An Evaluation Of The Effectiveness Of A Post Discharge Telephone Program To Decrease Hospital Readmissions For Patients With Heart Failure, Amy L. Donaldson

The goal of this quality improvement pilot project was to evaluate the effectiveness of a post discharge telephone program to decrease 30-day hospital readmissions for patients with heart failure at one acute care hospital in Ohio. The pilot project evaluated data collected on medication reconciliation, confirmed follow up appointment, a patient medication regime, and a patient's understanding of discharge instructions through the intervention of a post discharge phone call. Thirteen patients participated; one patient had a 30-day readmission to the hospital. The pilot demonstrated an impact to reducing readmissions in the high-risk population and identified opportunity to improve the care …

Health literacy and cultural competence in healthcare are now a priority focus of major health organizations. Healthcare professionals realize that patient-centered care is dependent upon clear communication and understanding. Due to the limitations in teaching time available and the amount of information that needs to be covered, student nurses receive minimal education on health literacy and communication methods with limited English Proficient (LEP) patients. This project focused on the development of an educational program for second year Associate Degree Nursing (ADN) students in order to increase cultural competency and health literacy prior to entering the clinical area.

The purpose of this project was to examine outcomes in African American adult patients with type 2 diabetes who received education and diet and exercise services compared to those who did not. Type 2 diabetes accounts for 90-95% of all diabetes cases, and is more prevalent among African Americans. Diabetes is the fifth leading cause of death in the African American community.

Objective: This practice improvement project was designed to test the efficacy and feasibility of implementing Shared Medical Appointments (SMA) as an innovative approach to improve the care of diabetic patients while containing costs in a rural private practice associated with an Accountable Care Organization (ACO).

Methods: Five Medicare quality measures specific to diabetes were used as patient outcomes: hemoglobin A1C control.

Results: Fifteen patients participated in one or more monthly SMAs with groups ranging from two to five participants per appointment over 10 months. Participants indicated a high level of satisfaction with themes associated with social support. No significant difference …

High Fidelity Simulation is a teaching strategy that is becoming well-entrenched in the world of nursing education and is rapidly expanding due to the challenges and demands of the health care environment. The problem addressed in this study is the conflicting research results regarding the effectiveness of HFS for students' knowledge acquisition after participating in simulation exercises. Specifically this researcher determined the effects of a formatted simulation scenario on knowledge acquisition among nursing students and the students' satisfaction and self-confidence with the simulation learning activity. The annual cost of evaluation and treatment for uncomplicated UTIs on the health care system is estimated to be approximately $2.14 billion (Barber, et al., 2013). The improper treatment of patients with uncomplicated UTIs will result in increased healthcare expenditures, increase exposure of patients to potential side effects and an increase in antibiotic resistance. Purpose: The purpose of this project was to develop and implement study sessions to reduce attrition and increase first semester nursing student success.

Background: Each semester, 80-100 students begin the nursing program at Lewis University. The attrition rate for first semester nursing students at a traditional Bachelor of Science in Nursing (BSN) program has been running between 20-40% over the past few years.

Nursing students experience stress from many different facets. Stress has a negative impact on academic performance and cognition. Research shows the positive effects mirthful laughter has on psychological and physiological health. This pilot project evaluated the effectiveness Laughter Yoga had on stress and coping. Cohen's Perceived Stress Scale-10 and a stress and coping questionnaire measured project outcomes. The overall results give anecdotal evidence for continuing projects to evaluate student stress and coping strategies.
