AbstractBackground and aims: Epilepsy is one of the most common neurologic disorders in childhood, but the etiology for the majority of the cases remains unknown. As a modifiable trait in pregnancy, gestational weight gain (GWG) has been shown to be a risk factor for multiple adverse birth outcomes. This study aims to investigate whether non-optimal GWG is associated with the risk of epilepsy in the offspring using a linked database in California. The Low-Income Home Energy Assistance Program (LIHEAP) does not currently meet the demands of households across the country, and by one estimate, “funding would need to increase 10 to 20 times above 2021 levels in order to cover the energy costs of all eligible low-income families” (Castillo & Daniel, 2022, para. 2). African trypanosomiasis, a neglected vector-borne parasitic disease transmitted by tsetse flies, poses a significant threat to human and animal health in Sub-Saharan Africa. As the effects of climate change alter weather patterns, these shifts in climate are predicted to impact the disease distribution and transmission dynamics through alterations in tsetse vector life history traits and infection prevalence.

Introduction: Eastern equine encephalitis virus (EEEV), a mosquito-borne alphavirus, is detected throughout the eastern United States. The primary enzootic vector, Culiseta melanura (Blacktailed mosquito), allows transmission among 50+ avian host species resulting in complex disease patterns. Spillover to dead-end host mammals, such as equines and humans, rarely occurs, however, human cases can cause severe neurological deficits and have a 30% mortality rate. Eco-epidemiology and avian host immunity patterns are not well characterized for EEEV and require ongoing surveillance to develop more targeted interventions. Urinalysis and urine cultures are important diagnostic tools for diagnosing urinary tract infections. Often urine cultures are ordered without appropriate clinical indications of infection attributable to the urinary tract. Inappropriate urine culture ordering can have significant downstream effects and contribute to the unnecessary use of antibiotics, increased prevalence of Clostridioides difficile infections and antibiotic resistance, as well as wasted resources. A quality improvement project was undertaken at the Veterans Affairs Connecticut Healthcare System (VACHS) to decrease the ordering of inappropriate urine cultures. Acute kidney injury (AKI) increases the risk of death in hospitalized adults, and early medical chart review and involvement by a nephrologist may lead to improved patient outcomes. A Kidney Action Team—comprised of a pharmacist and a board-certified nephrologist or nephrology fellow—can serve as a remote consultation service to deliver recommendations for AKI patient diagnosis, initial work up, and care. This dissertation examines the early development of Arabic grammar, up to its canonical founding text, the Kitāb of Sībawayh (d. c. 180 AH / 796 CE). It centers the competitive relationship between two social groups as key to understanding the beginnings of grammatical inquiry. These are the 'arab (conventionally, "Arabs"), and the naḥwiyyūn (conventionally, "grammarians," though generally with a connotation as well of "non-native speaker"). Sībawayh regards these groups as the two primary authorities of the linguistic data that comprise his Kitāb, and he devotes much of his energy throughout the Kitāb to adjudicating their arguments. In the first chapter, I describe the development of a modular synthetic route that allows for the de novo synthesis of novel pleuromutilin analogs, including analogs with modified cores. (+)-Pleuromutilin (11) is a diterpene fungal metabolite that displays moderate antimicrobial activity against Gram-positive pathogens. Semisynthetic modifications of 11 resulted in derivatives with improved activity and pharmacological properties. However, the majority of derivatization studies have focused on modifying the periphery of the molecule. The COVID-19 pandemic brought public attention to the implications of anti-vaccine sentiment to population health. However, the anti-vaccine movement began impacting school-aged children long before the pandemic. This study examines the role of socioeconomic factors in Connecticut kindergarten immunization rates between the 2018-2018 and 2022-2023 school years. Connecticut is the third smallest state in the United States, yet significantly diverse socioeconomic statuses exist within its small spatial boundary.

Background: Infectious diseases remain a leading cause of mortality and morbidity in Chad. We aimed to assess trends in the most recent ten years of surveillance data collected by the Ministry of Public Health in Chad to characterize the country’s current disease burden.

Methods: In this analysis of Chadian surveillance data, we analyzed reports of suspected cases across seven diseases from 2014-2023. Data included weekly aggregated case counts by district. We used the data to describe temporal and spatial trends, particularly from 2014-2019 compared to 2020-2023.

Findings: Measles case counts showed seasonal peaks of varying intensity throughout most of 2014-2023. …
