Factors Affecting Dimensions Of The Ethmoid Infundibulum And Maxillary Sinus Natural Ostium In A Normal Population, Mohammed A. Basurrah, Soo W. Kim

Saudi Medical Journal

Objectives: To investigate the ethmoid infundibulum (EI) and maxillary sinus natural ostium (MSNO) dimensions in normal sinuses of the Asian population; identified variation between sides, gender, and age groups; and to assess the effects of such variation on the measurements. Methods: We assessed EI dimension and MSNO diameter in computed tomography (CT) scans of the normal paranasal sinus of 100 patients who underwent trans-sphenoid endoscopic surgery. We compared demographic data and multiple anatomical variations. Results: The gap difference in EI length between the right and left sides significantly differed from 0 (0.47±1.38 mm). We found wider EI in people aged …

Physical Inactivity And Its Predictors Among Adolescents In Saudi Arabia: A Cross-Sectional Comparison Between Cities With And Without A Healthy Cities Program, Ibrahim Alasqah, Ilias Mahmud, Leah East, Nada Alqarawi, Kim Usher

Saudi Medical Journal

Objectives: To estimate the prevalence of insufficient physical activity and excessive screen time among adolescents living in cities participating in the Healthy Cities Program (HCP) and cities not running the program in the Qassim region of Saudi Arabia. Methods: We surveyed 1133 adolescents from the Qassim region using systematic random sampling between April and September 2017. Multivariable logistic regression analyses were conducted to investigate the predictors of insufficient levels of PA and excessive screen time. Results: The prevalence of <60 minutes of moderate- to vigorous-intensity physical activity/day was 82.4% and <3 days of vigorous intensity physical activity/week among adolescents was 59%. There was no significant association between living in healthy cities (HCPs) and insufficient levels of physical activity. The odds of excessive screen time were higher in HCPs than in cities not running the program (NHCPs) (OR: 1.49). The odds of insufficient daily PA (OR: 2.19) are higher among girls than boys. Increasing age is positively associated with insufficient weekly PA (OR: 1.19). The prevalence of excessive recreational screen time is 84.6% with higher odds in HCPs than in NHCPs (OR: 1.51). Conclusion: Our findings report a lack of evidence of any impact of the HCP on adolescents’ physical activity behaviors. This outcome warrants a further in-depth evaluation of the process and outcomes of the HCP in Saudi Arabia.

The Future Of Medical Thoracoscopy In The Kingdom Of Saudi Arabia, Enas M. Batubara, Ahmed S. Ibrahim, Suliman M. Alamro

Saudi Medical Journal

Medical thoracoscopy (MT) has changed how we manage exudative pleural effusion. It is a minimally invasive procedure used as a diagnostic and therapeutic tool in pleural disease. Here, we report a case of a lymphocytic exudative pleural effusion that needed a pleural biopsy for diagnosis. Medical thoracoscopy was performed, a biopsy was taken, and adhesiolysis was performed. Medical thoracoscopy has been practiced for a while worldwide, but it has not been utilized in the Kingdom of Saudi Arabia and as we believe that it is useful in diagnosing exudative pleural effusions. It limits patients in hospital-stay and it may be …

Effectiveness Of Using Tranexamic Acid In Total Hip And Total Knee Arthroplasty: Single Tertiary Center Experience, Khalid Al Sheikh, Ali Alhandi, Abrar Bin Dohaim, Khawlah Ateeq, Khalid Alaqeely

Saudi Medical Journal

Objectives: To measure the effectiveness of tranexamic acid (TXA); in total joint arthroplastyin terms of reducing post-operative blood loss. Methods: A retrospective cohort study in which patients who had underwent total hip arthroplasty (THA) and TKA between 2010-2018 in a single tertiary center. Those had received TXA (group 1) and those who had received no-TXA (group 2) were evaluated. Unpaired student’s t-test was used to compare TXA and no-TXA groups for continuous variables and Chi-square test was used for categorical variables. A p- value<0.05 was considered significant. Results: A total of 385 patients were included, 180 patients received TXA, and 205 patients received no-TXA. Post-operative blood loss was significantly less in TXA group (13.41±9.51, p <0.001). Total drainage output was significantly lower in TXA group compared to no-TXA (373.98±246.12 vs. 487.11±307.13 mL, p <0.001). Blood transfusion units were significantly less in TXA group than in the control group (20 units; 11.1% vs. 50 units; 24.4%, p <0.001). Conclusion: Tranexamic acid use in TKA and THA is effective in reducing post-operative blood loss in terms of drop in Hg level, total drainage output and blood transfusion rate.

Clinicopathological Characteristics Of Young Versus Older Patients With Breast Cancer: A Retrospective Comparative Study From The Madinah Region Of Saudi Arabia, Abdulkader M. Albasri

Saudi Medical Journal

Objectives: To compare the clinicopathological, histological and immunohistochemical features of breast cancer (BC) in between young (≤40 years) and older (>40 years) patients. Methods: This retrospective study included histopathological reports of all patients with primary BC diagnose at the King Fahad Hospital, Madinah, Saudi Arabia between January 2006 and November 2020. Results: During the study period, a total of 708 patients with BC were identified; 173 cases (24.4%) were ≤40 years old and 535 (75.6%) were above 40 years. There were significant differences in tumor size ( p =0.046), tumor grade ( p <0.0001), tumor stage ( p =0.047), lymphovascular invasion ( p <0.0001), and distant metastasis ( p <0.0001) between the 2 groups. Comparing the status of the hormone receptors and molecular subtypes between the 2 age groups, BCs in the younger patients were triple–negative ( p =0.008) in majority of the cases and less likely to be estrogen receptor (ER) and progesterone receptor (PR) hormone receptors–positive ( p =0.0001, p <0.0001). Young patients exhibited the HER2/neu type with more frequent statistical significance ( p =0.016) and the tumor type luminal A ( p =0.002) with less frequent statistical significance compared to old patients. Conclusion: Although BC in young women is uncommon, it may be different with regards to poor prognostic outcomes, which are attributed to more aggressive histopathological features and less favorable receptor status when compared to the cases diagnosed in older women.
