Rehabilitation Practice and Science
The purpose of this study was to investigate whether the differences in nerve conduction, soft tissues, and joint angles of the elbow exist between the bilateral arms of college tennis players and their controls. Sixteen healthy high-ranking tennis players and 15 controls without any history of pain in their upper extremities were recruited. Nerve conduction tests, ultrasonography, and X-ray evaluation were conducted on the elbows bilaterally of the players and controls. Results of the players showed that the sensory amplitudes of the ulnar nerve of the dominant arm were decreased (p=0.003). In addition, the thickness of the ulnar collateral ligament of …

Cardiopulmonary Function And Exercise Capacity In Children With Kawasaki Disease, Yu-Ling Wang, Ai-Lun Yang, Jue-Long Wang, Chun-Han Yang, Zheng-Yu Hoe, Chien-Ming Huang, Ko-Long Lin
Rehabilitation Practice and Science
Kawasaki disease complicated by coronary artery disease is the leading cause of acquired heart disease in children. Maximal anaerobic capacity and factors of restrictive exercise capacity need to be evaluated. To study the influence of Kawasaki disease on cardiopulmonary function and exercise capacity, sixteen children were recruited for the experimental group and given a cardiopulmonary exercise stress test. Sixteen healthy children were recruited as the control group. Based on the findings, the VO2(subscript max) (8.06±2.13/10.24±2.39MET, p=0.01) and the maximal systolic blood pressure (144.19±27.39/163.40±21.64mmHg, p=0.02) demonstrated statistically significant differences between children with Kawasaki disease and healthy children, but no significant difference in …

Femoral Nerve Injury Following Transfemoral Angiography: A Casereport, Chien-Ching Yu, Ying-Ju Shih, Su-Ju Tsai
Rehabilitation Practice and Science
Femoral nerve injury is a rare complication after femoral angiography. Symptoms including quadriceps weakness and atrophy, absent or reduced knee jerk and sensory impairment along the anterior thigh and medial lower leg have been found. This leads to both muscular weakness and sensory deficit, which impairs walking and interferes with quality of life. Early identification and management of femoral nerve injury could result in better prognosis. We report on the history, diagnosis and rehabilitative outcome of a 54-year-old woman with a medical history of peripheral arterial occlusion disease. She developed significant femoral neuropathy one month after angiography. Severe neurological deficits were …

Clinical Features And Rehabilitation Of Systemic Lupus Erythematosus Complicated With Moyamoya Syndrome: A Casereport, Wei-Chih Lien, Jo-Tong Chen, Shu-Cheng Lin, Nai-Wen Guo, Da-Shen Kuan
Rehabilitation Practice and Science
Moyamoya disease is an uncommon clinical entity characterized by progressive narrowing and occlusion of the bilateral distal internal carotid arteries, as well as the proximal parts of their major branches, namely, the middle and anterior cerebral arteries. Concomitant development of prominent collateral vascularity in the basal regions of the brain results in the typical angiographic appearance of moyamoya, which literally means a wavering puff of smoke. Moyamoya syndrome refers to findings of moyamoya vasculopathy in the context of known systemic disease. We present a 26-year-old woman with systemic lupus erythematosus who had several episodes of ischemic stroke. Her cerebral angiograms …

Spontaneous Spinal Epidural Hemorrhage With Shoulder And Neck Pain As Initial Presentation: A Casereport, Yu-Min Kuo, Wai-Keung Lee, Cheng-Chih Liao, Li-Rong Chen
Rehabilitation Practice and Science
Spontaneous spinal epidural hemorrhage is a very rare disease, with an annual incidence estimated at one per million. Emergency surgery is indicated for a good prognosis, without which the death rate would be higher. Although the initial symptoms of cervical, thoracic, and back pain make it difficult to differentiate spontaneous spinal epidural hemorrhage from other common disease with similar symptoms, magnetic resonance imaging (MRI) is a highly sensitive diagnostic method. We report the case of a 22-year-old male patient who initially complained of pain over the left shoulder and left cervical area. After nine days, numbness was noted in his left …

Delayed Diagnosis Of Spinal Arteriovenous Malformation And Management Of Its Urological Complication: A Casereport, Yuan-Jen Wang, Peir-Renn Wang, Yu-Hui Huang, Su-Ju Tsai, Liu-In Bih
Rehabilitation Practice and Science
Spinal arteriovenous malformations (Spinal AVM) are a rare disorder with an estimated incidence of about 1 per million people per year. Because of the diversity of the presenting symptoms, this disorder is difficult to diagnose, despite new advances in neuronimaging. We report the case of a girl who was diagnosed at 12 years old to have spinal AVM at a high cervical level and presented with four limbs weakness and dyspnea. She was found to have extremely high voiding pressure and a rapid development of grade 2 vesicoureteral reflux (VUR), which was thought to be a chronic complication after spinal …

Nasogastric Tube Syndrome: A Casereport, Meng-Chang Wu, Hung-Chou Chen, Jiunn-Horng Kang, Shin-Ching Chen
Rehabilitation Practice and Science
The nasogastric tube syndrome (NG tube syndrome) is a rare but a potentially life-threatening complication of a patient with indwelling nasogastric tube feeding. The cause of the syndrome is due to the NG tube irritation, resulting in ulceration and infection of the posterior cricoid region with subsequent dysfunction of vocal cord abduction. This dysfunction may result in complete loss of vocal cord abduction and cause serious airway obstruction. Reports of this syndrome are rare in the literature. A 64-year-old female patient with long-term NG tube feeding, suffered from serious upper airway obstruction. The patient had obvious desaturation while the tracheostomy …

Rehabilitation Practice and Science
Objective: This reviewarticle article describes the etiology, pathogenesis, clinical characters and management of low back pain (LBP) caused by myofascial trigger points (MTrPs), i.e. myofascial low back pain.Findings: Based on the currently available knowledge and our clinical experience, we analyzed the basic and clinical aspects of myofascial LBP. Most cases of myofascial LBP are related to injuries, either current or previous. Active MTrPs that cause LBP are usually activated as a consequence of other etiological lesions. Therefore, it is important to determine and treat the underlying pathological lesions in order to avoid recurrence of myofascial LBP. If the underlying pathological …

Investigation Of Flatfoot Screening In Developmentally Delayed Children, Li-Chen Tung, Pei-Chi Hsiao, Willy Chou, Cheng-Cheng Hsu, Wen-Chih Lin, Isbel Tou
Rehabilitation Practice and Science
Flatfoot disease is caused by disappearance or collapse of the medial longitudinal arch of the foot. Most of patients with flatfoot disease have what is known as ”flexible flatfoot”. Flatfoot in developmentally delayed children, however, is due to muscle weakness of lower limbs, which leads to disproportionate muscle growth or joint contracture. A study conducted by Chii-Jeng Lin in 2001 indicated that flat-footedness may influence gait, single-foot standing, and jumping ability. The author considered that flat-footedness was not only a problem in skeletal structure but that it may also be a consequence of dynamic foot function. Children with developmental delays …

Rehabilitation Practice and Science
The purpose of this casereport is to demonstrate the remote effect of myofascial trigger point (MTrP) injection for pain control. It describes a patient suffering from chronic pain due to MTrPs in bilateral upper trapezius muscle with poor results to direct MTrP injection to upper trapezius. After injection to the MTrPs in bilateral extensor carpi radialis longus, extensor indicis, and first dorsal interosseous muscles, the effectiveness of MTrP injection to bilateral upper trapezius muscles remarkably enhanced. This patient finally became almost pain free. The reason for poor relief of pain immediately after the first MTrP injection (directly on the upper …

A 31-year-old lady complained of multiple painless masses on her left arm. Two masses were clearly visible in the left palm and distal forearm, and a smaller lump was also palpable in the left antecubital area. A neurogenic tumor in the left palm was diagnosed by magnetic resonance image (MRI). Sonography demonstrated highly vascular neurilemmomas along the median nerve in the palm, distal forearm, and antecubital area. A nerve conduction velocity study of the median nerve showed prolonged distal latency of the motor nerve portion but normal sensory function. The tumors were resected, and pathology examination confirmed the diagnosis of …

Initial Blood Glucose Level In The Emergency Department As A Prognostic Factor On Functional Outcome Of Ischemic Stroke, Yi-Min Chen, Gwo-Chi Hu, Jiann-Perng Chen, Chien-Shen Chen, Cheng-Tsong Liao, Kuan-Wen Chen
Rehabilitation Practice and Science
Although many studies are available in focus on prognostic factors for acute cerebral vascular disease, there are few reports to discuss the impact of the initial glucose level on the functional outcome after stroke. The initial glucose level in the emergency department is more representative of acute glucose response after stroke and is not influenced by the treatment in the emergency department. The aim of our study is to evaluate the prognostic role of the initial glucose level on the functional outcome after stroke. We recruited individuals who sustained first-ever stroke and were admitted to an inpatient stroke rehabilitation program. Patients …

Rehabilitation And Prognosis Of Childhood Arterial Ischemic Stroke: A Casereport, Chia-Huang Dai, Wei-Jang Yen, Ta-Shen Kuan, Chang-Zern Hong
Rehabilitation Practice and Science
The incidence of ischemic stroke in childhood is two to 13 per 100,000 children per year. Although the incidence is much lower than adults, strokes in children not only cause a large number of deaths, but also induce significant disabilities. Because childhood ischemic strokes are rare, multi center, controlled trials should be performed to provide evidence based data related to effectiveness of medical treatment and rehabilitation. Current studies on the management of stroke in children are mainly based on casereports, uncontrolled trials, or are adapted from adult literature. An eleven-year-old, obese boy presented to the Emergency Department with an initial presentation …

Combining Botulinum Toxin Type A Injection And Modified Constraint-Induced Movement Therapy In A Taiwanese Patient With Spastic Hemiplegic Cerebral Palsy: A Casereport, Yi-Chen Tseng, Jui-Yun Su, Yu-Ching Lin
Rehabilitation Practice and Science
Hypertonia interferes with selective motor control and performance of activities in children with spastic cerebral palsy. At present, most therapists use traditional therapeutic techniques such as neurodevelopmental treatment or other approaches to handle the problems posed by cerebral palsy but the results are not very good. Many studies had shown that botulinum toxin type A (BTX-A) injection can control abnormal muscle tone effectively without major side effects. Constraint-induced movement therapy (CIMT) includes constraint of the sound side upper limb and intensive therapy of the affected limb. In order to reduce spasticity and enhance the use of the affected limb simultaneously, …

Reinvigorating Tax Expenditure Analysis And Its International Dimension, J. Clifton Fleming Jr., Robert J. Peroni
Reinvigorating Tax Expenditure Analysis And Its International Dimension, J. Clifton Fleming Jr., Robert J. Peroni
Faculty Scholarship
Tax expenditure analysis (TEA) was rigorously criticized from its inception and continues to draw negative reviews. Notwithstanding this criticism, the Congressional Budget and Impoundment Control Act of 1974 requires the President's annual budget submission to contain a list of tax expenditures, and Congress's Joint Committee on Taxation has produced its own tax expenditure list each year since 1972. Although TEA has not restrained or reversed the growth of tax expenditures, TEA continues to play a major role in tax policy debates to the chagrin of its detractors. The persistence of TEA in a hostile environment suggests that it has meaningful …
