An Evidence Based Tool For Safe Configuration Of Electronic Health Records: The Esafety Checklist., Pritma Dhillon-Chattha Jan 2018

An Evidence Based Tool For Safe Configuration Of Electronic Health Records: The Esafety Checklist., Pritma Dhillon-Chattha

Yale School of Nursing Digital Theses

Background: Electronic health records (EHRs) are transforming the way healthcare is delivered. They are central to improving the quality of patient care and have been attributed to making healthcare more accessible, reliable and safe. However, in recent years, evidence suggests that specific features and functions of EHRs can introduce new, unanticipated patient safety concerns that can be mitigated by safe configuration practices. Objective: To develop an evidence-based checklist of safe configuration practices for use by clinical informatics professionals to safely configure hospital-based EHRs. Methods: A literature review was conducted to synthesize evidence on safe configuration practices; data were analyzed to …

Validation Of A Toolkit On Innovative Business Models For Nurse Practitioner Practice, Sapana V. Patel Jan 2018

Validation Of A Toolkit On Innovative Business Models For Nurse Practitioner Practice, Sapana V. Patel

Yale School of Nursing Digital Theses

Background: NPs have a great opportunity to undertake entrepreneurial opportunities. However, according to the International Council of Nurses, “only 0.5%-1% of working nurses are nurse entrepreneurs” (ICN, 2004, pg. 9). This is primarily due to a lack of resources that support entrepreneurial inquiry.

Objective: To validate a toolkit on innovative business models for NP practice.

Methods: The toolkit was developed by: 1) conducting a comprehensive review of scientific literature, 2) reviewing the needs and barriers of entrepreneurship by interviewing experts and 3) validating the toolkit with a panel of experts.

Results: There was 100% agreement on the relevance and importance …

Essential Evidence-Based Introductory Bioterrorism Content For Practicing Nurses, Manu Sharma Jan 2018

Essential Evidence-Based Introductory Bioterrorism Content For Practicing Nurses, Manu Sharma

Yale School of Nursing Digital Theses

Bioterrorism content is too often absent from nursing education post-licensure. According to the Centers for Disease Control and Prevention (CDC), bioterrorism is defined as “the deliberate release of viruses, bacteria, or other germs (agents) used to cause illness or death in people, animals, or plants.” A literature search and a review of available resources elucidated pertinent content items on bioterrorism and its associated precautions for nurses. Next, two rounds of expert panel consisting of five members rated the identified content and proffered comments to validate the curriculum. Finally, a four-hour in-person pilot educational session was implemented based on the results. …

DEPERSONALIZATION IN THE EMERGENCY DEPARTMENT. Damian Apollo, Karen Jubanyik. Department of Emergency Medicine, Yale University, School of Medicine, New Haven, CT.

Yale medical students have given feedback about the emergency medicine rotations via the end of clerkship feedback and the annual student mistreatment questionnaires collected by the medical school and stated that they felt distress and that patient depersonalization occurred during resuscitations. Accordingly, the objectives of this study were to assess medical students’ perspectives with regards to resuscitation exposure, depersonalization, utilization of support services, and support services that students thought would be helpful as well as the timeframe in which …

Inter-Rater Reliability Of Physical Abuse Determinations And Abusive Fracture Incidence At A Level 1 Pediatric Trauma Center, Kate Buesser Jan 2018

Inter-Rater Reliability Of Physical Abuse Determinations And Abusive Fracture Incidence At A Level 1 Pediatric Trauma Center, Kate Buesser

Yale Medicine Thesis Digital Library

As there is no “gold standard” in determining whether a fracture is caused by accident or abuse, agreement among medical providers is paramount. Using abstracted medical record data from children <36 months of age presenting to a level 1 pediatric emergency department (ED), we examined the extent of agreement between specialists who evaluate children with fractures for suspected abuse. To simulate clinical scenarios, two pediatric orthopaedists and two child abuse pediatricians (CAPs) reviewed the full abstraction and imaging, whereas the two pediatric radiologists reviewed a brief history and imaging.

Each physician independently rated each case using a 7-point ordinal scale designed to distinguish accidental from abusive injuries. For any discrepancy in independent ratings, the two specialists discussed the case and came to a joint rating. We analyzed 3 types of agreement: (1) within specialties using independent ratings, (2) between specialties using joint ratings, and (3) between clinicians (orthopaedists and CAPs) with more versus less experience. Agreement between pairs of …

"I'M Used To Doing It By Myself": Exploring Self-Reliance In Pregnancy, Blair Mcnamara

Yale Medicine Thesis Digital Library

The goal of this thesis is to characterize self-reliance during pregnancy as described by a diverse urban cohort of women. We report on qualitative findings from a study conducted to explore the impact of a new pregnancy on women's lives based in New Haven, CT from June 2014 to June 2015. Each participant completed an enrollment survey and an in-depth semi-structured interview about intentions, thoughts, and feelings about her new pregnancy. We used framework analysis to identify concepts from our data and to assess thematic relationships. Eighty-four English-speaking women completed qualitative interviews. Participants averaged 26 years of age, and their …

The Impact Of Place-Based Determinants Of Health On Utilization Of Emergency Department Services, Tehreem Rehman

Yale Medicine Thesis Digital Library

The purpose of the study is to investigate whether higher prevalence of place-based structural determinants of health inequity is directly associated with frequent utilization of services in the emergency department. Chi-square and t-test analyses found that compared to non-frequent ED users, frequent ED users were older (mean age 43.18 vs. 35.23, p

Social support during pregnancy is an important indicator for improved maternal and neonatal birth outcomes. We sought to understand Spanish-speaking Latina women’s perspectives about social support and social isolation during pregnancy. We recruited Spanish-speaking women ages 15-44 at less than 24 weeks estimated gestational age from urban walk-in pregnancy care sites in New Haven, CT, during the period from June 2014 to June 2015. We collected quantitative and qualitative data via self- administered, in-person surveys and semi-structured interviews, respectively. Interviews were conducted to explore women’s pregnancy intentions, initial reactions to new pregnancy, and feelings about the impact of this pregnancy …
