Why Is The Ocean Surface Slightly Warmer Than The Atmosphere?, Lakshika Girihagama, Doron Nof Jan 2018

Why Is The Ocean Surface Slightly Warmer Than The Atmosphere?, Lakshika Girihagama, Doron Nof

Journal of Marine Research

How much warmer is the ocean surface than the atmosphere directly above it? The present study offers a means to quantify this temperature difference using a conceptual nonlinear one-dimensional global energy balance coupled ocean–atmosphere model ("Aqua Planet"). The significance of our idealized model, which is of intermediate complexity, is its ability to obtain an analytical solution for the global average temperatures. Our analytical model results show that, for the present climate, predicted global mean ocean temperature is 291.1 K whereas surface atmospheric temperature above the ocean surface is 287.4 K. Thus, the modeled surface ocean is 3.7 K …

High-Frequency Variability In The North Icelandic Jet, B. E. Harden, R. S. Pickart Jan 2018

High-Frequency Variability In The North Icelandic Jet, B. E. Harden, R. S. Pickart

Journal of Marine Research

We describe the high-frequency variability in the North Icelandic Jet (NIJ) on the Iceland Slope using data from the densely instrumented Kögur mooring array deployed upstream of the Denmark Strait sill from September 2011 to July 2012. Significant sub-8-day variability is ubiquitous in all moorings from the Iceland slope with a dominant period of 3.6 days. We attribute this variability to topographic Rossby waves on the Iceland slope with a wavelength of 62 ± 3 km and a phase velocity of 17.3 ± 0.8 km/day–1 directed downslope (–9° relative to true-north). We test the theoretical dispersion relation for these …

Self-Efficacy And Dependence In Personal & Clinical Relationships: A Qualitative Analysis Of Narratives About Life With Chronic Back Pain, Elizabeth Cummings Jan 2018

Self-Efficacy And Dependence In Personal & Clinical Relationships: A Qualitative Analysis Of Narratives About Life With Chronic Back Pain, Elizabeth Cummings

Yale Medicine Thesis Digital Library

Objectives: People with chronic back pain encounter considerable psychological and social challenges. Through personal narratives, this project examines the ever-evolving relationship between chronic back pain, sense of self-efficacy, and perceived role in interpersonal relationships, both in the community and within the health care system.

Methods: In-person semi-structured interviews were conducted with 20 adult patients attending a specialized interventional spine pain clinic. The interview transcripts were subjected to inductive thematic analysis, and themes were labeled descriptively. Participant responses were intentionally not analyzed within the context of an existing theoretical framework, so that the content of participant responses would directly drive the …

The S.H.I.V.E.R.I.N.G. Trials: A Novel Vaccine Strategy And Analysis Of Serologic Durability In Patients With Plasma Cell Dyscrasias, Eamon Duffy Jan 2018

The S.H.I.V.E.R.I.N.G. Trials: A Novel Vaccine Strategy And Analysis Of Serologic Durability In Patients With Plasma Cell Dyscrasias, Eamon Duffy

Yale Medicine Thesis Digital Library

ABSTRACT

Background: Patients with plasma cell dyscrasias are at an increased risk for infections due to their dysfunctional immune system. Each year these vulnerable patients are advised to receive the flu shot, but this vaccine has been shown to induce a serologic response that is not sufficiently protective in these patients. More effective methods for vaccinating patients with plasma cell dyscrasias are necessary.

Methods: The Study of High-Dose Influenza Vaccine Efficacy by Repeated dosing IN Gammopathy patients (SHIVERING 1) Pilot Trial was implemented at the Yale Cancer Center during the 2014-2015 flu season. Patients with plasma cell dyscrasias (n=51) received …

Smoking Cessation And Outcome After Ischemic Stroke Or Tia, Katherine Abigail Epstein Jan 2018

Smoking Cessation And Outcome After Ischemic Stroke Or Tia, Katherine Abigail Epstein

Yale Medicine Thesis Digital Library

People who smoke cigarettes are 2-4 times more likely to have a stroke and 2-5 times more likely to have a myocardial infarction compared with people who do not smoke. On a population level, it is estimated that smoking accounts for 33% of all cardiovascular deaths. Fortunately, quitting smoking can rapidly normalize risk. After an MI, smoking cessation reduces the risk of recurrent coronary events to the level of a never-smoker's after 3 years. While the benefits of smoking cessation in patients with cardiovascular disease are well-known, the benefits of smoking cessation after stroke have never been adequately examined and …

Improving Therapeutics In Head And Neck Cancer, Cyril Sergei Gary Jan 2018

Improving Therapeutics In Head And Neck Cancer, Cyril Sergei Gary

Yale Medicine Thesis Digital Library

Patients with head and neck cancer suffer not solely from disease but also from sequelae of radiation and chemotherapy treatment, and there is a large, unmet need to both develop novel therapies and improve existing ones to decrease deleterious, life-long side effects and improve patient survival. In the last several years, growing evidence has suggested that head and neck cancer represents two distinct disease entities based on Human Papilloma Virus (HPV) status. Unfortunately, HPV status is currently only used for prognosis and not for guiding management. Subsequently, this work focuses on 1) developing a novel targeted therapy for HPV-positive (HPV(+)) …

Reducing T Cell-Mediated Cardiac Injury With Cpg Oligodeoxynucleotides, Erik Alexander Barfod Levinsohn Jan 2018

Reducing T Cell-Mediated Cardiac Injury With Cpg Oligodeoxynucleotides, Erik Alexander Barfod Levinsohn

Yale Medicine Thesis Digital Library

Lymphocytic myocarditis is a common condition associated with both infectious diseases and immunological disorders, and is often associated with severe morbidity and mortality, but few effective treatments. In many cases, the pathophysiology involves a failure of central and/or peripheral immune tolerance leading to a cardiac-specific autoimmune T cell response. Previous studies indicate that the PD-1:PD-L1 axis plays an important role in limiting inflammation in the heart. CpG ODN are TLR9 agonists with known immunoregulatory capacity, in part through their potent induction of type I IFN, a known inducer of PD-L1. In this study, we used human tissue to determine the …

Importance Of Communication In Medicine: Views On Bedside Rounding And Readmissions, Allen F. Shih Jan 2018

Importance Of Communication In Medicine: Views On Bedside Rounding And Readmissions, Allen F. Shih

Yale Medicine Thesis Digital Library

Objectives: To study communication in medicine within the context of readmission rates and patient satisfaction, by assessing 1) the perspectives of primary care physicians (PCPs) and home care nurses (HCNs) on why older adults are readmitted to the hospital within 30 days of discharge, and 2) patient perceptions regarding the implementation and value of bedside rounding.

Design: Two studies were performed independently. 1) A qualitative study consisting of PCPs and HCNs of patients readmitted to the hospital within 30 days of discharge home. 2) A concurrent mixed methods study consisting of patients admitted to the inpatient medicine service who participated …

Quality Assessment Of Clinical Practice Guidelines In Emergency Medicine, Alyssa B. Zupon Jan 2018

Quality Assessment Of Clinical Practice Guidelines In Emergency Medicine, Alyssa B. Zupon

Yale Medicine Thesis Digital Library

Clinical practice guidelines (CPGs) have been published by the American College of Emergency Physicians (ACEP) since 1990 to advance the delivery of emergency medical care. These guidelines have raised controversy, and recent research shows that they are largely based on lower classes of evidence and expert opinion despite Institute of Medicine recommendations. The rigor of development and overall quality of these guidelines have not yet been assessed. Thus, we performed a systematic review and meta-analysis to evaluate the quality of ACEP Clinical Policies using a recognized, validated appraisal instrument: Appraisal of Guideline for Research & Evaluation (AGREE II).

The AGREE …

Convergence Towards An Optimal Currency Area In The European Union, Santiago Botto Tornielli Jan 2018

Convergence Towards An Optimal Currency Area In The European Union, Santiago Botto Tornielli

Harvey M. Applebaum ’59 Award

This essay uses the European Union as a case study to analyze the conditions for an optimal currency area, the regional differences that give rise to divergences from optimal conditions, and how to measure divergence. The essay reviews the extensive body of theoretical literature on optimal currency areas (OCA), analyzes the historical evidence and documents that outline the economic institutions of the EU, and performs an empirical factor analysis on different possible indicators of convergence to identify to the most important variables. In the absence of adjustment mechanisms, economic geography plays an essential role in forming an OCA because members …
