rural children were more likely than their urban counterparts to be uninsured, and also were more likely to have family incomes in the range targeted by SCHIP (100- 200% of Federal Poverty Level-FPL).2

Approximately 2 to 16 percent of the general population has a disruptive behavior disorder such as oppositional defiant disorder (ODD) or conduct disorder (CD) (Corcoran & Walsh, 2006). In a school setting, youth with these disorders may engage in acts such as disrespecting teachers, swearing at others, threatening others, assaulting staff or students, stealing, or vandalizing property and may benefit from a day treatment program which provides mental health services in a school setting. In the United States, truancy has been identified as one of today’s top ten educational problems (Zhang, Katsiyannis, Barret& Wilson, 2007). Across the country, every day there are hundreds of thousands truant students (U.S. Department of Education, 1996). Attendance can be influenced by mental health issues, substance abuse, teenage pregnancy, student employment, family responsibilities, teacher attitudes, size of school, and inconsistent application of truancy policies and lack of meaningful consequences for truancy. West Nile Virus (WNV) made its initial appearance in the New York City (NYC) metropolitan area in 1999 and was implicated in cases of human encephalitis and the extensive mortality in crows (Corvus sp.) and other avian species. Mosquitoes were found to be the primary vectors and NYC’s current policy on control strategies involved an eradication program that depends on the synchronicity of the summer mosquito population’s increases with the occurrence of cases in humans. The discussion in this paper elaborates a tiered set of reforms designed to (1) protect foreign investment, while (2) affording appropriate policy space for governments to develop and regulate their economies in a sustainable manner and (3) ensuring equitable governance of investment disputes such that foreign investors are not privileged, procedurally or substantively, over domestic investors and citizens. The paper includes a detailed appendix with proposed reforms to the NAFTA Chapter 11 text.

The present study examined child sexual abuse in the Boy Scouts of America (1970 to 1984). In particular, this study explored the role of the institution in child sexual abuse. The data analyzed was gathered from the Boy Scouts Ineligible Volunteer File system and included a sample of 529 adult males who had volunteered to serve as adult role models for the Boys Scouts and were suspected of alleged misbehavior. Analysis of the data revealed a profile of the alleged abusers. These men were typically in their mid-30s, single and had spent an average of 41.4 months in the Boy …

Rural-Urban Differences In Health Care Access Vary Across Measures, Erika C. Ziller Phd, Jennifer D. Lenardson Mhs

Rural uninsured rates are higher than urban,1 and the uninsured often have difficulty obtaining needed care.2 Difficulties recruiting and retaining health care providers have resulted in longstanding disparities in rural and urban physician supply.3 This combination of factors suggests that rural residents may face greater barriers to accessing health care than their urban counterparts. Analyses of data from the 2006 Medical Expenditure Panel Survey (MEPS) for non-elderly rural and urban residents partly supports this premise, yet rural residents fare better on some measures of access.

Background

Physical maps have been historically one of the cornerstones of genome sequencing and map-based cloning strategies. They also support marker assisted breeding and EST mapping. The problem of building a high quality physical map is computationally challenging due to unavoidable noise in the input fingerprint data.

Results

We propose a novel compartmentalized method for the assembly of high quality physical maps from fingerprinted clones. The knowledge of genetic markers enables us to group clones into clusters so that clones in the same cluster are more likely to overlap. For each cluster of clones, a local physical map is first …
