OBJECTIVE: Idiopathic headache is a stressful complaint, whose treatment is not well established. Our study is an attempt to examine the relationship between idiopathic headache and inferior turbinate cauterization as a surgical treatment, and to explain the central physiological effect of cauterization. METHODS: Thirty-four patients, 15 females and 19 males complaining of idiopathic headache were treated by inferior turbinate cauterization. Cauterization of the inferior-medial aspect of the turbinate was carried out once under local anesthesia for few seconds. OBJECTIVE: The aim of this study is to screen infants admitted to the Neonatal Intensive Care Unit (NICU) for hearing loss. METHODS: One hundred and thirty newborns admitted to NICU were screened for hearing loss using otocaustic emission (OAE). Tympanometry was performed using Grason Stadler Incorporation (GSI) 37 and auditory brain response (ABR). RESULTS: Ninety-six of the infants had known risk factors such as prematurely, low birth weight, hyperbilirubinemia, asphyxia and congenital abnormalities. Out of 130 infants 80 passed the OAE test, 50 had to be rescreened, and 19 had ABR. Only 13 were found with moderate to severe hearing …

Silicosis is an occupational respiratory disease caused by inhaling respirable crystalline silica dust. We report the case of silicosis in a 50-year-old woman that presented with pulmonary fibrosis and later developed pulmonary tuberculosis. Her condition is believed to be due to inhalation of dust produced by scrubbing a silica rich surface during usual daily housework.

Traumatic fat embolism syndrome occurs most often following fractures of long bones sustained in road traffic accidents and is a common cause of medical consultation from the orthopedic surgery department. The sub-clinical presentation is subtle and expresses itself by the presence of hypoxemia, while the full clinical syndrome compromises respiratory insufficiency, an altered consciousness and a characteristic petechial rash. Recognition is simple once the patient is viewed in the context of his or her clinical setting. Diagnosis is aided further by the presence of hematological and biochemical abnormalities including anemia, thrombocytopenia, an elevated erythrocyte sedimentation rate and fat macroglobulinemia. OBJECTIVE: The aim of the study is to assess the pregnancy outcome among Omani women with sickle cell trait (SCT), and to compare it with a control group of Omani women with normal hemoglobin. METHODS: Relevant data of both groups was obtained from labor room records, patient files and computerized records at the Nizwa Hospital, Dakhliya Region, Nizwa, Oman from January 1999 to July 2000. Sickle cell anemia (SCA) is one of the commonly inherited hemoglobinopathies in the Kingdom of Saudi Arabia. It is characterized by periods of remissions and exacerbations called crises as well as certain pathological phenomenon such as acute chest syndrome, priapism, hepatopathy, and cerebrovascular stroke. Blood transfusion (BT) as therapy and prophylaxis in SCA, although was advocated as early as the 19409s, there are still debates regarding its benefits and risks. This is a review of the value of BT in patients with SCA with emphasis on the risks and benefits as well as guidelines towards safe BT.

OBJECTIVE: To report the results of repeat intracytoplasmic sperm injection (ICSI) after complete failure of fertilization with initial ICSI. METHODS: The medical records of the couples undergoing repeat ICSI at the Human Reproductive Biology Unit, Soliman Fakeeh Hospital, Jeddah, Kingdom of Saudi Arabia, after complete failure of fertilization by initial ICSI between December 1994 and December 2000 were retrospectively examined. RESULTS: Seven hundred and eighty two oocytes from 146 women failed to fertilize by initial ICSI. The main indications for the procedure were severe oligoasthenoteratozoospermia or azoospermia. Complex congenital anomalies of the mullerian ducts can occur in isolation or in association with other developmental disorders. They result from non-development or non-fusion of the mullerian ducts or the failure of reabsorption of the uterine septum. Early diagnosis is necessary to relieve symptoms, optimize preservation of the genital organs and prevent the development of endometriosis. We present a case report to highlight this phenomenon.

Laparoscopic Versus Open Appendectomy In Females With A Clinical Diagnosis Of Appendicitis, Abdulrahman S. Al-Mulhim, Faisal M. Al-Mulhim, Abdulmohsen A. Al-Suwaiygh, Nabil A. Al-Masaud

OBJECTIVE: Appendectomy can be performed using either a laparoscopic technique (LT) or an open technique (OT). We compared the following items operative, anesthesia, length of stay, post-operative pain, medicine, wound healing, days to return to normal activity in both groups. METHODS: This study was carried out at King Fahad Hospital, Hofuf, Al-Hassa, Kingdom of Saudi Arabia, from January 1999 to April 2000. We randomly assigned 60 female patients to appendectomy by LT or OT. The 2 groups were compared concerning demographic data. The differences were considered statistically significant at a P value < 0.05. RESULTS: The open group had shorter anesthesia and operative time (68, 50 versus 85, 65 minutes). The laparoscopic group had a significant reduced postoperative narcotic requirement (P<0.05), quicker reintroduction of diet and quicker return to normal activity. CONCLUSION: Laparoscopic appendectomy in female patients with clinical diagnosis of appendicitis is the procedure of choice for the diagnosis and the management of acute appendicitis.
