Evaluation Of Independent Prognostic Importance Of Hyperuricemia In Hospital Death After Acute Myocardial Infarction, Shahram Homayounfar, Mostafa Ansari, Khosro M. Kashani
Saudi Medical Journal
OBJECTIVE: To evaluate the role of hyperuricemia (serum uric acid level greater than 7 mg/dl) as an independent short term (in hospital) prognostic factor after acute myocardial infarction (AMI). METHODS: Included in the study were 2218 patients who were hospitalized with well established AMI from June 1996 through to December 2002 in the Coronary Care Unit of Ekbatan General Hospital, Hamedan University of Medical Sciences, Iran. All patients with exclusive criteria, were omitted from study. Furthermore, frequency of hyperuricemia in patients (N=59) who expired after AMI was compared with patients (N=104) whom were discharged from the hospital after AMI. RESULTS: …

Saudi Medical Journal
OBJECTIVE: To assess the levels of adiposity and physical activity among Saudi preschool children from Jeddah. METHODS: Participants included 224 Saudi preschool children, randomly selected from public and private preschools in Jeddah during April and May of 2006, using a multistage stratified sampling technique. Measurements included weight, height, body mass index, triceps and subscapular skinfolds, fat percentage, fat mass (FM), fat-free mass (FFM), FM index (FMI) and FFM index (FFMI), time spent watching television and physical activity levels using electronic pedometer for 3 continuous days during weekdays. RESULTS: The fat content averaged 20.6% of body weight, while the prevalence of …

High Prevalence Of Osteoporosis In Saudi Men, Mahmoud I. El-Desouki, Riad A. Sulimani
Saudi Medical Journal
OBJECTIVE: To determine the prevalence of osteoporosis in healthy Saudi men. METHODS: We randomly recruited 429 Saudi men from the community. The recruited Saudi men were subjected to an interview to reveal their lifestyle parameters, calcium intake and level of activity. Bone densitometry was assessed at lumbar spine (L1-4) and the femoral neck. The dual x-ray absorptiometry (DXA) scan was carried out in the Nuclear Medicine at King Khalid University Hospital, Riyadh, Saudi Arabia from September 2002 to December 2004. The World Health Organization definition of low bone mineral density was used. RESULTS: Poor oral calcium intake and low level …

Male Genital Self-Amputation In The Middle East, Samir S. Shirodkar, Fayez T. Hammad, Naseem A. Qureshi
Saudi Medical Journal
Genital self-mutilation, whether partial or complete, is a rare condition, which usually occurs in psychotic patients and occasionally has a religious background. The initial management of complete genital self-mutilation usually involves a formation of perineal urethrostomy or a more complex procedure to form a short penile stump. Here, we present a case of complete genital self-mutilation in a psychotic male who was managed with simple urethral spatulation to form an anterior urethrostomy.

METHODS: Data taken from the Coronary Artery Disease in Saudis Study which is a National Epidemiological Health Survey carried out between 1995 and 2000. Participants included 17395 Saudi males and females aged 30-70 years, selected randomly using a multistage stratified cluster sampling technique. Type 1 Diabetes-Related Epidemiological, Clinical And Laboratory Findings. An Evaluation With Special Regard To Autoimmunity In Children, Gulay Karaguzel, Seda Ozer, Sema Akcurin, Doga Turkkahraman, Iffet Bircan
Saudi Medical Journal
OBJECTIVE: To evaluate our data related with epidemiologic features, clinical presentation, and laboratory findings in children with type 1 diabetes mellitus (DM1) and to compare specific characteristics of immune-mediated subtype (DM 1A) with idiopathic one (DM 1B). METHODS: We classified 115 children with DM1 according to the presence (DM 1A, n=77) or absence (DM 1B, n=38) of diabetes-related autoantibodies in Akdeniz University Hospital, Turkey from January 2000 to December 2005. RESULTS: A total of 43 patients (37%) in the whole group, had onset of DM1 during the winter months and the lowest frequency occurred in summer (p<0.005). The duration of breast-feeding, introduction time of cow9s milk, and seasonal distribution of birth-month or onset of disease did not significantly differ in both groups. When compared with patients who had no documented honeymoon period, the patients who had a documented honeymoon period had lower HbA1c levels (p<0.01) at the onset. A large percentage of patients with DM 1A were positive for glutamic acid decarboxylase antibody (GAD65). CONCLUSION: There was no significant difference between patients with DM 1A and DM 1B with respect to epidemiologic features, and clinical presentation suggested that these factors do not play a major role either in creating a disease-initiating effect or in the development of islet autoimmunity. However, determination of GAD65 with HbA1c levels at the onset of the disease may ensure some useful information regarding clinical course.

Incidence Of Ectopic Pregnancy After Assisted Reproduction Treatment, Mohammed Alsunaidi
Saudi Medical Journal
OBJECTIVE: To assess the ectopic pregnancy risk among women who conceived with assisted reproductive technology (ART) procedures. METHODS: We performed a retrospective analysis of pregnancies as a result of ART procedure at McGill University Health Center, Quebec, Canada for the period from 1997 to 2006. RESULTS: The ectopic pregnancy rate was 1.6% and the heterotopic pregnancy rate was 13%. Maternal age, number of embryos transferred, ovarian hyperstimulation syndrome, use of assisted hatching, and Intra Cytoplasmic Sperm Injection (ICSI) were not significant predictors of ectopic pregnancy. CONCLUSION: Ectopic pregnancy rate in ART pregnancies is comparable to that of spontaneous pregnancies.

Hypoplastic Left Heart Syndrome And Valvular Pulmonary Stenosis. A Rare Association That Limits The Management Options, Fuad I. Abbag, Abdulla A. Alhayani, Hind A. Moussa, Jobran M. Al-Qahtani, Ahmed A. Al-Barki
Saudi Medical Journal
A term baby had cyanosis 2 hours after delivery; echocardiography showed hypoplastic left heart syndrome HLHS and valvular pulmonary stenosis PS. The opinions of the cardiac centers in Saudi Arabia were taken; however, the baby was not accepted probably because the associated PS made him a poor candidate for a Norwood procedure and because cardiac transplant is not available for infants in Saudi Arabia. He died after 15 days. This mother should have fetal echocardiography in future pregnancies, and if HLHS is suspected she might be advised to seek medical advice antenatally at highly specialized cardiac centers abroad, as delivery …

Familial Hypercholesterolemia, Parvaiz A. Koul, Rafi A. Jan, Abdul B. Wahid, Tariq A. Bhat, Syed M. Mudassir
Saudi Medical Journal
Familial homozygous hypercholesterolemia is a rare autosomal disorder characterized by high levels of cholesterol, extensive tendon xanthomatosis and premature development of atherosclerotic disease. Early coronary artery disease with myocardial infarctions and sudden deaths are common. We reported a family of familial hypercholesterolemia from the Kashmir valley of the Indian subcontinent. The appearance and the severity of the cutaneous xanthomas was found to be age related suggesting a role for the duration of hypercholesterolemia in the development of xanthomatosis.

RESULTS: We showed that the ELR-CXC chemokine, IP-10/Mob-1 is secreted from isolated rat hepatocytes immediately after isolation and early during culture and IP-10/Mob-1, expression by hepatocytes was also stimulated in response to heat shock. CONCLUSION: …
