Multivariable Prognostic Model For Dialysis Patients With End Stage Renal Disease: An Observational Cohort Study Of Pakistan By External Validation, Maryam Siddiqa, Alan Charles Kimber, Javid Shabbir
Saudi Medical Journal
Objectives: To develop an externally validated multivariable prognostic model for an underprivileged dialysis population. Methods: This was a multicenter retrospective cohort study of 5 years duration from January 2013 to December 2017. A total of 758 patients (37.5% female; mean±SD age, 44.26±14.77 years) were enrolled for construction of the prognostic model. The data were analyzed using a proportional hazards model to identify predictors of survival. Three risk groups were identified at the 25th and 75th percentiles of the resultant prognostic index. The model was externally validated with another dataset of 622 dialysis patients. Results: The prognostic index included 5 predictor …

Saudi Medical Journal
Objectives: To investigate whether gastric Helicobacter pylori (H. pylori ) infection is associated with the progress of nonalcoholic fatty liver disease (NAFLD) in Dali city, China. Methods: One thousand and one hundred eighty-five adults who had visited a health examination center were enrolled. Nonalcoholic fatty liver disease and H. pylori infection were assessed by abdominal color ultrasound and 13 C-urea breath test, respectively. Serum lipid metabolic indices were compared between NAFLD subjects without and with H. pylori infection. Results: The prevalence rate of NAFLD for the entire study population (n=1185) was 44.6% (n=529), including 167 women and 362 men. Among …

Kawasaki Disease In An Infant After Administration Of Hexavalent Vaccine, Meshal Z. Almeshary, Saud A. Alanazi, Khalid M. Almoosa, Rolan K. Bassrawi
Saudi Medical Journal
Kawasaki disease is a vascular disorder of unknown etiology that affects children. Kawasaki disease mainly involves medium-sized blood vessels and may cause cardiovascular complications, particularly coronary artery aneurysms. Concern has been raised against various types of vaccines becoming potential risk factors for Kawasaki disease. Here, we describe a case of a 4-month-old Saudi infant who presented with incomplete Kawasaki disease a few hours after receiving his hexavalent vaccine and there was a significant dilatation of all coronary arteries. Although a relationship between vaccinations and Kawasaki disease has been suggested, there is no strong evidence of an increased risk or causal …

Objectives: To assess the sex hormone levels in young Saudi female migraineurs during a migraine attack and during pain-free periods and compare them with control subjects. Methods: A case-control study involving 14 Saudi female migraineurs and 21 control subjects was conducted between December 2019 and March 2020. Demographic and disease history data were collected through participant interviews. Blood samples were drawn during the migraine attack and pain-free periods. Results: Follicular (30.00±19.60; p <0.001) and luteal (39.79±11.45; p =0.037) estrogen levels were significantly higher in patients with non-menstrual related migraine (NMM), while luteal testosterone levels (1.10±0.31; p =0.023) were significantly higher in patients with menstrually related migraine (MM). Body mass index (BMI) was higher in patients with NMM (25.77±6.53; p =0.013), and it was found to be associated with follicular estrogen ( p =0.016), progesterone ( p =0.018), and pain intensity ( p =0.042). Luteal estrogen level was significantly lower (13.96±7.88; p =0.036) in patients with luteal onset of attack. Conclusion: High estrogen levels were found to mediate NMM, their effect being more pronounced with increase in BMI; whereas low luteal estrogen levels mediated MM. Young females with MM might have high luteal testosterone levels, and a compensatory protective role could be surmised accordingly.

Extracorporeal membrane oxygenation (ECMO) is considered as a supportive treatment that provides circulatory and ventilatory support and can be thought off as a bridge to organ recovery. Since 2009, it has been applied as a rescue treatment for patients with severe adult respiratory distress syndrome (ARDS) mainly due to viral causes. In December 2019, several patients presented with a constellation of symptoms of viral pneumonia in China. A new strain of the corona virus family, called COVID-19, has been discovered to be the cause of this severe mysterious illness that was named severe acute respiratory syndrome coronavirus 2 (SARS‑CoV‑2). Prevalence And Risk Factors Of Parasitic Diseases Among Saudi Children: An Updated Review, Jamila S. Al-Malki
Saudi Medical Journal
Intestinal parasite infection (IPI) is a declining global health problem. However, in certain parts of the world, the misconception that certain diseases are avoided owing to the existence of prevention strategies, medication and the frequency of awareness campaigns remains underestimated. Recent studies in Saudi Arabia have reported high prevalence rates for IPI; however, the prevalence in Saudi Arabia varies from city to city and from age group to age group. Moreover, the most prevalent causative species vary in different areas of Saudi Arabia due to differences in the characteristics of emerging populations. The prevalence of IPI has also been correlated …

Economic Perspective Of Evaluating Fertility Treatment In Obese And Overweight Infertile Women, Munazzah Rafique, Ahmad Al-Badr, Ahmed Saleh, Dania H. Al-Jaroudi
Saudi Medical Journal
Objectives: To evaluate direct cost of in-vitro fertilization/intracytoplasmic sperm injection (IVF/ICSI) cycle and reproductive outcomes among infertile women with different body mass index (BMI). Methods: A retrospective study of 826 subfertility patients who had IVF or IVF-ICSI in 2017 to 2018 were reviewed. The patients were divided into 4 groups bestowing to BMI to normal weight (18.5-24.9 kg/m 2 ), underweight (<18.5 kg/m 2 ), overweight(25-29.9 kg/m 2 ), and obese(≥30 kg/m 2 ). Data on treatment costs of IVF/ICSI and reproductive outcomes were collected and analyzed. A total of 338 patients (40.9%) were overweight, and 300 (36.3%) patients were obese. A bottom-up methodology was used to measure the resource utilization. The capital inputs needed for individual procedures were defined and calculated by consulting with appropriate clinicians and priced using market prices for 2017-2018. Results: There was no statistical significant difference for live birth rate (LBR) among the BMI groups, with the occurrence of LBR in 23 women with normal BMI (13.1%), in 48 (14.2%) women who were overweight, in 48 (16%) in women who were obese ( p =0.7). The median cost for IVF/ICSI treatment cycle did not differ across BMI groups; the cost was 10,380 SAR for women of normal weight, 10,440 SAR for women who are overweight and obese ( p =0.6). Conclusion: Our results suggest that costs of IVF/IVF-ICSI is not significantly affected in women who are overweight or obese.
