OBJECTIVE: To conduct a comparative study of sufentanil and fentanyl on emergence agitation in pediatric patients. METHODS: We conducted a prospective, randomized, and double-blind study in 80 preschool children admitted at the Pediatric Surgery, Mianyang Central Hospital, Sichuan, China between March 2011 and April 2012. They underwent sevoflurane anesthesia for elective repair of unilateral inguinal hernia. Children received a single intravenous dose of sufentanil 0.15 ug/kg or fentanyl 1.5ug/kg just before skin incision. The emergence agitation scale and the frequency of severe emergence agitation were measured. Patients who required additional fentanyl during surgery and postoperative rescue fentanyl were recorded. OBJECTIVE: To investigate the efficacy and safety of deep sclerectomy (DS) in Saudi patients with primary and secondary open-angle glaucoma (OAG). METHODS: In this retrospective cohort study, a total of 194 eyes of 152 patients with OAG were consecutively enrolled to undergo DS with Mitomycin-C (MMC) at King Abdulaziz University Hospital, Jeddah, Kingdom of Saudi Arabia between January 2002 and September 2010. OBJECTIVE: To investigate the frequency of breast cancer related lymphedema in our region, and to heighten its awareness among health workers. METHODS: This is a cross-sectional study of patients treated with breast cancer at King Hussein Cancer Center, Amman, Jordan, between January 2004 and December 2009. We excluded patients with bilateral breast cancer. A total of 515 patients were included. We asked the patients to complete questionnaires that included questions related to arm symptoms. Mucinous cystic neoplasms (MCN) with malignant sarcomatous stroma are rare aggressive tumors and there are few recorded cases. We report a case of MCN that had adenocarcinoma in situ and invasive adenocarcinoma with foci of sarcomatous stroma in a 40-year-old woman. Clear transition from adenocarcinoma areas into sarcomatoid foci was noted. The stromal component showed immunoreactivity for CK7 and Cam 5.2 supporting epithelial origin of the sarcomatoid areas. Associated areas of cytologically benign MCN epithelium were present and were immunoreactive for positive staining with pan-cytokeratin (AE1/AE3), cytokeratin 7 (CK 7), cytokeratin 20 (CK 20), pan-cytokeratin (Cam 5.2), epithelial membrane antigen …

The ocular involvement of tuberculosis is not uncommon but diverse. Rarely, patients initially present with ocular signs that simulate intraocular malignancy. We report 3 cases of isolated presumed choroidal tuberculoma masquerading as intraocular tumor. Two patients had no systemic evidence of tuberculosis with strongly positive purified protein derivative skin test and one patient had evidence of inactive old pulmonary tuberculosis demonstrated by chest computed tomography. Antituberculous regimen including isoniazid 5 mg/kg/day, rifampicin 600 mg/day, ethambutol 15 mg/kg/day, and pyrazinamide 30 mg/kg/day were employed in all cases for 9 months. We describe the third confirmed case of novel coronavirus infection in a resident of the Arabian Peninsula. Our patient presented, as did 2 prior cases, with severe pneumonia and renal dysfunction requiring intensive care support including assisted ventilation. However, unlike the earlier cases, and despite underlying chronic disease and a single kidney, he survived his infection and has been discharged home. The Ministry of Health continues active surveillance for additional cases. As this case report goes to press, 2 additional confirmed cases have been identified in Riyadh, Saudi Arabia. Foreign objects in the female genital tract neglected for a long period of time may lead to many clinical problems including recurrent vaginal discharge and infertility. We present a case of 3-year long infertility, and 11-year long recurrent vaginal discharge in a 24-year-old female. She was treated empirically with several courses of antibiotics, which did not resolve her symptoms. Before her marriage, the family declined initial attempts to perform a physical examination due to fear of tearing the hymen, which has many social implications in the Kingdom of Saudi Arabia. Current published data makes clear the relationship between genital human papillomavirus (HPV) infection and cervical cancer. Although there is an opportunity for screening programs that could obviate the disease, cervical cancer still remains the second most common cancer among women worldwide. The subtypes HPV 16 and 18 affect the anogenital tract mucosal surfaces, and accounts for nearly 70% of all cervical cancers, and 90% or more of anal cancer cases. When the 493,000 new cases of cervical cancer and 274,000 deaths per year are taken into consideration, the importance of treatment and prophylaxis modalities for HPV can clearly be recognized. …

METHODS: We conducted a cross-sectional survey study among patients attending a chronic disease clinic at the Family and Community Medicine Department, Prince Sultan Military Medical City, Riyadh, Kingdom of Saudi Arabia between June and September 2010. Adherence intention was assessed using Modified Morisky Scale. Predictors of low motivation and/or knowledge were determined using logistic regression models. OBJECTIVES: To assess the nutrition and health status, nutrients intake, and physical activity among Saudi medical students. METHODS: A cross-sectional survey using a questionnaire, anthropometric measurements, and laboratory assessments was conducted from January to May 2011 on 194 randomly selected Saudi medical students at Taibah University, Madinah, Kingdom of Saudi Arabia. The adequacy of nutrient intake was compared with the recommended daily intake (RDI) per the National Research Council. RESULTS: Caloric intake was derived from carbohydrates (72.1%), fats (19.4%) and proteins (8.4%). Severe neonatal primary hyperparathyroidism is a very rare type of primary hyperparathyroidism in pediatrics that might present during infancy with non-specific symptoms. We report an infant who presented with a very high level of calcium, yet responding to simple medications to draw the attention to pediatricians to this disease as early diagnosis and treatment will ameliorate its devastating outcome.

It is established that the prevalence of major depressive disorder (MDD) in coronary heart disease (CHD) populations is high and is associated with increased mortality. In this systematic review, we examined the evidence for the effective treatment of MDD in CHD patients by reviewing randomized control trials (RCTs) between 1980 and 2011 and then assessing whether these treatments were clinically meaningful. A total of 8 RCTs were retrieved. Sertraline, citalopram, and mirtazapine were safe from a cardiac perspective, but only sertraline and citalopram were clearly more effective than placebo in CHD patients with moderate-to-severe type, recurrent MDD, or MDD episode …

OBJECTIVE: To conduct a cross-sectional analysis of the demographical, etiological, clinical pattern, and the outcome of pediatric burn injuries. METHODS: A cross-sectional study of 459 pediatric burn patients admitted to Al-Noor Specialist Hospital in Makkah, Saudi Arabia from January 2008 to December 2010 were evaluated using a structured questionnaire. RESULTS: The mean age was 5.97, and the male to female ratio was 1.5:1. The most common cause was scalding (81.7%). Approximately 92.8% of patients had burn injuries involving 25% of the total body surface area, or less. The mean hospital stay was 9.51 days. Approximately 92% of patients were treated …

OBJECTIVE: To investigate the effect of Khat (Catha edulis) acute administration on blood pressure (BP) and electrocardiogram (ECG) in vivo. METHODS: This study was performed between January and February 2009 at the Physiology Laboratory, Medical College of King Khalid University, Abha, Kingdom of Saudi Arabia. Two groups of Wistar rats (n=10), weighing 190-200 g were divided into control group and Khat treated group. Throughout the study, arterial BP and ECG were recorded for 60 consecutive minutes.
