OBJECTIVE: To examine the preferred patient approach to chronic suppurative otitis media (CSOM) of the safe type. METHODS: We designed a prospective study to note the variables in the patient9s initial choice for approach to CSOM of the safe type at Buraidah Central Hospital, Al-Qassim, Saudi Arabia from January 2003 to December 2004. RESULTS: During the 24-month study period, we included 63 consecutive patients diagnosed with CSOM of the safe type. We excluded patients with perforations associated with cholesteatomata, polyps, and acute infections. OBJECTIVE: To investigate the prevalence of a group of different autoantibodies, in Omani patients with gastric cancer, and to examine whether their presence correlates with clinical course of the disease. METHODS: Ninety-three Omani patients with gastric cancer, and 100 gender-matched blood donors were investigated for the presence of 15 different auto-antibodies against nuclear antigens (ANA), extractible nuclear antigens (ENA), Scleroderma antigen (Scl-70), Sjogren syndrome antigen A/B (SSA/B), Smith antigen (Sm), ribonucleoprotein (RNP), Jo-1 antigen, double stranded DNA (ds-DNA), parietal cell antibodies (APCA), reticulin antibodies (ARA), smooth muscle antibodies (ASMA), proteinase 3 (PR3), myeloperoxidase (MPO), and mitochondria antibody (AMA). OBJECTIVE: To assess the awareness, attitude and practice of evidence base medicine (EBM) among Al-Taif Consultant Physicians and to define the perceived barriers for practicing EBM. METHODS: It is a cross-sectional study conducted during December, 2004, at all governmental hospitals in Western region of Saudi Arabia at Al-Taif Governorate. Two hundred and eight self administrated questionnaires had been distributed to all consultant physicians in different specialties who met the inclusions criteria for this study. Atherosclerosis begins with the accumulation of small lipoprotein particles within arterial intima. These particles coalesce together and are modified and then they induce localized endothelial inflammation, thereby attracting leukocytes. Scavenger receptors over the surface of monocytes bind to the modified low density lipoprotein particles, which transform into foam cells that become a source for further inflammatory cytokines. The level of inflammation is heightened in ruptured coronary plaques. In patients dying of an acute myocardial infarction, the level of inflammation is heightened in all lesions in the coronary tree. OBJECTIVE: To assess the effects of green and black tea on carbon tetrachloride (CCl4)-induced oxidative stress in rats. METHODS: This study was completed in the period of January to April 2005. Male wistar albino rats (6 weeks old) were randomly assigned to one of 3 diets (12 rats/group), control diet, black tea diet, or green tea diet. At the end of dietary treatment, a single dose of CCl4 (2.5 ml/kg intraperitoneally) in paraffin (1:1 v/v) was given to 6 rats in each group after overnight fasting. OBJECTIVE: The human epidermal growth factor receptor-2 (HER-2)/neu is a proto-oncogene that is amplified in 10-30% of breast cancers. It is known to be associated with a poor overall survival. We studied the relationship between its amplification and different histological gradings of breast cancer. METHODS: We studied 196 patients diagnosed with breast cancer in 2005 at the Omid and Ghaem Training Hospital, Mashhad Medical University, Iran. The HER-2/neu oncoprotein was measured by immunohistochemistry and the histological gradings were carried out according to the Bloom-Richardson Grading system. OBJECTIVE: To investigate the effect of low doses of subcutaneous testosterone in hypogonadal men since the intramuscular route, which is the most widely used form of testosterone replacement therapy, is inconvenient to many patients. METHODS: All men with primary and secondary hypogonadism attending the reproductive endocrine clinic at Royal Victoria Hospital, Monteral, Quebec, Canada, were invited to participate in the study. OBJECTIVE: To identify the causes that affect the time from the onset of symptoms to admission to the hospital, in patients with a diagnosis of acute myocardial infarction (MI). OBJECTIVE: To analyze the factors affecting survival of patients with chronic obstructive pulmonary disease (COPD) during the follow-up period using a 4-year prospective study. METHODS: The study included 276 out-patients with COPD. The study took place in Ankara University, Cebeci Hospital, Ankara, Turkey between September 2000 and January 2005. We used cox proportional hazards model in investigating the effects of clinical variables on survival. Risk factors related with mortality were analyzed. OBJECTIVE: To estimate the prevalence and clinical spectrum of gastroesophageal reflux disease (GERD) in a young population and evaluate the risk factors. METHODS: This cross-sectional study using a reliable and valid questionnaire was performed during June and July 2005, on randomized selection of 620 students form Azad University of Tabriz. Gastroesophageal reflux disease was defined as at least weekly heartburn and/or acid regurgitation. RESULTS: Mean age (± SD) of the responders was 22.48 ± 1.98 years. The knowledge of vascular variations like other anatomical variations, is important during the operative, diagnostic, and endovascular procedures in abdomen. This report describes multiple variations in the upper abdominal vessels as found during the routine dissection in a 60-year-old male cadaver. The variations found were; presence of a celiaco-mesenterico-phrenic trunk, a common inferior phrenic trunk, 2 right renal arteries originating from abdominal aorta, 2 suprarenal arteries originating from the lower right renal artery, 3 right renal veins opening separately into inferior vena cava, and termination of right testicular vein into the lowest vein among the 3 right renal veins. We described a Saudi patient infected with Diphyllobothrium latum D. latum. A 38-year-old male presented, complaining of passing worms. He had a history of recent travel to Europe and South East Asia. Stools examination revealed typical D. latum eggs. He was treated with praziquantel followed by saline purge, after which he discharged an intact tapeworm. Macroscopic and microscopic examinations of the worm confirmed the diagnosis of D. latum. This is the first case of diphyllobothriasis to be reported in Saudi Arabia. The epidemiology and methods of prevention of diphyllobothriasis are discussed.

Ant allergy is a rare clinical problem that ranges from local to systemic reaction and life-threatening anaphylaxis. Different types of ants including the imported fire ants, the black (samsum) ants, and others, are considered health hazard in many parts of the world. We report a 32-year-old Saudi female from Hafr-Al-Batin in the Northern region of Saudi Arabia, with history of recurrent anaphylaxis following black (samsum) ant stings and we review the related literature. This is the first report of black (samsum) ant allergy in Saudi Arabia.

OBJECTIVE: To assess whether Ramadan fasting affects the expiratory flow rates in healthy subjects, and to know if these effects correlate to a change in other variables. METHODS: This unmatched case-control longitudinal study includes 46 non-smoking healthy subjects who undertook lung function testing at the Aga Khan University, Pakistan. Expiratory flow rates and body mass were measured in 3 Islamic months, corresponding to November 2001 to January 2002. RESULTS: There was a significant reduction in body mass in Ramadan compared to pre and post Ramadan. No significant changes in expiratory flows were seen during Ramadan as compared to the pre …
