We report 2 extreme preterm neonates who developed cardiac tamponade secondary to perforation of the myocardium by the percutaneous silastic central venous catheters, which were inserted for parental nutrition. Percutaneous pericardiocentesis was performed and pericardial effusion was aspirated, later proved by analysis to be total parental nutrition. The lines were removed and the patients successfully resuscitated and survived, both were sent home in good condition.

OBJECTIVE: Evaluation of known predictors of gangrene in neonates with necrotizing enterocolitis (NEC) and identification of those suggestive of severe disease, requiring expeditious laparotomy rather than primary peritoneal drainage as a definitive treatment. The incidence of metabolic syndrome (MS) is rising worldwide. This is partly due to a significant increase in the prevalence of obesity. Observational cross-sectional studies as well as demographic health surveys from the Middle East, point out that the prevalence of obesity increases from an average of 6% in healthy children to 20% in adolescent males and to a further 32% in elderly patients. OBJECTIVE: To estimate the prevalence of hypocalcemia in children examined for serum calcium. METHODS: A record-based study was carried out in the Specialized Pediatric Center in Sana9a city, Yemen during the 4 year period 1999 to 2003. Out of 90600 patients seen for different causes, 310 (0.34%) were subjected to serum calcium level examination. Data regarding age and gender were also collected. Dietary Macronutrient Intake Of Saudi Males And Its Relationship To Classical Coronary Risk Factors, Eman M. Alissa, Suhad M. Bahijri, Gordon A. Ferns

OBJECTIVE: To investigate whether the dietary intake of energy; macronutrients; and fiber differ between age groups, racial groups and socio-economic classes among males from the Western province of Kingdom of Saudi Arabia (KSA). METHODS: Data were collected from 303 male subjects, aged 15-80 years. They were selected randomly from King Abdul-Aziz University Hospital, Jeddah, KSA from October 2001 to November 2003 and grouped according to their age into 3 groups. The subjects were asked to complete a questionnaire concerning their demographic characteristics, health history, lifestyle, and dietary habits. RESULTS: Energy and carbohydrates intake fell with age (p<0.05). Total dietary carbohydrates and fat intake were similar for all groups when expressed as a percentage of energy intake. The percentage energy as protein increased with age (p<0.05). Mean cholesterol intake was high for all groups, but fell with age group (p<0.0001). Saturated fat and monounsaturated fat intake, expressed as percentage energy intake were both high, whereas polyunsaturated fat intake was low. The youngest group had the highest percentage energy provided by saturated fatty acid (p<0.001), and the lowest percentage energy as polyunsaturated fatty acid (p<0.05) compared to the other groups. The intake of fibre rose with age was significantly higher in the older group (p<0.05). CONCLUSION: Diet consumed by urban dwellers in KSA appears to have resulted in an imbalance of macronutrient intake among all sectors of the population. This problem can only be averted by raising public awareness and the development of appropriate population-specific nutritional guidelines.

In the last decade Leuconostoc species have been reported with increasing frequency as human pathogens, causing bacteremia, meningitis and peritonitis. We report here a child with short-bowel syndrome who developed bacteremia following multiple surgeries for necrotizing enterocolitis. Leuconostoc species was isolated from the blood cultures. The child was successfully treated with ampicillin and gentamycin. He however remained total parenteral nutrition dependent due to his multiple abnormalities. We call the attention of microbiologists and pediatricians to this emerging pathogen, which is intrinsically resistant to vancomycin and can be misidentified in the microbiology laboratory as Viridans streptococci or Enterococci. Since the nature of abdominal tuberculosis is mimicking a number of diseases, this may cause delayed diagnosis resulting in evident increased morbidity and mortality. Most of the time, serologic and bacteriologic tools are not enough. We report 3 adolescents with distinct presentations, one mimicking Crohn9s disease, one with hepatitis, and the last one with ascites. Terminal ileitis and mesenteric lymphadenitis were found in laparotomy of the first case mimicking Crohn disease. Granulomatous hepatitis was found in the liver biopsy of the second patient, and peritonitis was found by laparoscopy of the third patient. Tuberculosis could be diagnosed merely by histopathologic …

Cardiovascular disease CVD is the leading cause of morbidity and mortality in the world. Vitamin E as an anti-oxidant vitamin, was suggested to have a role in the prevention of CVD. We did a meta-analysis, using the Cochrane Group Methodology, of all available randomized controlled trials RCTs to evaluate the role of vitamin E in the prevention of CVD. Nine studies met inclusion criteria, including 80,645 participants. The aim of this study is to investigate whether Ramadan fasting can affect these metabolic parameters in non-insulin dependent diabetes mellitus (NIDDM) patients. METHODS: A retrospective study of 36670 pregnancies registered and managed in the Department of Obstetrics and Gynecology, King Khalid University Hospital, Riyadh, Kingdom of Saudi Arabia (KSA) from March 1993 to February 2003. OBJECTIVE: In end-stage renal failure, dyslipoproteinemia is linked to risk of cardiovascular disease. Increased concentrations of triacylglycerol-rich, very low density lipoproteins (VLDL) and decreased concentrations of high density lipoproteins (HDL) are usual, whilst total cholesterol and low density lipoprotein (LDL) concentrations are not increased. Medieval Arab scientists established the basis of medical practice and gave important attention to the publication of scientific results. At present, modern scientific publishing in the Arab world is in its developmental stage. Arab biomedical journals are less than 300, most of which are published in Egypt, Lebanon, and the Kingdom of Saudi Arabia. Yet, many of these journals do not have on-line access or are indexed in major bibliographic databases. OBJECTIVE: To estimate the magnitude and determinants of refractive error in school children, a study was undertaken to review the school screening and refraction data. METHODS: Trained physicians screened 416,157 students to evaluate their visual status and identified 28,765 students with defective vision. Refractionists refracted 25,733 (89.5%) of them, determined the refractive error and prescribed spectacles. Students with ocular co-morbidity and visual disability were re-examined and treated by the ophthalmologists.
