Background: Child sexual abuse (CSA) in a preventable national public health problem that results in adverse health consequences. Research suggests many victims are currently attending school. Screening for early detection of health issues is integral to school nurse (SN) practice but there is little evidence that screening for CSA is occurring in schools despite recommendations that it should be. Lack of confidence and knowledge related to screening for suspected CSA contribute to a SN’s missed opportunity for early intervention.

The metabolism of phosphatidylcholine (PC), a major eukaryotic membrane lipid, has been shown to be an important aspect of pathogenesis in Candida albicans. Of note, PC turnover product, glycerophosphocholine (GPC), is abundant renal metabolite available within the host. GPC is readily imported by C. albicans, and deletion of the major GPC transporter, Git3, leads to decreased virulence in a bloodstream infection model. Here I have characterized two potential metabolic fates of internalized GPC. The first metabolic fate explored is internalized GPC can be hydrolyzed to release Pi. According to a 2022 report, organs destined for transplantation are discarded 15% of the time for various reasons (Israni et al. 2022). This highlights a critical need in transplant practices. Transplant donor tissues are in need of essential nutrients, including molecular oxygen, to maintain integrity during preservation. A current trend in preservation is to perfuse the ex vivo tissue graft with nutrient-rich aqueous perfusate on a closed circuit. Pollution resulting from abandoned mine drainage (AMD) is widespread across the nation and impacts thousands of miles of watersheds in Pennsylvania alone. AMD contaminants pose hazards to ecosystems and to human health by lowering the pH and by suspending toxic heavy metals in water. This dissertation consists of two projects investigating statistical methods in causal inference and personalized medication using observational data. In the first project, we propose a parametric marginal structural ordinal logistic regression model (MS-OLRM) to assess treatment effects on ordinal outcomes. Average treatment effect (ATE) is used to measure the difference of the mean outcomes if all patients would have been treated compared with the outcomes if they would not have been treated. Suicide has proven to be difficult to predict and prevent at the individual level,1–3 so much so that a recent analysis of individual risk assessments found a maximal positive predictive value of 5.5% for suicide death,2 and suicide continues to be a leading cause of death among adolescents age 10-19 years.4 By targeting modifiable risk factors at the population level, policy has the potential to extend our reach to people and subgroups that are missed by individual level interventions. Objective: This study examined differences between users and non-users of campus food pantry, conducted communication audit, and made recommendations for future food pantry communication. Methods: Cross-sectional twofold study, first analyzing existing intrapersonal, interpersonal, and environmental data to test predictors of campus food pantry usage characteristics, using inferential statistics. Secondly, conducting a communication audit that evaluates the reach of student communication. Results: Significant differences were found between users and non-users of food pantry that support hypotheses in relation to gender, age, race, class, marital status, housing type, housing description, and Pell eligibility. Loss of myelin causes severe neurological disorders and functional deficits in white matter injuries (WMI) such as traumatic spinal cord injury (SCI). This dissertation is focused on autophagy in OL development and the OL translatome after SCI. Chapter I describes the history of myelin, OL development, and their involvement in neurodegenerative diseases and SCI. The proteostasis network, in particular autophagy, and its contributions to white matter pathology is discussed. This study sought to explore the lived experiences of Dysphoric Milk Ejection Reflex (D-MER) during postpartum breastfeeding. A phenomenological method based on Giorgi’s phenomenological underpinnings was used. The setting for this study was virtual interviews in a secure online platform. The sample consisted of 13 participants, self-identifying as experiencing D-MER. Semi-structured interviews, with thematic analysis of transcription were conducted. Bystander-initiated CPR is an important aspect in the survival and outcomes of those that experience a cardiac arrest in the community. Eating disorders, including anorexia and bulimia nervosa, and binge-eating disorder, affect approximately 20 million women in the United States (National Eating Disorders Association, 2022d). The dominant approach to studying eating disorders is based on the master narrative of biomedicine. The present dissertation examines the existing master narrative by investigating the meaning that women themselves ascribe to their eating disorder experiences. Drawing upon notions of embodiment, the present dissertation proposes narrative theorizing as an appropriate lens through which to view women’s narrative sense-making processes and employs narrative research methods. While marker-based motion capture remains the gold standard in measuring human movement, accuracy is influenced by soft-tissue artifacts, particularly for subjects with high body mass index (BMI) where markers are not placed close to the underlying bone. Obesity influences joint loads and motion patterns, and BMI may not be sufficient to capture the distribution of a subject’s weight or to differentiate differences between subjects. Subjects in need of a joint replacement are more likely to have mobility issues or pain, which prevents exercise. Obesity also increases the likelihood of needing a total joint replacement. This dissertation was an exploratory study of experiences of fatness in boyhood using a hermeneutic phenomenological qualitative method. The author conducted in-depth, open-ended interviews with participants who identify as men and for whom fatness or related body-difference was an issue in childhood or adolescence to gather data on the meanings of fatness for boys and the men they become. Data analysis was organized around the existential dimensions of embodiment, temporality, and relationality. Objectives: This study analyzed the impact of Medicaid expansion in Kentucky on coverage of adults aged 19-64 and the diagnosis of type 2 diabetes mellitus (T2DM) and diabetic peripheral neuropathy (DPN) and the use of antidiabetic medications or insulin (antihyperglycemics) for T2DM, controlling for Social Determinants of Health (SDOH). Method: This study assembled ICD-based claims and prescription drug NDC claims from all adult residents of Kentucky enrolled in Medicaid from 2011 to 2018. Introduction Alcohol-associated liver disease (ALD) is a common chronic liver disease and healthcare burden, and a spectrum of pathologies ranging from steatosis to steatohepatitis, fibrosis, and alcohol-associated hepatitis (AH). Despite its prevalence, there are limited effective therapeutic options for the treatment of ALD/AH. Previous evidence suggested that inhibition of soluble epoxide hydrolase (sEH), a lipid metabolism enzyme which degrades beneficial metabolites of dietary polyunsaturated fatty acids, may be a beneficial strategy. Global change is a crucial process, affecting every organism, from the billions of humans on earth, to the billions of microbes in the soil belowground. Evaluating the impacts of the many drastic changes associated with global change is essential in advancing our understanding in how to mitigate these consequences. Alongside the production of basic ecological knowledge associated with these environmental changes, improving education practices to promote science literacy surrounding global change is another step towards a more optimistic future. Birth outcomes for Black women in the United States (US) are disproportionately worse in comparison to non-Black women, even when controlling for potential confounders. Existing studies suggest foreign-born (FB) Black women present better birth outcomes in comparison to US-born (USB) Black women. Also, the longer FB Black women live in the US, the more their perinatal health advantage diminishes. Racial discrimination has been hypothesized as a potential reason for this decline.
