Hemophilus Influenzae Serotype-B Meningitis In An Adult Immunocompetent Patient, Hanan A. Babay, Awadh R. Al-Anazy, Abubakar B. Saidu, Abdelmageed M. Kambal

Saudi Medical Journal

We present a case of meningitis caused by Hemophilus influenzae type-b in an immunocompetent 41-year-old Saudi lady. The patient was successfully treated with Ceftriaxone for 10 days. A review of Hemophilus influenzae meningitis in adults and the impact of conjugated vaccine on the epidemiology of the disease are given.

Anxiety, Stress, And Health In Northern Plains Native Americans, Tami J. De Coteau, Debra A. Hope, Jessiline Anderson

In the present study, the Beck Anxiety Inventory (Beck, Epstein, Brown, & Steer, 1988), Anxiety Sensitivity Inventory (Peterson & Reiss, 1992), the Holmes-Rahe Social Readjustment Rating Scale (Holmes & Rahe, 1967), Northern Plains Bicultural Inventory (Allen & French, 1994), and a health questionnaire were administered to investigate the relationship between anxiety, stressful events, health, and cultural participation among 147 Native American adults from a Midwestern reservation community. The results of these self-report measures indicated that, as has been found in the majority culture, stressful life events predicted physical health problems and self-reported anxiety. Characterization Of Pasteurella Multocida Associated With Pneumonia In Bighorn Sheep, Glen C. Weiser, Walter J. Delong, Julia L. Paz, Bahman Shafii, William J. Price, Alton C.S. Ward

Pasteurella multocida is a highly diverse group of bacteria recognized as important pathogens. Although P. multocida is not ordinarily associated with disease in Rocky Mountain bighorn sheep (Ovis canadensis canadensis), numerous isolates were cultured in high numbers from free-ranging bighorn sheep in the Hells Canyon area of Idaho, Washington, and Oregon (USA) during the winter of 1995–96. Animals captured in Hells Canyon and held in captivity, and their offspring, also harbored P. multocida. The threat of bioterrorism forces the public health infrastructure to focus attention on overall issues related to challenges posed by emerging and re-emerging infectious diseases. There is a crucial need to strengthen existing surveillance systems and to validate real-time approaches to surveillance that can provide timely alerts of epidemics whether they occur naturally or through a bioterrorism attack. The purpose of this study is to implement and evaluate a bioterrorism syndromic surveillance system called BioDefend™, to determine if the system could detect a potential epidemic/bioterrorism attack within 24-36 hours, more rapidly than it would be identified by routine health surveillance. …

Dartmouth Scholarship

The bacterium Pseudomonas aeruginosa causes chronic respiratory infections in cystic fibrosis (CF) patients. Such infections are extremely difficult to control because the bacteria exhibit a biofilm-mode of growth, rendering P. aeruginosa resistant to antibiotics and phagocytic cells. During the course of infection, P. aeruginosa usually undergoes a phenotypic switch to a mucoid colony, which is characterized by the overproduction of the exopolysaccharide alginate. Alginate overproduction has been implicated in protecting P. aeruginosa from the harsh environment present in the CF lung, as well as facilitating its persistence as a biofilm by providing an extracellular matrix that promotes adherence. Sensitivity Analysis For The Assessment Of Causal Vaccine Effects On Viral Load In Hiv Vaccine Trials, Peter B. Gilbert, Ronald J. Bosch, Michael G. Hudgens

Vaccines with limited ability to prevent HIV infection may positively impact the HIV/AIDS pandemic by preventing secondary transmission and disease in vaccine recipients who become infected. To evaluate the impact of vaccination on secondary transmission and disease, efficacy trials assess vaccine effects on HIV viral load and other surrogate endpoints measured after infection. A standard test that compares the distribution of viral load between the infected subgroups of vaccine and placebo recipients does not assess a causal effect of vaccine, because the comparison groups are selected after randomization. It is of interest in some applications to determine whether there is a relationship between a hazard rate function (or a cumulative incidence function) and a mark variable which is only observed at uncensored failure times. We develop nonparametric tests for this problem when the mark variable is continuous. Tests are developed for the null hypothesis that the mark-specific hazard rate is independent of the mark versus ordered and two-sided alternatives expressed in terms of mark-specific hazard functions and mark-specific cumulative incidence functions. Temporal Stability And Geographic Variation In Cumulative Case Fatality Rates And Average Doubling Times Of Sars Epidemics, Alison P. Galvani, Xiudong Lei, Nicholas P. Jewell

U.C. Berkeley Division of Biostatistics Working Paper Series

We analyze temporal stability and geographic trends in cumulative case fatality rates and average doubling times of severe acute respiratory syndrome (SARS). In part, we account for correlations between case fatality rates and doubling times through differences in control measures. We discuss factors that may alter future estimates of case fatality rates. We also discuss reasons for heterogeneity in doubling times among countries and the implications for the control of SARS in different countries and parameterization of epidemic models.
