Risk Of Malignancy In Thyroid Nodules Bethesda Iii Sub Classification Into Nuclear Atypia And Architectural Atypia. A Retrospective Study
Saudi Medical Journal
ABSTRACT Objectives: To identify the risk of malignancy in the Bethesda III category based on histopathological subclassification. Methods: We retrospectively analyzed 193 patients with Bethesda III thyroid nodules who underwent surgical resection. The primary outcome was the malignancy risk associated with each histopathological sub-classification. Results: Of 193 patients, final histopathology revealed malignant nodules in 96 (49.7%). The malignancy rates varied among the Bethesda III subcategories, with Hürthle cell atypia of undetermined significance demonstrating the highest rate (55.6%), followed by cytological atypia (55.4%), architectural atypia (50.6%), and combined cytological and architectural atypia (33.3%). However, no significant difference in malignancy rates was …

Effects Of Antidiabetic Drugs On The Level Of Serum Uric Acid In Patients Who Have Type 2 Diabetes, Gang Wang, Guixia Wang
Effects Of Antidiabetic Drugs On The Level Of Serum Uric Acid In Patients Who Have Type 2 Diabetes, Gang Wang, Guixia Wang
Saudi Medical Journal
ABSTRACT Hyperuricemia and type 2 diabetes mellitus (T2DM) are interconnected, in that each disorder increases risk for the other. Some antidiabetic drugs may decrease the level of serum uric acid (SUA). This narrative review describes the effects of multiple antidiabetic drugs on the SUA level and their possible mechanisms of action in patients with T2DM. The results showed that sodium glucose contransporter-2 inhibitors (SGLT2is), thiazolidinediones, metformin, and linagliptin decreased the SUA; insulin, sitagliptin, and alogliptin increased the SUA; and glucagon-like peptide-1 receptor agonists (GLP-1 RAs), sulfonylureas, and alpha-glucosidase inhibitors had little effect on the SUA. Sodium glucose contransporter-2 inhibitors appeared …

Saudi Medical Journal
Objectives: To assess the effectiveness and safety of somapacitan, a long-acting growth hormone analog, as a promising treatment to improve adherence in pediatric growth hormone deficiency (GHD) management. Methods: The observational study included 184 children diagnosed with GHD. Carried out over 72 weeks at the Pediatric Endocrinology Outpatient Clinic in collaboration with King Abdulaziz University Hospital in Jeddah, Saudi Arabia. Patients received weekly somapacitan doses, starting at 0.16 mg/kg and adjusted according to insulin-like growth factor 1 (IGF-1) levels. Key outcomes included changes in height velocity and IGF-1 levels, with statistical significance set at a p -value of <0.05. Results: The cohort, with an average age of 10.1 years and 67.4% male, showed substantial improvements by week 72: IGF-1 levels increased from 166.97 to 363.74, and height velocity gains were most pronounced in children at Tanner Stage 1. Treatment-naive children exhibited greater initial height velocity improvements compared to those switching from daily GH therapy, although these differences leveled over time. Somapacitan was well-tolerated, with minimal side effects, supporting its long-term safety. Conclusion: Weekly somapacitan offers an effective, safe pediatric GHD treatment, with individualized dosing based on IGF-1 levels. Routine IGF-1 monitoring is advised to maximize benefits and minimize risks. Further studies on diverse populations are recommended to validate these findings in pediatric endocrinology.

ABSTRACT Splanchnic artery aneurysms, particularly those involving the left gastric artery, are infrequent occurrences and often lack noticeable symptoms, but their rupture involves a significant risk of mortality. We present a case involving an unruptured left gastric artery aneurysm (LGAA) that extended into the right hemithorax. This case report outlines the categorization, clinical manifestations, and diagnostic challenges associated with such aneurysms. Our findings could offer valuable insights for clinicians and radiologists when adopting a systematic approach for the early detection and management of LGAA.
