Strengths And Challenges Of A Rural Ohio Public Health Department: Considerations For Effective Service Delivery And Emergency Preparedness, Cindy L. Harris Graessle
Master of Public Health Program Student Publications
The local public health department is the main entity in cities and counties established to ensure the safety, health, and well-being of its community members. In many cases, particularly in rural areas, the health department may be the community’s closest access to health care, providing not only clinical care, but preventive services as well. These rural health departments have additional challenges that may not be experienced in the larger, more populated metropolitan areas. These challenges include lack of resources and funding that impact daily operations, expanded registered sanitarian responsibilities, and emergency preparedness difficulties with resource allocation and communication. Despite these …
The Expression Of Major Histocompatibility Class I And Major Histocompatibility Class Ii On Macrophages In The Presence Of Aryl Hydrocarbon Antagonist (Ch-223191), Caitlin Wilson
Browse all Theses and Dissertations
Macrophages are crucial for ridding the body of debris and foreign cells. The aryl hydrocarbon receptor (AhR) also plays a critical role in immunity. This study examined the effect of the AhR on the expression of major histocompatiability complex class I (MHCI) and MHC class II (MHCII) in two murine macrophage cell lines. This study used Raw264.7 and J774A.1 murine macrophage cell lines. The Raw264.7 cells are from male BALB/c mice while the J774A.1 cells are from female BALB/cN mice. The addition of the AhR anatagonist CH-223191 (AhRa) showed that the AhR does not significantly impact MHCI expression. However, MHCII …

While the neurotransmitter dopamine has been well-studied for its role in mood regulation and activation of the intrinsic reward pathway, several psychiatric disorders linked to dopamine are also known to cause memory impairment, a phenomenon which has attracted much less attention. In the current study, whole-transcriptome RNA sequencing was performed, and transcript levels of several dopamine-related genes were compared to results of behavioral assays designed to test spatial and emotional memory, as well as anxiety. The results suggest a positive relationship between expression level of Nurr1, a nuclear receptor known to initiate transcription of genes necessary for dopaminergic signaling, …

In this study an assessment (VirEMiA) on college students' knowledge and misconceptions of the Ebola virus was created and validated. VirEMiA was then used to determine what misconceptions college students have about Ebola, if there is a difference in misconceptions between students with and without a strong science background, and if Just-in-Time Teaching (JiTT) increases students' knowledge of Ebola and decreases their misconceptions. VirEMiA was shown to be a valid and reliable assessment whether confidence was integrated (seprel=0.97) or not (seprel=0.98), and for measuring misconceptions (seprel=0.97). If psychology and nursing majors were considered to have a strong background in science, …

Since brain injury is common in the United States, it is important for health professionals and the public to have accurate knowledge about traumatic brain injury (TBI). Understanding misconceptions is important for health educators, nurses, and physicians, who work with TBI patients. While previous studies on misconceptions about TBI have been undertaken, these have not focused on pre-medical students, nor utilized validated assessments. The purpose of this study was to develop and validate a one tier diagnostic test with a confidence index to determine premedical students' misconceptions about TBI. Using the theoretical framework suggested by Treagust (1986, 1988, and 1995), …

Herpes simplex virus type 1 (HSV-1) is a worldwide pathogen that affects humans and has the ability to establish a latent state of infection in the sensory nerve ganglia after primary infection of epithelial cells (Boutell and Everett, 2003). HSV-1 is a very contagious virus, which can be transmitted from person to person and cause cold sores in the infected person. Rarely, infection can lead to more serious complications, such as encephalitis. Most HSV-1 infections usually occur in childhood with lifelong potential for symptomatic or asymptomatic viral shedding episodes (Looker et al., 2015). HSV- 1 infects 60%-80% of people throughout …

ΔNp63α, the most physiologically relevant isoform of p63, a p53 family member protein, maintains the proliferative capacity of the basal layer of epithelial tissues and is highly expressed in non-melanoma skin cancer. It has been implicated in a variety of different cancers, including breast, lung, and prostate cancers, however the complex role it plays in disease remains poorly understood. Elucidating ΔNp63α’s regulatory network may provide insight into its role in epithelial tissues and in cancer progression. The objective of this thesis is to investigate ΔNp63α’s upstream regulation in the context of non-melanoma skin cancer as well as expand on ΔNp63α’s …

The immunoglobulin heavy chain gene (Igh) is regulated by numerous regulatory elements including the 3'Igh regulatory region (3'IghRR). Several transcription factors are involved in modulating the 3'IghRR including NF-B, AP-1, and the aryl hydrocarbon receptor (AhR). The AhR is a ligand-activated transcription factor that mediates the transcription of genes involved in the metabolism of environmental toxicants such as TCDD. TCDD binds AhR and regulates immunoglobulin (Ig) expression in B cells of animal models. This modulation appears to be directly mediated by binding of the AhR to dioxin response elements (DRE) within the 3'IghRR. Structural differences have been found between the …

In diseases like AIDS, Multiple Myeloma, Multiple Sclerosis (MS), Type 1 Diabetes, and Systemic Lupus Erythromatosus the detection of major histocompatibility complex type 1 molecules (MHC 1) can be a helpful component of disease diagnosis and prognosis. Most somatic mammalian cells display to varying degrees major histocompatibility complex (MHC) class 1 antigen on the cell surface. MHC1 molecules consist of a polymorphic alpha a chain and a monomorphic beta chain, beta 2 microglubulin (ß2m). The ß2M composition is fairly constant within a species whereas the alpha chain is not. The polymorphic alpha chain is encoded by an MHC complex gene …

Background: Although significant epidemiological evidence indicates that cigarette smoke exposure increases the incidence and severity of viral infection, the molecular mechanisms behind the increased susceptibility of the respiratory tract to viral pathogens are unclear. Adenoviruses are non-enveloped DNA viruses and important causative agents of acute respiratory disease. The Coxsackievirus and adenovirus receptor (CAR) is the primary receptor for many adenoviruses. We hypothesized that cigarette smoke exposure increases epithelial susceptibility to adenovirus infection by increasing the abundance of apical CAR. Methodology and Findings: Cultured human airway epithelial cells (CaLu-3) were used as a model to investigate the effect of sidestream cigarette …

Scabies, caused by the mite S. scabiei that burrows in the skin of humans, is a contagious skin disease that affects millions of people worldwide. It is a significant public health burden in economically disadvantaged populations, and outbreaks are common in nursing homes, daycare facilities, schools and workplaces in developed countries. It causes significant morbidity, and in chronic cases, associated bacterial infections can lead to renal and cardiac diseases. Scabies is very difficult to diagnose by the usual skin scrape test, and a presumptive diagnosis is often made based on clinical signs such as rash and itch that can …

Introduction
The two most significant impediments to renal allograft survival are rejection and the direct nephrotoxicity of the immunosuppressant drugs required to prevent it. Calcineurin inhibitors (CNI), a mainstay of most immunosuppression regimens, are particularly nephrotoxic. Until less toxic antirejection agents become available, the only option is to optimize our use of those at hand.
Aim
To determine whether intensive rabbit anti-thymocyte globulin (rATG) induction followed by CNI withdrawal would individually or combined improve graft function and reduce graft chronic histopathology–surrogates for graft and, therefore, patient survival. As previously reported, a single large rATG dose over 24 hours was well-tolerated …

Introduction
The pharmacokinetics of vancomycin are highly variable among neonates, which makes dosing challenging in this population. However, adequate drug exposure is critical, especially when treating methicillin-resistant Staphylococcus aureus (MRSA) infections. Utilization of population pharmacokinetic models and Bayesian methods offers the potential for developing individualized therapeutic approaches. To meet this need, a neonatal vancomycin population pharmacokinetic model was recently published. The current study sought to externally evaluate the predictive performance and generalizability of this model.
Methods
A retrospective chart review of neonates who received vancomycin and had ≥1 peak and ≥1 trough concentrations at five Intermountain Healthcare neonatal intensive care …
