The aim of this case report is to describe the obstetric performance of a patient with multiple uterine and supravaginal cervical fibroids. A 36-year-old, gravida 3 para 0+2 with multiple uterine and cervical fibroids presented with inevitable abortion at 17 weeks gestation. She had a spontaneous rupture of membranes followed by expulsion of fetus as breech with entrapment of aftercoming head by a cervical fibroid. Oxytocin infusion and digital traction were able to deliver the fetus. The placenta, however, was trapped in the fundal area and could not be delivered under general anesthesia because of mechanical obstruction by the fibroid. …

Acquired Reactive Perforating Collagenosis Associated With Sick Euthyroid Syndrome, Mohamad I. Fatani, Yousef M. Al-Ghamdi, Khalid A. Al-Afif, Magdy R. Abdulghani, Talal M. Karima

Acquired reactive perforating collagenosis is a rare skin disorder associated with several systemic diseases, particularly diabetes and chronic renal failure. A 52-year-old Saudi female patient with a known case of diabetes mellitus type II, chronic renal impairment, hypertension, peripheral vascular disease, congestive heart failure, stroke and left hemiplegia presented with multiple pruritic skin eruption on the trunk and extremities. We believe that this is the first case of acquired reactive perforating collagenosis in association with sick euthyroid syndrome to be reported.

Hydroxyurea is an established drug that has been used for the treatment of myeloproliferative disorders and some solid tumors for some time. In recent years it has also been found to be effective in the treatment of sickle cell disease. Short term side effects are not serious, and are manageable. The major concern is the potential leukemogenesis with long term use. The risk of leukemogenesis is not defined with its use in benign hematological conditions. Adolescent Risk Factors For Sexual Victimization: A Longitudinal Analysis Of Rural Women, Janine M. Zweig, Aline Sayer, Lisa J. Crockett, Judith R. Vicary

Longitudinal data were used to examine risk factors for sexual victimization among 237 young adult rural women. In this sample, 8% reported experiencing forced sex (i.e., physically violent experiences) and a separate 22% reported experiencing coerced sex (i.e., external psychological manipulation, substance-related coercion, or internal psychological pressure.) Women who had more educated mothers had a greater probability of reporting forced sex. In addition, mothers’ education moderated the relationship between individual risk factors and the probability of reporting forced sex. For women with less educated mothers, higher frequency of sexual activity during adolescence was related to an increased probability of reporting …

America's novel use of special forces, precision weapons, and indigenous allies has attracted widespread attention since its debut in Northern Afghanistan last fall. It has proven both influential and controversial. Many think it caused the Taliban's sudden collapse. For them, this "Afghan Model" represents warfare's future, and should become the new template for US defense planning. Objective - This study examined the association between cardiorespiratory fitness and C-reactive protein (CRP), with adjustment for weight and within weight categories.

Methods and Results - We calculated median and adjusted geometric mean CRP levels, percentages of individuals with an elevated CRP (≥2.00 mg/L), and odds ratios of elevated CRP across 5 levels of cardiorespiratory fitness for 722 men. CRP values were adjusted for age, body mass index, vitamin use, statin medication use, aspirin use, the presence of inflammatory disease, cardiovascular disease, and diabetes, and smoking habit. In developing TMDL waste-load allocations for the Snake River-Reservoir system in Western Idaho and Eastern Oregon, determinations of the assimilative capacity of the system and the impact of pollutant reduction strategies has been performed using both a statistical-correlation approach and a mechanistic modeling approach. The system included the Lower Snake River, Brownlee Reservoir, Oxbow Reservoir, and Hells Canyon Reservoir with the focus was on Brownlee Reservoir.

Microchannels are being considered in many advanced heat transfer applications, including automotive, fuel cells, and electronics cooling. However, there are a number of fundamental issues still unresolved with respect to heat transfer and fluid mechanics perspective. An experimental investigation of the heat transfer, pressure drop, and flow patterns during flow boiling in microchannels is performed. Six parallel microchannels with a mean hydraulic diameter of 207 micron are manufactured and tested. Flow patterns have been observed in the channels under diabatic conditions. In this paper, we present a new framework to automatically group similar shots into one scene, where a scene is generally referred to as a group of shots taken place in the same site. Two major components in this framework are based on the motion characterization and background segmentation. The former component leads to an effective video representation scheme by adaptively selecting and forming keyframes. We address the text content mining problem through a concept based framework by constructing a conceptual knowledge base and discovering knowledge therefrom. Defining a novel representation called the Concept Frame Graph (CFG), we propose a learning algorithm for constructing a CFG knowledge base from text documents. An interactive concept map visualization technique is presented for user-guided knowledge discovery from the knowledge base. Through experimental studies on real life documents, we observe that the proposed approach is promising for mining deeper knowledge.

The route optimization operation in Mobile IP Version 6 (MIPv6) allows direct routing from any correspondent node to any mobile node and thus eliminates the problem of "triangle routing" present in the base Mobile IP Version 4 (MIPv4) protocol. Route optimization, however, requires that a mobile node constantly inform its correspondent nodes about its new care-of addresses by sending them binding update messages. Unauthenticated or malicious binding updates open the door for intruders to perform redirect attacks, i.e., malicious acts which redirect traffic from correspondent nodes to locations chosen by intruders. People may believe that personal attributes are fixed entities that cannot be changed (hold an entity theory). Alternatively, they may believe that qualities of a person are malleable (hold an incremental theory). In the present research, the authors used Sternberg's (1966) memory search task to examine entity and incremental theorists' cognitive strategies in memory search. It was hypothesized that entity theorists, who have a greater tendency to make spontaneous evaluation of people, would organize impressions in short-term memory according to whether the stimulus persons are positively or negatively evaluated. The paper presents a technique for computing the individual throughputs and the average queue occupancy when multiple TCP connections share a single bottleneck buffer. The bottleneck buffer is assumed to perform congestion feedback via randomized packet marking or drops. We first present a fixed point-based analytical technique to compute the mean congestion window sizes, the mean queue occupancy and the individual throughputs when the TCP flows perform idealized congestion avoidance. We study the dependence of the capacity of multi-hop wireless networks on the transmission range of nodes in the network with TCP regulated traffic. Specifically, we examine the sensitivity of the capacity to the speed of the nodes and the number of TCP connections in an ad hoc network. By incorporating the notion of a minimal acceptable QoS metric (loss) for an individual session, we argue that the QoS-aware capacity is a more accurate model of the TCP-centric capacity of an ad-hoc network. We continued with sputter depth profiling of 316 and 316L steel samples that have been exposed to LBE. We also calibrated the sputter depth profiling using a sample of SiO2 on Si, and the SEM. This is a valuable independent determination of the thickness of oxide layers.

In the laboratory, progress continues using the XPS machine. Experiments have been performed on steel samples using Argon ions to mill away the surface of the sample, thereby making measurements as a function of depth.
