Anti-melanoma differentiation-associated gene 5 antibody (anti-MDA-5 Ab) is associated with amyopathic dermatomyositis (DM). These patients are particularly at high-risk for developing acute and rapidly progressive interstitial lung disease (ILD). Given the lack of muscle-related symptoms, along with its sudden onset and rapid clinical progression, the diagnosis of anti-MDA-5 Ab + ILD represents a challenge for clinicians. Even after the diagnosis is established, prognosis remains dismal owing to a hyperinflammatory state, mimicking cytokine storm, commonly refractory to potent immunosuppressive therapy. Hence, we present an elderly African American man who developed acute and rapidly progressive ILD in the setting of positive anti-MDA5 …

Outdoor recreational use is growing rapidly in the western United States, which increases the pressure on multiple-use public lands. Balancing recreational use with conservation goals requires considering the spatial and temporal intensity of recreational use and examining its effects on multiple species within an ecosystem. In 2019–2021, we assessed the relationship between recreational intensity and the abundance of a dominant prey species (Piute ground squirrel [Urocitellus mollis]), the abundance of avian and mammalian facultative scavengers that rely on ground squirrels, and the abundance and nesting success of ground-nesting birds at a national conservation area in southwestern Idaho, USA, where recreational …

Patellofemoral joint (PFJ) pain is an incessant musculoskeletal disorder underreported in older adults. This study examined the effect of age and surface on the magnitude and waveform of PFJ stress during a stair ascent task. 12 healthy young adults and 12 healthy older adults completed a stair ascent and descent task on a normal, uneven, and slick surface. During stair ascent age impacted PFJ stress waveform (p < 0.001), but not magnitude, as older adults exhibited greater PFJ stress from 55 to 59% and 74 to 84% of stance (p < 0.037). The increase in older adults PFJ stress late in stance may be attributed to a later peak PFJ stress (p = 0.002) and larger PFJ stress-time integral (p = 0.003) than young adults. Surface also impacted PFJ waveform (p < 0.001). On the uneven surface during stair ascent, PFJ stress was smaller from 9 to 24%, but greater from 75 to 88% of stance (both: p < 0.001), and PFJ stress-time integral (p = 0.010) was greater than normal surface. A significant age by surface interaction was observed for time of peak PFJ stress (p = 0.041). On the uneven surface, older adults exhibited a later peak PFJ stress compared to young adults (p = 0.008), and later peak PFJ stress compared to normal and slick surface (both: p = 0.014). For stair descent, age impacted PFJ contact area range (p = 0.034) and peak knee flexion angle (= 0.022), but no other PFJ stress measure. On the uneven surface during stair descent, partipicants exhibited greater PFJ stress-time integral, reaction force impulse, and mean contact area (all: p < 0.05) compared to the slick surface. Surface also impacted PFJ stress waveform during stair descent, where participants exhibited greater stress from 0 to 16% of stance but smaller stress from 98 to 100% of stance on the uneven compared to slick surfaces. Older adults may be more likely to develop PFJ pain, as they exhibited larger and later PFJ stress, particularly when navigating an uneven surface. When navigating the uneven surface, all participants exhibited knee biomechanics, including larger PFJ stress in terminal stance and greater PFJ stress-time integral that may increase overall joint stress and increases risk of developing PFJ pain.

Introduction
Endoscopic ultrasound (EUS)-guided lumen-apposing metal stents (LAMS) represent a novel tool in therapeutic endoscopy. However, the presence of LAMS may dissuade surgeons from operations with curative-intent. We report three clinical scenarios with deployment of LAMS in patients that subsequently underwent pancreaticoduodenectomy (PD).
Methods
Six patients identified from our IRB-approved pancreas cancer database had EUS-LAMS placement prior to PD. Patient, tumor, treatment-related variables, and outcomes are herein reported.
Results
Two patients underwent a LAMS gastrojejunostomy (GJ) for duodenal obstruction. Another patient underwent LAMS choledochoduodenostomy (CDS) for malignant biliary obstruction. In three patients, a LAMS gastrogastrostomy or jejunogastrostomy was deployed post …

Reducing anastomotic leak rates after bowel resection is a priority among patients undergoing gynecologic oncology surgery. While near-infrared (NIR) angiography has been investigated in the colorectal literature, more recent work has demonstrated promising results when used in gynecologic cancer surgery. It has been repeatedly shown to be a safe intervention that can offer real time assessment of bowel perfusion, offering the surgeon the opportunity to act on the results in the hopes of decreasing the risk of complications.

Artificial intelligence (AI) poses a serious challenge for data protection and ethics. Whereas ethics is based on the moral and religious understanding of good and evil, adherence to religious norms is vital for many nations. In Islam, privacy is a fundamental value that is deeply rooted in the principles of Shariah law and focuses on the dignity of the individual, personal boundaries and moral behaviour. Therefore, the use of AI technologies by Islamic nations raises concerns about the compatibility of these traditional privacy norms with ethical frameworks for AI. To ensure that the field of AI is not only innovative …

New technologies such as virtualization and Software Defined Networking (SDN) have given traditional networks unprecedented flexibility. A Service Function Chain (SFC) formed by concatenating multiple Virtual Network Functions (VNFs) expands network functions to meet the growing personalized network requirements of applications. However, due to the decentralization of VNF instances and the tight cloud load, how to ensure Quality of Service (Qos) while deploying the SFC brings new challenges. Hence, a SFC deployment strategy based on multi-objective optimization, named MO-SACO is proposed, which regards delay optimization, reliability assurance and cost reduction as the main objectives. Specifically, we first design a VNF …

Behavioural finance invalidates the rationalistic assumptions of the efficient market hypothesis by proposing a realistic explanation for overreaction and underreaction. These phenomena are caused by investors making financial decisions based on their emotions without realizing them. This study attempts to establish the effect of investors' emotional intelligence on behavioural biases, namely, herding, overconfidence bias, and disposition effects, and its consequences for the churning frequency of mutual fund portfolios. This quantitative cross-sectional study was undertaken to collect data from 499 mutual fund investors using a self-administered questionnaire. We found that the disposition effect has a negative impact and that overconfidence bias …

Sodium iodide dopant influencing the properties of polysaccharide polymer sodium carboxymethyl cellulose is analyzed in this work by FTIR, XRD, DSC, TGA, SEM, UTM, and electrical. Solvation of I− and Na+ ions by the −OH group has disrupted the intra– and intermolecular hydrogen bonds between the polymers escorting to a decrease in crystallinity and creating a pathway for ion diffusion. The formation of transient crosslink and the physical environment created by the polymer-ion interaction have affected the rigidity of the polymer chains as well as the thermal stability of the NaCMC−NaI system. I− ion influences the ESW …
