The use of system data to make predictions about the future system state commonly known as prognostics is a rapidly developing field. Prognostics seeks to build on current diagnostic equipment capabilities for its predictive capability. Many military systems including the Joint Strike Fighter (JSF) are planning to include on-board prognostics systems to enhance system supportability and affordability. Current research efforts supporting these developments tend to focus on developing a prognostic tool for one specific system component. During late spring through summer of 1994 and 1995, 290 randomly selected stream sites in Nebraska, Kansas, and Missouri were sampled once for several parameters including conductivity, turbidity, total phosphorus, nitrate-nitrite nitrogen, the index of biotic integrity, and a habitat index. Based on landscape data from watersheds that were delineated for each sampling location, interrelationships were examined between these water quality parameters and land use/land cover, the normalized difference vegetation index (NDVI), and vegetation phenological metrics derived from the NDVI. The generation of membership functions for fuzzy systems is a challenging problem. We show that for Mamdani-type fuzzy systems with correlation-product inference, centroid defuzzification, and triangular membership functions, optimizing the membership functions can be viewed as an identification problem for a nonlinear dynamic system. This identification problem can be solved with an extended Kalman filter. We describe the algorithm and compare it with gradient descent and with adaptive neuro-fuzzy inference system (ANFIS) based optimization of fuzzy membership functions. In this paper, we propose a data mining approach to recommending new library books that have never been rated or borrowed by users. In our problem context, users are characterized by their demographic attributes, and concept hierarchies can be defined for some of these demographic attributes. Books are assigned to the base categories of a taxonomy. Our goal is therefore to identify the type of users interested in some specific type of books. Pulmonary tuberculosis continues to be a major health problem all over the world. Medical treatment with anti­biotics is effective in most cases. However, when medical treatment fails or complications develop, sur­gery is indicated. Between July 1994 and June 2000, fifty-two patients with pulmonary tuberculosis, thirty-six males and sixteen females, under­went major surgery at Mansoura Uni­versity Hospitals. Mean age was 35 years (range 22-54). Chronic suppu­ration and/or haemoptysis were the main symptoms. Eleven patients had positive sputum at the time of surgery. Thirteen patients had pleuro-pneumonectomy or pneumonectomy. Thirty-six had partial lung resection, two had decortication, and one had thoracoplasty. All …

Objective : To assess the function of the left atrioventricular valve (LAW) following surgical repair of complete atrioventricular septal defect (CAVSD) and find out the possible risk factors for development of left atrioventricular valve regurgitation (LAVVR). Surgical Outcome Of Coronary Artery Bypass Grafting In Patients With Poor - Left Ventricular Function Running Title: Cabg In Patients With Lf Ef, Mohamed-Adel Elgamal, Sayed Abou Elsoud

Objective : To evaluate the early and midterm outcome in patients with coronary artery disease (CAD) and severe left ventricular (LV) dysfunc­tion, as defined by ejection fraction (LV EF) less than 35%, after coronary artery bypass grafting (CABG). Patients and methods : From April 1999 to June 2002, ninety seven pa­tients with CAD and EF < 35% had isolated CABG surgery. Preoperative-ly, patients were evaluated by history, physical examination, 12-leads ECG, echocardiographic study, and cardiac cathetenzation. In elective cases, via­bility studies were performed. All pa­tients had on pump CABG surgery. Patients were managed in the cardiac surgical ICU. After discharge, patients were followed up regularly in the car- diology out-patients clinic. Results: There were 76 males and 21 females. History of prior myocar-dial infarction was present in 46%. The main presenting symptoms were angina pectoris, heart failure, cardio-genie shock and pulmonary edema. Preoperatively, 59% of the patients were in New Your Heart Association Classification (NYHA) class III or IV. Coronary angiography revealed left main or triple vessel disease in 82%. More than half of the patients (54%) had moderately to severely dilated LV. The LV EF was: 30 - 35% in 26; 25 - 30% in 54; and 20 - 25% in 17 patients. The need for vaccination against schistosomiasis is an achievable goal. In this work the protective and antifi-brotic effect of 74 KDa schistosoma protein antigen recognized by protec‌tive monoclonal antibody was evaluat‌ed histologically and immunohisto-chemicalty. Swiss mice were immunized with 50 mg of purified 74KDa antigen without and with Freund's adjuvant. Then mice were infected with 200 S. mansoni cercari-ae three days after final immunization. The present study included muscle biopsies from 12 infants (5 females and 7 males) with Werding-Hoffman's disease (WHO). The mean age of presentation was 11.8 months. Histo­ry of consanguinity was detected in 58.3%(7/12) of cases. The infants were presented with typical floppy baby pattern since birth or early in fi­rst few months of life. The testicuiar tissues are affected adversely by zinc deficiency. These changes are correlated with the de­gree of deficiency. In comparison with the control (C) group, moderate zinc deficiency (MZnD) leads to regressive changes in the adluminal germ cells but not in spermatogonia located in the basal compartments while severe zinc deficiency (SZnD) can affect both the adluminal and basal compartme­nts. We measured serum M-CSF con­centrations in 25 patients with idio-pathic thrombocytopenic purpura (ITP) to determine if they were elevat­ed in this condition, and to investigate the effect of corticosteroid therapy on the level of this growth factor in 8 ITP patients with profound thrombocyto-penia. M-CSF levels were measured by enzyme immunosorbent assay (ELISA). The mean M-CSF was sig­nificantly increased (P<0.05) in pa­tients with ITP in comparison to con­trol group (mean + SO were 929 + 127u/ml and 364 + 69 u/ml respec­tively). There was a significant negative correlation between M-CSF and plate- let count (P<0.05). The mean level of M-CSF was significantly decreased three weeks after corticosteroid thera­py in comparison to the high pretreat-ment level (P<0.05). Thus we con­cluded that the elevated level of M-CSF in children with ITP may reflect activation of reticuloendothelial sys­tem which could result in positive feedback to increase the destruction of platelets. Corticosteroid therapy could induce thrombocytosis by de­crease of M-CSF level in patients with ITP.

The goal of this study is to evaluate the response of medical students and tutors who participated in actual coursework using PBL in order to better understand the power of PBL at National Defense Medical Center. The subjects of this study were 152 medical students and tutors in pathology and pharmacology courses at NDMC. Tutors and medical students were surveyed using a structured questionnaire after the courses concluded, in order to compare levels of satisfaction and effectiveness for PBL with traditional instructional methods. 小組教學是問題導向教學中重要的教學方式，但是缺乏人本導向所具備的小組教學本質內容的探究。本研究目的在探討以人本教育立場所發展的小組教學要素。研究者以教師即參予觀察者的研究法，收集紀錄參予小組教學中的資料，包括小組互動過程、教師之教學日誌，學生之學習心得。以繼續比較法進行內容分析所收集之資料。結果找出人本教育之小組教學的要素是：學習方法、體驗知識、思考提問、整理經驗、夥伴關係。最後討論人本導向小組教學與自我學習及專業知識傳承的關係。

Community-oriented medical education has become a major part of the main curriculum for medical students in recent years. How to design an ad-equate community medicine program suitable for the environment in Taiwan is a very important issue. College of Medicine, National Taiwan University, started to offer a two-week community medicine program to their 5th grade medical students ten years ago, one of the rare examples in Taiwan.
