Executive Decision-Making: Piloting Project Echo® To Integrate Care In Queensland, Perrin Moss, Nicole Hartley, Jenny Ziviani, Dana Newcomb, Trevor Russell

Project ECHO Bibliography

Introduction: A Queensland project team secured grant funding to pilot Project ECHO®, a telementoring model, to drive vertical and horizontal integration across paediatric, education and primary care services. This study sought to understand what influenced healthcare executives’ decision-making processes to organisationally commit to and financially invest in the pilot proposal within an organisational context.

Theory and Methods: A phenomenological approach methodology was adopted to investigate healthcare executives’ conscious decision-making processes. Semi-structured interviews with key stakeholders were conducted alongside project documentation analyses to create a thematic framework.

Results: The qualitative thematic analysis identified five key themes that influenced the decision-making processes …

Indications for Continuation of Alpha-1 Agonist Midodrine Upon ICU Discharge

Hussain, Fahad, MSII; Rao, Deepti, MD

Introduction:

Persistent hypotension in patients affected by circulatory shock is a common barrier to discharge in the ICU. Even after these patients have been otherwise stabilized, low-level hypotension often ensures that they remain in the ICU on IV vasopressors. Midodrine, an oral α1-agonist, has shown promise as an adjunct therapy to wean patients off IV vasopressors and facilitate earlier discharge from the ICU. According to a recent study done at the Mayo Clinic ICU, Rochester, discharge on Midodrine was associated with a shorter ICU …

BACKGROUND: The recently updated European LeukemiaNet risk stratification guidelines combine cytogenetic abnormalities and genetic mutations to provide the means to triage patients with acute myeloid leukemia for optimal therapies. Despite the identification of many prognostic factors, relatively few have made their way into clinical practice.

METHODS: In order to assess and improve the performance of the European LeukemiaNet guidelines, we developed novel prognostic models using the biomarkers from the guidelines, age, performance status and select transcript biomarkers. The models were developed separately for mononuclear cells and viable leukemic blasts from previously untreated acute myeloid leukemia patients (discovery cohort,

Recommendations For Clinical Laboratory Testing For Antithrombin Deficiency; Communication From The Ssc Of The Isth, Elizabeth M. Van Cott, Christelle Orlando, Gary W. Moore, Peter C. Cooper, Piet Meijer, Richard Marlar, Subcommittee On Plasma Coagulation Inhibitors

Pathology Research and Scholarship

Hereditary deficiency of antithrombin, a natural anticoagulant, causes a thrombophilia with a high risk for venous thromboembolism. Guidance for laboratory testing to diagnose antithrombin deficiency include the use of an activity assay for initial testing, performing an antigen test and activity-to-antigen ratio when the activity level is low, using pediatric reference ranges until the age of 6 months, excluding acquired causes of low antithrombin (e.g. liver dysfunction, proteinuria, heparin, disseminated intravascular coagulation, thrombosis, surgery) or falsely normal/elevated results (e.g. argatroban, bivalirudin, dabigatran in factor IIa-based assays; rivaroxaban, apixaban, edoxaban, but not betrixaban in Xa-based assays). Molecular testing, if available, may …

Diagnostic Performance Of Two Molecular Assays For The Detection Of Vaginitis In Symptomatic Women, Alexandra Thompson, Karen Timm, Noelle Borders, Liz Montoya, Karissa Culbreath

Pathology Research and Scholarship

The three main causes of vaginitis are bacterial vaginosis (BV), vulvovaginal candidiasis (VVC), and trichomoniasis (TV). Two multiplex assays are commercially available for detection of DNA from organisms associated with vaginitis: BD Affirm™ VPIII Microbial Identification Test (Affirm) and BD MAX™ Vaginal Panel (MAX VP). Here, the performance of MAX VP was compared to that of Affirm, which was considered the standard of care. Four vaginal swabs were collected from each subject with the following: BD Affirm™ VPIII Ambient Temperature Transport System (ATTS), BD MAX™ UVE Specimen Collection Kit, Hologic Aptima® Vaginal Swab Specimen Collection Kit, and BD ESwab™ collection …

Guidance On The Establishment, Implementation And Performance For Proficiency Testing Programs For Thrombosis And Hemostasis, Richard A. Marlar, Piet Meijer, Ian Jennings, John Olson

Pathology Research and Scholarship

Thrombosis and hemostasis testing is one of the most difficult areas in the clinical laboratory for
obtaining accurate results. With these shortcomings, comparing patient results is sometimes very difficult.
It is extremely important that a good External Quality Assessment Scheme (EQAS) is available to assess the accuracy of the assays and analytes of the coagulation laboratory. The objective of this document is to outline the major requirements of an EQAS for thrombosis and hemostasis testing. This document will discuss the
establishment of an EQAS, the major components necessary for the program, the basic methods to evaluate data and the reporting …

Pathology Research and Scholarship

BACKGROUND: Patients 65 years old and older largely represent (>50%) hospital-admitted patients with acute coronary syndrome (ACS). Data are conflicting comparing efficacy of early routine invasive (within 48-72 hours of initial evaluation) versus conservative management of ACS in this population.

OBJECTIVE: We aimed to determine the effectiveness of routine early invasive strategy compared to conservative treatment in reducing major adverse cardiovascular events in patients 65 years old and older with non-ST elevation (NSTE) ACS.

DATA SOURCES: We conducted a systematic review of randomized controlled trials (RCTs) through PubMed, Cochrane, and Google Scholar database.

STUDY SELECTION: The studies included were …

A small subepithelial esophageal lesion was seen 21 cm from the incisors in an otherwise unremarkable examination. Subsequent upper-EUS examination revealed a 14 mm by 21 mm well-circumscribed, hypoechoic lesion originating from the muscularis propria (layer 4). The initial EUS impression was suggestive of a GI stromal tumor, however, examination of EUS-guided fine-needle biopsy specimens showed a haphazard arrangement of spindle cells in a fibrotic stoma.

Hemostasis And Thrombosis; Pathology: A Modern Case Study, Marian Rollins-Raval Jan 2020

Hemostasis And Thrombosis; Pathology: A Modern Case Study, Marian Rollins-Raval

Pathology Research and Scholarship

Following injury or invasion by a pathogen, the body protects against the loss of blood and attempts to maintain hemostasis by activating the primary and secondary hemostatic pathways.* The culmination of these events is the formation of a fibrin-rich platelet plug at the site of vessel damage. Along with its role in hemostasis, fibrin also plays roles in the inflammatory and wound healing processes that are occurring simultaneously following an injury (see Chapter 2). Fibrin deposition is seen frequently in areas of inflammation, whether vessel injury is present or not. Local fibrin deposition is able to induce adhesion …

Heparin-Induced Thrombocytopenia Associated With Collection Of Hematopoietic Progenitor Cells By Apheresis, Jay S. Raval, Yara A. Park, Irina Perjar, Marshall A. Mazepa, Benjamin G. Vincent, Alice D. Ma, Marian A. Rollins-Raval Jan 2020

Heparin-Induced Thrombocytopenia Associated With Collection Of Hematopoietic Progenitor Cells By Apheresis, Jay S. Raval, Yara A. Park, Irina Perjar, Marshall A. Mazepa, Benjamin G. Vincent, Alice D. Ma, Marian A. Rollins-Raval

Pathology Research and Scholarship

Heparin-induced thrombocytopenia (HIT) can occur following exposure to heparin and is characterized by thrombocytopenia with increased risk for thrombosis. This condition is mediated by formation of immunoglobulin G antibodies against platelet factor 4/heparin complexes that can subsequently lead to platelet activation. Herein, we detail the clinical and laboratory findings, treatments, and outcomes of two patients who developed HIT and thrombosis after undergoing collection of hematopoietic progenitor cells by apheresis (HPC-A) for autologous HPC transplant. Given that heparin may be used during HPC-A collections, these cases emphasize the importance of prompt consideration of HIT in patients that develop thrombocytopenia and thrombosis …

Tetraspanin Scaffold Proteins Function As Key Regulators Of Hematopoietic Stem Cells, Victoria D. Balise, Chelsea A. Saito-Reis, Jennifer M. Gillette Jan 2020

Tetraspanin Scaffold Proteins Function As Key Regulators Of Hematopoietic Stem Cells, Victoria D. Balise, Chelsea A. Saito-Reis, Jennifer M. Gillette

Pathology Research and Scholarship

Hematopoietic stem and progenitor cells (HSPCs) are responsible for the development, maintenance, and regeneration of all the blood forming cells in the body, and as such, are critical for a number of patient therapies. For successful HSPC transplantation, stem cells must traffic through the blood and home to the bone marrow (BM) microenvironment or "niche," which is composed of soluble factors, matrix proteins, and supportive cells. HSPC adhesion to, and signaling with, cellular and extracellular components of the niche provide instructional cues to balance stem cell self-renewal and differentiation. In this review, we will explore the regulation of these stem …

Hierarchies Of Elitism And Gender: The Bluebook And The Alwd Guide, Steven K. Homer

Faculty Scholarship

Hierarchies persist in legal academia. Some of these, while in plain view, are not so obvious because they manifest in seemingly small, mundane choices. Synecdoche is a rhetorical device used to show how one detail in a story tells the story of the whole. This Article examines hierarchies of elitism and gender through a lens of synecdoche. The focus is on the choice of citation guide. Even something as seemingly benign and neutral as choosing a citation guide can reveal hierarchies of elitism and gender bias in legal education and the legal profession. Put another way, the choice of citation …
