Objectives To investigate whether COVID-19 patients with pulmonary embolism had higher mortality and assess the utility of d-dimer in predicting acute pulmonary embolism.
Patients and methods Using the National Collaborative COVID-19 retrospective cohort, a cohort of hospitalized COVID-19 patients was studied to compare 90-day mortality and intubation outcomes in patients with and without pulmonary embolism in a multivariable cox regression analysis. The secondary measured outcomes in 1:4 propensity score-matched analysis included length of stay, chest pain incidence, heart rate, history of pulmonary embolism or DVT, and admission laboratory parameters.
Results Among 31,500 hospitalized COVID-19 patients, 1117 (3.5%) patients were diagnosed …

Introduction: 25-60% of septic patients experience relative adrenal insufficiency (RAI) and glucocorticoid (GC) is frequently used in septic patients. However, the efficacy of GC therapy and whether GC therapy should be based on the status of RAI are highly controversial. Our poor understanding about the pathogenesis of RAI and a lack of RAI animal model present significant barriers to address these critical issues.
Methods: Scavenger receptor BI (SR-BI) regulates stress-induced GC (iGC) production in response to stress. We generated SF1CreSR-BIfl/fl mice and utilized the mice as a RAI model to elucidate the pathogenesis of RAI and GC therapy in sepsis. …

Greenhouse gas (GHG) emissions have been implicated in global warming and extreme climate change conditions. The Environmental Protection Agency (EPA) has identified agriculture, more specifically cattle production, as a major contributor to global GHG production. When expressed as liters of gas, CO2 is considered the primary GHG produced, however, in terms of global warming potential (GWP), CH4 has 25-times greater potential than that of CO2. Due to the elevated GWP of CH4, we have constructed a series of experiments to investigate the use of a halogenated CH4 analogue derived from kelp (i.e., bromoform) as an inhibitor of enteric CH4 production …

In Interface, five Kentucky-area artists explore a new language of representation with the aid of digital tools like Artificial Intelligence (AI) and Automatic Facial Recognition Software (AFR). Some artists use algorithms to alter celebrity faces beyond recognition, others feed data sets of existing art to AI models in an attempt to generate portraits of no one in particular. Historically, research has identified Black relationships as deviant, unstable, and pathological as compared to White relationships (Frazier, 1966; Moynihan, 1965). This outlook centers on White experiences as a baseline for comparison and dismisses the cultural nuance within Black relationships. Examining intraracial Black relationship formation dynamics – specifically romantic partner preferences – using an intersectional framework seems more relevant as Black people are least likely of all races to date White people (Batson et al., 2006; Yancey, 2009). As Black immigrants continue to increase their presence in the U.S. (Anderson & López, 2018), the current research examines how cultural factors influence …

Autism is one of the fastest growing educational disability groups (U.S. Department of Education, Office of Special Education Programs, 2021). The Individuals with Disabilities Education Improvement Act (IDEIA, 2004) ensures all students have access to a free and appropriate public education. Individualized Education Programs (IEP) have been described as central to IDEIA (2004) and are the most common service received by children with autism (Berrcera et al., 2017; Yell et al., 2003). Microglia, which are productively infected by HIV-1, are critical for brain development and maturation, as well as synaptic plasticity. The pathophysiology of HIV-infected microglia and their role in the pathogenesis of HIV-1-associated neurocognitive and affective alterations, however, remains understudied. Three complementary aims were undertaken to critically address this knowledge gap. First, the expression of HIV-1 mRNA in the dorsolateral prefrontal cortex of postmortem HIV-1 seropositive individuals with HAND was investigated. Utilization of a combined RNAscope multiplex fluorescent and immunostaining assay revealed prominent HIV-1 mRNA in microglia of postmortem HIV-1 seropositive individuals with HAND. By adopting systems-based thinking, we can identify areas of the healthcare system to change to eliminate these disparities. This paper examines several popular approaches in the United States for enhancing early childhood literacy and language development in two special education populations— d/Deaf or hard-of-hearing (d/DHH) students and students with autism spectrum disorder (ASD). In addition to examining the effectiveness of these approaches, I adopt an educational equity lens to describe how variability in implementation can impact outcomes for these children. Context: Environmental exposures are associated with approximately 19% of disease globally, and exposure to neurotoxic chemicals is estimated to cost the United States $50 billion per year. Despite calls from the Institute of Medicine to increase training for clinicians regarding environmental health since the 1990s, there is still little instruction in environmental health for clinicians. This leaves gaps in knowledge that need to be bridged through outreach and education to practicing clinicians. Academic detailing (AD) is an educational intervention associated with improved prescribing practices in healthcare professionals but has not been applied to preventive or environmental health. The study is about virtual training and coaching to Indonesian parents of preschool-age children with social-communication delays to implement naturalistic intervention. These intercontinental coaching sessions were delivered asynchronously because of the time difference between the coach and the participants and internet bandwidth barrier in some rural areas in Indonesia. Multiple-baseline design across behaviors was utilized to determine the effect of asynchronous training and coaching program to teach three Indonesian parents three naturalistic strategies (i.e., modeling, mand-model, and time delay) during natural routines. Parent-child dyads video-recorded their interaction before and after intervention.

As K-12 teachers worked to meet the educational, social, emotional and health needs of students and families, their personal well-being was taxed. It is critical to understand the experiences of K-12 teachers during the COVID-19 pandemic and to leverage this understanding to develop effective strategies for supporting K-12 teachers in the future. Blood-brain barrier disruption has been identified to associate with the pathogenesis several neurological diseases such as dementia [1, 2], multiple sclerosis[3, 4], acute or chronic cerebral ischemia[5], brain trauma[5], meningitis[5], encephalitis[5], stroke[6], and seizures[7]. Being able to effectively identify blood-brain barrier disruption is limited in methodology. The current standard is using a cerebrospinal fluid (CSF) albumin to serum albumin index, which requires the use of a lumbar puncture. In the present study, a concurrent operant analysis (COA) was conducted for one elementary aged student in a publicly funded school to determine if the results yielded an interpretable outcome to identify a potential reinforcer to increase work completion. A simultaneous treatments design was used during the COA to assess choice making behavior. The results of the COA indicated the student’s choice making behavior was maintained by access to attention. Laurel wilt disease (LWD) is a lethal vascular disease impacting lauraceous hosts caused by Harringtonia lauricola, the fungal symbiont of the redbay ambrosia beetle (Xyleborus glabratus Eichoff) (RAB) (Coleoptera: Curculionidae). LWD has caused tree mortality throughout the southeastern United States and is continuing to spread into new regions. Current management methods have not been successful in preventing spread, warranting investigation into innovative techniques including RNA interference (RNAi).

An organism’s ability to respond to changing conditions can be vital to its success. Indeed, plasticity is a common feature of living organisms. Much of the research in this area, though, has focused on effects caused by environmental conditions. What has received relatively less attention is how social experiences and broader features of an organism’s social environment can lead to long-lasting changes in health and behavior. In the larval stage, insects experience the fastest growth with voracious feeding and multiple cuticle shedding resulting in hundreds-of-fold increasing of size. During metamorphosis, insects stop increasing body size but go through dramatic tissue transformation, developing wings and other adult features. Underground mining operations utilize the room-and-pillar mining method that extracts the orebody and creates ‘rooms’ and leaves the remaining in-situ material as ‘pillars’ to support the overburden. Room-and-pillar mines are traditionally designed in square or rectangular-shaped pillars. This work presents laboratory experiments testing the compressive strengths of specimens modeled after square and parallelogram-shaped pillars and numerical models of square and -parallelogram-shaped pillar systems. The uniaxial compressive strength of the specimens’ shape and the observations of the extent of the specimen core post-failure were analyzed. Background and objective: Whole body manipulation of the renin-angiotensin system (RAS) consistently exerts profound effects on experimental atherosclerosis development. A deficit in the literature has been a lack of attention to the effects of sex. Also, based on data with gene-deleted mice, the site of RAS activity that influences lesion formation is at an unknown distant location. Since angiotensin (AngII) concentrations are high in kidney and the major components of the RAS are present in renal proximal tubule cells (PTCs), this study evaluated the role of the RAS in PTCs in atherosclerosis development.

Kentucky has the highest incidence and mortality rate of all site cancers, and Kentuckians residing in the Appalachian region often have worse outcomes, where cancer is a leading cause of death. Focusing on colorectal cancer (CRC) specifically, Kentucky ranks first nationwide for incidence (50 cases per 100,000 people) and fifth for mortality (about 17 deaths per 100,000 people). The Kentucky Colon Cancer Screening Program increased screening rates and reduced mortality since its launch. Yet, CRC remains a leading cause of death for Kentuckians. The field of local news is often associated with news deserts, commonly defined as geo‐based communities without news‐ papers or other legacy media as providers of locally oriented news and civic information. This phenomenon is expanding in global society due to the diminishing presence of newspapers at moments of accelerated digitization. This study examines the multiplex nature of news deserts in rural and suburban areas in China. Introduction Health and social service organizations, including the emergency department (ED) and public assistance programs, constitute a social safety net that may serve as an “access point” for substance use treatment utilization. Racialization of substance use disorder (SUD) and gender disparities in access to treatment contribute to differences in health and social service utilization, including substance use treatment for Black women. We therefore explored the role of various access points in facilitating the use of substance use treatment among Black women with substance use and involvement in the criminal justice system.
