Addressing Nursing Retention: A Web-Based Approach Focusing On Joy In Work, Faye Christen Jan 2021

Addressing Nursing Retention: A Web-Based Approach Focusing On Joy In Work, Faye Christen

Yale School of Nursing Digital Theses

AbstractThe nursing workforce is in a crisis caused by critical imbalances which is evident through the nursing shortage, alarming rates of clinician burnout, and poor patient outcomes; this is expected to produce one million vacant positions within the next five years (U.S. Bureau of Labor Statistics, 2020). Turnover rates and burnout continue to increase nation-wide while the workforce remains disengaged (Dempsey & Assi, 2018). This project addressed nursing retention by promoting engagement, interconnectedness, and shared governance in a varied group of healthcare professionals working in the COVID-19 vaccine clinic at LAC+USC Medical Center. A virtual medium was created to implement …

Giving Voice To The Values Of Nurses, Sylvia K. Martin

Yale School of Nursing Digital Theses

Problem Nurses working on the frontlines in Labor and Delivery and Radiology face moral challenges in the workplace. It can be difficult for theses nurses to take morally courageous action when they lack the knowledge, skills and confidence to do so successfully. The goal of this DNP project was to enable nurses in labor and delivery and radiology settings to practice moral courage by examining relevant skillsets and providing training in core concepts. The program's intent was to support a moral culture and contribute to the retention of frontline nurses and improved safe patient care. Methods This project had three …

Patient-To-Nurse Sexual Harassment, Erin Frances Micale Jan 2021

Patient-To-Nurse Sexual Harassment, Erin Frances Micale

Yale School of Nursing Digital Theses

Patient to nurse sexual harassment is an issue that can have significant effects on nurses physical and mental health, occupational function, and patient outcomes. It is an under-researched and often unaddressed issue, leaving many nurses unsure of what sexual harassment by a patient is and how to respond to it. This paper reviews the literature related to patient to nurse sexual harassment and the development and effectiveness of a webinar on recognizing, preventing, intervening, and coping with patient to nurse sexual harassment. The webinar is a two-hour educational program that covers topics related to what sexual harassment is, risk assessment …

Evaluation Of A Nursing-Led Telephonic Self-Management Program For Patients With Copd On Health Care Utilization, Esther Pavlovsky Jan 2021

Evaluation Of A Nursing-Led Telephonic Self-Management Program For Patients With Copd On Health Care Utilization, Esther Pavlovsky

Yale School of Nursing Digital Theses

In 2010, it was estimated that the U.S. spent approximately $50 billion to treat chronic obstructive pulmonary disease (COPD). Nurse-driven interventions are essential in value-based care. The purpose of this quality improvement (QI) project is to reduce the number of preventable COPD-related admissions and emergency department (ER) visits by implementing a telephonic nurse-led evidence-based COPD self-management program. A 50-patient pilot has been initiated. A pre-and-post test will be used to evaluate the effect on preventable hospitalizations and ER visits. If the outcomes of the program are unfavorable, a root cause analysis will be conducted to obtain nursing feedback on perceived …

Reducing Early Hospital Readmission Rates After Bariatric Surgery, Payal Sharma Jan 2021

Reducing Early Hospital Readmission Rates After Bariatric Surgery, Payal Sharma

Yale School of Nursing Digital Theses

The prevalence of 30-day readmission after bariatric surgery is 0.6% to 11.3%, with a single hospital readmission nearly tripling the average 180-day cost of the surgery. Given that nearly 50% of early readmissions are preventable, close postoperative follow-up may allow for early identification of high-risk patients and preventative interventions.

This 7-month quality improvement project augmented clinical follow up post-bariatric surgery by incorporating a 7-10 day post-discharge call by an RD following a routine 1-3 day post-discharge call by an APP. Impact on readmission rate was examined. 166 participants included men and women ≥18 years of age, status post primary bariatric …

Preventing Hospital Readmission In The Sepsis Patient: A Multi-Modal Discharge National Framework, Ron Yolo Jan 2021

Preventing Hospital Readmission In The Sepsis Patient: A Multi-Modal Discharge National Framework, Ron Yolo

Yale School of Nursing Digital Theses

Sepsis, the leading cause of hospital readmissions in the United States, accounts for $23.7 billion of aggregate costs. The Medicare Hospital Readmissions Reduction Program (HRRP) requires hospitals to provide efficient patient discharge coordination to prevent readmissions or risk financial penalties. Although a substantial value capture opportunity exists to reduce avoidable readmissions, a significant gap exists in sepsis-specific discharge interventions. The purpose of this quality improvement project was to design and evaluate an evidence-based framework for reducing sepsis readmissions in the acute care setting with national impact. The novel, multidisciplinary framework developed here integrated literature on: (1) Sepsis Nurse-Navigator driven discharge …

Assessing Changes In The Epidemiology Of Cutaneous Leishmaniasis In Southwest Colombia Following The Peace Accord With Fuerzas Armadas Revolucionarias De Colombia (Farc), Matthew Stephen Benczkowski

Public Health Theses

The Colombian Conflict persisted for over 50 years, recently coming to an end in November 2016 after the signing of Colombia’s Peace Agreement between the government and FARC guerrilla group. Internal armed conflict is often entangled with the exacerbation of health outcomes, including promoting the spread of infectious diseases like cutaneous leishmaniasis (CL). CL is a vector-borne parasitic disease localized to the human tegument that decreases quality of life through lesions leading to scars and associated stigmatization. This study assessed changes in demographic and clinical characteristics of patients with CL in Southwest Colombia before and after the signing of the …

The Utility Of A Maximum Entropy Species Distribution Model For Ixodes Scapularis In Predicting The Public Health Risk Of Lyme Disease In Ontario, Canada, Holly Alexandra Burrows

Public Health Theses

Introduction: Lyme disease is an emerging public health threat in Ontario, Canada due to the range expansion of the tick vector, Ixodes scapularis. Tick abundance and density are important predictors of human Lyme disease risk and are typically measured using active tick surveillance (dragging). New cost-effective tools are needed to augment current surveillance activities. Objective: The objective of this study was to evaluate the ability of a Maximum entropy species distribution model to predict I. scapularis density and abundance in three regions of Ontario – Ottawa, Kingston, and southern Ontario – in order to determine its utility in predicting the public …

Is Now The Time For Supervised Consumption In New Haven? Assessing The Complexities Of Global Examples, National Efforts, And City Contexts To Support The Progress Of Local Harm Reduction, Mariah Frank

Public Health Theses

The overdose crisis has been an issue of public health and safety in the U.S. over the past 30 years, as a wave of increased addiction and overdoses beginning in 1990 brought it to public attention and political salience.1 The impact of the current overdose crisis is felt deeply across the state of Connecticut and locally in the City of New Haven. Statewide data show that people in Connecticut are more likely to die from a drug overdose than they are from a car accident, firearm death or homicide, Alzheimer’s, influenza/pneumonia, diabetes, kidney disease, or septicemia (data not including deaths …

Exploring Thai Physicians’ Perceptions Of And Willingness To Prescribe Pre-Exposure Prophylaxis (Prep), Brandi E. Moore

Public Health Theses

Background: Oral pre-exposure prophylaxis (PrEP) is a daily medication that can prevent HIV infection. PrEP may further support HIV prevention initiatives in Thailand, particularly as it becomes more accessible in the country’s government clinics and hospitals. While studies have explored perceptions of PrEP for at-risk people in Thailand, there has been limited investigation into Thai physicians’ opinions of and willingness to prescribe PrEP. Methods: Using convenience sampling, this cross-sectional study recruited 132 Thai physicians to complete an anonymous, online survey. The survey assessed physicians’ concerns about PrEP, experience with PrEP, and willingness to prescribe PrEP. Bivariate and multivariable logistic regressions …

Too Fast, Too Furious: Slaughterhouse Line Speeds In The Era Of Covid-19, Zoe Novic Jan 2021

Too Fast, Too Furious: Slaughterhouse Line Speeds In The Era Of Covid-19, Zoe Novic

Public Health Theses

Since 1998, the United States Department of Agriculture (USDA) has allowed meat companies to increase slaughter line speeds in order to increase production of meat and poultry. The USDA grants approvals known as “line speed waivers” that bypass the requirements of current speed and inspection regulations. During the Covid-19 pandemic, slaughterhouses became epicenters of Covid-19 outbreaks, which called into question safety regulations established at these facilities. The USDA continued to grant line speed waivers despite evidence that faster line speeds result in increased worker injuries, animal welfare violations, environmental protection violations, food safety violations, and higher risk of Covid-19.

The Bandana Project: Program Evaluation Plan, Stephanie Nickole Vazquez Jan 2021

The Bandana Project: Program Evaluation Plan, Stephanie Nickole Vazquez

Public Health Theses

Mental health concerns are highly prevalent within the college student population. While there are various campus mental health resources available for students to seek help, many choose not to due to their lack of knowledge around navigating resources and the pervasive stigma around mental illnesses. A solution to this problem is The Bandana Project (BP), an innovative mental health awareness and suicide prevention program aimed at changing social norms on college campuses. The initiative’s goals are to combat the stigma around mental illness and seeking care all while boosting the social support between students and their campus peers.

After five …

Why Not Here? An Examination Of Statewide Hospital Use Among Patients With Idu-Associated Diagnoses And Procedures In Lawrence And Lowell Ma, 2005-2019, Frank Wu

Public Health Theses

This study sought to investigate the factors that led residents with IDU-associated diagnoses and procedures to seek care outside their home cities of Lawrence and Lowell, Massachusetts. Endocarditis diagnoses were found to be no different between hospitals in and out of Lawrence and Lowell, however, echocardiograms (CI = -0.19, -0.14; p-value = <0.001), skin and soft tissue infections (CI = 1.11, 1.23; p-value = <0.001), and irrigation and drainage procedures (CI = 0.15, 0.21; p-value = <0.001) were all found to be significantly different. These changes in hospital use highlight the need to account for patient-resident mobility during the creation of future similar studies.

Preauthorizations In Orthopaedic Surgery: An Analysis Of A Single Institution’S Spine Center, Kareem Jamal Kebaish

Yale Medicine Thesis Digital Library

Introduction

Preauthorizations are frequently required by commercial insurers before patients can undergo magnetic resonance imaging (MRI) of the spine. In addition to the administrative burden, such processes can lead to delays in care. The current study assessed the rates and eventual disposition of preauthorizations required prior to spine MRIs ordered from an academic spine center.

Methods

Patients from a single academic institution’s spine center who were covered by commercial insurance and referred for a spine MRI between January 2013 and December 2019 were included in the analysis. The requirement for preauthorization and eventual disposition of each of these studies was …
