In many healthcare organizations the needed support for second victims is lacking or unavailable. This paper describes the evaluation of debriefing support program for nurses who are second victims. Unanticipated and tragic patient outcomes will happen to excellent healthcare providers, the results being anxiety, depression, guilt, and fear. The effects of being a second victim can result in post-traumatic stress and compassion fatigue, with providers ultimately leaving the profession. After obtaining IRB approval, a volunteer debriefing support program was implemented. Hospital leaders have been directed to focus on assuring patient safety and improving outcomes through the development of a strong safety culture. The Safety Attitude Questionnaire (SAQ) at a small rural hospital showed a decline, over three years, in the perceived safety climate of the organization, by clinical staff. The purpose of this quality improvement project was the implementation of a structured debriefing program, promoting teamwork and collaboration between leadership and front-line staff, to improve the safety culture of the organization, exhibited by an improvement in SAQ scores within one year. Nurses who graduate from the associate degree program at Valencia College lack the skills needed for the current complex health care environment. These new nurses are unable to translate the knowledge learned in the didactic environment to the clinical experience. The faculty of Valencia College felt that using a student-run clinic would be an effective means of addressing the academic drift seen in the new nurse. Two city hospitals launched Dedicated Education Units (DEUs) in collaboration with a Midwest state university. The culture of excellence created by the academic practice partnership produced an improvement in patient satisfaction scores at both hospitals. Hospital Consumer Assessment of Healthcare Providers and Systems (HCAHPS) survey scores improved dramatically within one month of DEU launch. This manuscript presents the pre- and post- DEU HCAHPS scores and relates surprising trending with student presence on the units.

The incidence of multidrug-resistant tuberculosis (MDR-TB) is on the rise globally. MDR-TB takes a minimum of 2 years to treat and the treatment regimen produces many adverse drug reactions (ADRs). The World Health Organization (WHO) has called for further research on the treatment of community- based MDR-TB patients as care is being decentralized to outpatient settings. In the WHO's TB progress report for 2015, they note there is a dearth of literature about anti- TB drug-induced mortality, morbidity and loss in quality of life, particularly in low- resource settings.

Guidelines are established to address specific needs of a clinical problem and assist clinicians with decision-making. The complexity of Type 2 diabetes warrants the use of such guidelines to help provide evidence-based treatment. The American Diabetes Association (ADA) foot care guidelines recommend providing education to all patients despite their risk, in order to increase awareness of lower extremity complications and prevent them from occurring. In this project, a diabetic lower extremity educational intervention based on ADA guidelines was provided to 16 Type 2 diabetics over four months. Purpose: To examine the ability of patient-selected music to reduce pain perception, decrease post-anesthesia care unit (PACU) time, and improve patient satisfaction relative to usual ambient noise.

Design: Patients presenting for emergent surgery at a Midwestern community hospital (n=38) received music in one of five genres they selected via headphones within 5 min of PACU arrival. Control patients (n=39) had surgery as usual during the same 12 month period.

Methods: Outcomes included PACU time, medication use, pain scores, and patient satisfaction via standardized questionnaire.

Children often experience the uncomfortable effects of invasive procedures as a part of primary health supervision and during times of illness. Inadequate procedural comfort management can lead to numerous lasting harmful effects including distrust of healthcare providers, future intensified pain responses, negative cognitive and emotional experiences, and psychosocial health problems (Czarnecki et al. 2011). The purpose of this evidence based practice project is to improve usability of the Electronic Medical Record (EMR) to demonstrate consistent compliance with venous thromboembolism (VTE) and heart failure (HF) Core Measures. Consistently meeting core measures is important for optimal patient care and reimbursement..

Strength of Evidence: Systematic reviews, a multinational cross-sectional study, and a multicenter, randomized controlled trial all demonstrate a positive impact on suboptimal prophylaxis and treatment for VTE and HF quality measures, with improved EMR usability.

Health literacy is fundamental to promotion, prevention, and maintenance of one's health. It is pertinent to one's ability to obtain, understand, and use information in order to make informed healthcare decisions. The provision of patient-centered nursing care is required by Centers for Medicare and Medicaid Services and accreditation agencies. An acute care hospital in the Midwest experienced low patient satisfaction scores specific to communication with the nursing staff.
