Background

Patients who present to medical practices without health insurance or with serious co-morbidities can become fiscal disasters to those who care for them. Their consumption of scarce resources has caused consternation among providers and institutions, especially as it concerns the amount and type of care they should receive. In fact, some providers may try to avoid caring for them altogether, or at least try to limit their institutional or practice exposure to them.

Ron Paul worked in the restaurant business for 18 years, including 15 as the owner of Ron Paul Charcuterie . Then, as chief of staff in Commissioner Charlie Hales' office in 2000, Paul assembled a citizens group that began to explore the feasibility of a public market in the city. He now leads the effort for the Portland Public Market, drawing upon both of his career paths from the last 20 years. Analysis Of Bioelectrical Tissue Conductivity In Malaysian Adults, Fatimah Ibrahim, Wan Abu Bakar Wan Abas, M.N. Taib

This paper presents the bioelectrical tissue conductivity (BETC) analysis of Malaysian adults using bioelectrical impedance. A total of 142 subjects (91(64.1) female and 51(35.9) male) with no past medical history were studied. The BETC was measured using phase angle (a), resistance (R), reactance (Xc) and body capacitance (BC). There is a significant difference (P<0.001) in the BETC mean values of the subjects. The mean phase angles measured were 7.4±0.8° and 6.3±0.6° with corresponding mean body capacitances of 818±158pF and 557±101pF for male and female, respectively. The results show that a and BC mean values are higher in the male than the female. However, the female R (635±71.5Ω) and Xc (69.9 ± 8.4Ω) were higher than the male R (504 ± 63.4Ω), and Xc (64.6 ± 6.8Ω), respectively. These results show that the BETC values in Malaysian adults are in agreement with the normal healthy subjects at large. © 2004 IEEE.

One of the undeniable facts about living is that everyday we are getting older. By 2050, it is projected that one out of every five Malaysians will be 60 or older. The economic challenge of this demographic change will affect development and the financial implications of sustaining well being of this group are formidable and complex. Pharmacophore Based Synthesis Of 3-Chloroquinoxaline-2-Carboxamides As Serotonin(3) (5-Ht3) Receptor Antagonist, R. Mahesh, R.V. Perumal, P.V. Pandi

A series of 3-chloroquinoxaline-2-carboxamides were designed and prepared by the condensation of 3-chloro-2-quinoxaloylchloride with appropriate Mannich bases of the p-aminophenol in the microwave environment. The synthesized compounds were evaluated for serotonin(3) (5-HT3) receptor antagonistic activities in longitudinal muscle-myenteric plexus (LMMP) preparation from guinea pig ileum against the 5-HT3 agonist, 2-methyl-5-HT. Compound 3g exhibited comparable 5-HT3 antagonistic activity (pA(2) 6.4) to that of standard antagonist Ondansetron (pA(2) 6.9), while the other compounds exhibited mild to moderate 5-HT3 antagonistic activities.

Over a period of 6 months from January to June 2002, an unusual increase in the isolation of highly resistant Pseudomonas aeruginosa strains was observed in the various wards and intensive care units of a large general hospital in Johor Bahru, Malaysia. An equal number of multidrug-resistant (MDR) and drug-susceptible strains were collected randomly from swabs, respiratory specimens, urine, blood, cerebral spinal fluid, and central venous catheters to determine the clonality and genetic variation of the strains. Designing a sound cow-calf nutritional program requires knowledge of nutrient requirements, diet quality, and intake. Effectively using the NRC (1996) beef cattle requirements model (1996 NRC) also requires knowledge of dietary degradable intake protein (DIP) and microbial efficiency. Objectives of this paper are to 1) describe a framework in which 1996 NRC-applicable data can be generated, 2) describe seasonal changes in nutrients on native range, 3) use the 1996 NRC to predict nutrient balance for cattle grazing these forages, and 4) make recommendations for using the 1996 NRC for forage-fed cattle. Inadequate dietary energy intake and poor body condition can negatively affect reproductive function. Supplemental lipids have been used to increase energy density of the diet and may also have direct positive effects on reproduction in beef cattle. Several fatty acid sources have been studied as they relate to reproductive function. Common sources include sunflower, safflower, cottonseed, rice bran, soybeans, fishmeal, animal tallow, and calcium salts of fatty acids. Fats have been fed before and after calving, during the breeding season, and during heifer development. Total mixed rations containing conventional forage sorghum, brown midrib (bmr)-6 forage sorghum, bmr- 18 forage sorghum, or corn silage were fed to Holstein dairy cows to determine the effect on lactation, ruminal fermentation, and total tract nutrient digestion. Sixteen multiparous cows (4 ruminally fistulated; 124 d in milk) were assigned to 1 of 4 diets in a replicated Latin square design with 4-wk periods (21-d adaptation and 7 d of collection). Diets consisted of 40% test silage, 10% alfalfa silage, and 50% concentrate mix (dry basis). Acid detergent lignin concentration was reduced by 21 and 13%, respectively, for the bmr-6 …

Two finishing trials were conducted to determine the effects of adding different types of corn bran, a component of corn gluten feed, on cattle performance. In Trial 1, 60 English crossbred yearling steers (283 ± 6.7 kg) were used in a completely randomized design with four dietary treatments. Treatments were diets with no corn bran, dry corn bran (86% DM), wet corn bran (37% DM), and rehydrated dry bran (37% DM). Bran was fed at 40% of dietary DM. All finishing diets had (DM basis) 9% corn steep liquor with distillers solubles, 7.5% alfalfa hay, 3% tallow, and 5% supplement. …

Three finishing trials were conducted to determine the effects of dietary manipulation and management on N losses from open feedlots. In each experiment, 96 steers were assigned randomly to 12 nutrient balance pens. In Trial 1, calves were fed for 180 d during the winter/spring months; in Trial 2, yearlings were fed for 132 d in the summer. In Trials 1 and 2, N losses from pens were compared directly by adding OM to the pen surface or indirectly by feeding digestible ingredients designed to increase OM excretion. A Prisoner's Right To Religious Diet Beyond The Free Exercise Clause, 51 Ucla L. Rev. 1151 (2004), Benjamin Liu

Are religious prisoners entitled to dietary accommodations consistent with their religious beliefs? The current answer for this question derives from two 1987 cases, Turner v. Safley and O'Lone v. Estate of Shabazz, in which the U.S. Supreme Court articulated a factor-driven balancing test. Under this test, a prison regulation may burden an inmate's rights only if, on balance, the regulation reasonably serves a penological interest. However, the application of the Turner test often leads to conflicting results in the lower courts. The conflicting legal decisions stem from ambiguities in the concept of "reasonableness." Fetal exposure to alcohol can lead to extensive pathology in the CNS causing fetal alcohol syndrome or alcohol-related neurodevelopmental disorder. Our previous research has revealed that alcohol has detrimental effects on development of neurons and glial cells, including microglia. However, the effects of alcohol on microglial function as well as interactions between microglia and neurons remain relatively unexplored. Microglia produce immunomodulatory cytokines and chemokines that directly control the survival, development, and function of neurons and glia. Pronghorn (Antilocapra americana) were reintroduced into Wind Cave National Park (WCNP) in 1914 and thus, have inhabited the park for almost a century. A decline in the population has raised concern for the continued existence of pronghorn inside the park. Historically, pronghorn numbers reached greater than 300 individuals but were estimated to be less than 50 individuals during this study. Knowledge of food habits, genetic structure, causes of mortality, survival rates, and information related to movements were important to developing an understanding of factors contributing to this decline. Research in the 1990s indicated that populations of white-tailed deer (Odocoileus virginianus) in the northern and central Black Hills had declined since the late 1970s likely due to habitat deterioration. In August 2000, the Jasper fire consumed nearly 35,000 ha of ponderosa pine (Pinus ponderosa) forest in the southern Black Hills. To ascertain the nutritional condition of white-tailed deer and mule deer (O. hemionus) and their response to fire, 5 adult white-tailed deer and 5 adult mule deer were collected in burned and unburned habitat, in February and August, 2002 and 2003. Biological thiols are important antioxidants, and recent studies showed that their contents vary depending on the groups of foodstuffs. Therefore, we investigated the levels of some biological thiols in various vegetables and fruits by using a sensitive high-performance liquid chromatography (HPLC) technique. Biological thiols measured in some vegetables and fruits include glutathione (L-glutamyl-L-cysteinly glycine, GSH), N-acetylcysteine (NAC), captopril [CAP (C9H15NO3S)], homocysteine (HCYS), cysteine (CYS), and -glutamyl cysteine (GGC). Our results show that biological thiol contents are between 3-349 nM/g wet weight in vegetables and 4-136 nM/g wet weight in fruits. CAP is only found in asparagus (28 nM/g wet weight). …

Research to date has focused on increasing the supply of glucose precursors in transition dairy cows to reduce ketosis. Ketones and ketone bodies represent energy sources for body tissues; therefore, conserving blood glucose for lactose synthesis in the mammary gland. This led to the hypothesis that transition cows might benefit from diets formulated to stimulate rumen epithelial ketogenesis or dietary ingredients supplying ketone precursors.
