Pay-For-Performance In Five State Medicaid Programs: Lessons For The Nursing Home Sector, Edward M. Miller, Julia Doherty

This project looks at the pay-for-performance program in five state Medicaid programs and lists the lessons that the Nursing Home Sector can learn from. They are: The federal government has traditionally sought to ensure quality outcomes through nursing home (NH) surveys conducted by state officials; Some states have begun to experiment with pay-for-performance (P4P) incentives, which provider higher Medicaid reimbursement to those facilities achieving desired outcomes; By 2007, there were 9 state P4P programs covering 20% of NHs and 16.7% of residents; Little is known about the use of P4P to promote quality and efficiency in the NH sector.

The St. Mary’s Women @ Work program partners with the Department of IT at UMass Boston to provide young women with a path to self sufficiency. It does so by placing these youth for a 3 month period, in a real-work environment where they get paid and receive hands-on practical training.

The Gastón Institute responds to requests for periodic demographic updates on the Latino population in Massachusetts and other New England states. The rapidly changing makeup of the Latino population of Massachusetts calls for a deep understanding of the characteristics and situations of the different sectors of the community. While the non-Latino white population from Massachusetts declined around 4% over the last decade, the Latino population grew approximately 45%. This fact highlights the importance of Latino population to the State’s economic, political and social growth and stability.

Camp Shriver at UMass Boston provides summer recreational programming for children in the surrounding communities of Boston, Brockton and Quincy. Through Camp Shriver, UMass Boston reaches out to children and families who have few opportunities, particularly families of children with disabilities.

The Harbor Point Outreach Partnership is a community-university tutoring and afterschool youth enrichment program based on the work of students in service-learning classes, volunteers and work study students from UMass Boston and the Community. We are part of the UMASS Boston-Harbor Point Apartment Community Memorandum of Agreement. Our primary partner is the Walter Denney Youth Center and most activities take place at this location. We also partner with the Dever-McCormack School.

A large population of underserved students and residents in the City of Boston suffer from behavioral and/or discipline problems, as well as substance and/or alcohol abuse. A side effect of this behavior is the increasing instance of trash such as cigarette butts, condoms, syringes and packaging being discarded in public areas. The South Boston Youth Action Center (SBYAC) is tackling this problem by making the community aware of the locations and probable sources of this problem. The Gastón Institute collaborates with government agencies, community organizations and foundations in applied research aimed at evaluating the impact of policies, programs and practices. This poster describes a three-way collaboration among researchers and practitioners from Boston Public Schools (BPS), the Gastón Institute and the Center for Collaborative Education (CCE) which yielded two comprehensive studies: 1. The enrollment and outcomes of English language learners in BPS; 2. Best ELL practices in four schools that were consistently high performing or steadily improving with respect to ELL student outcomes.

This program involves creating five-year longitudinal community-based program evaluation reports annually based on outcomes for early college program for academically average students in grades 10-12, in a suburban high school partnership with MA community college. The Mixed methodology employed uses surveys and interviews and quantitative student data to illuminate program design, measure student outcomes, investigate changes in teaching practice and examine effective leadership practices. The two-year results show impressive credit accumulation and improved college readiness skills in students. It also shows that strong support mechanisms promote student success. This Program is now being replicated in other communities.

Nantucket’s shellfish resources are an important part of the Island’s history, culture, and economy. Nantucket waters support one of the country’s last wild-caught bay scallop fisheries. Elsewhere along the Atlantic coast, fishing pressure, habitat loss, and disease have severely depleted bay scallop populations. Although Nantucketers continue to make a living harvesting shellfish from the Island’s waters, many do so with concern for the future of the resources and the habitats that support them. This is a 6-credit Capstone course, Psychology majors intern 15 hours/week at a wide variety of human service agencies around Massachusetts. They in a weekly seminar to share their experiences, course readings, and have case presentations that complement the learning experience. UMass Boston has hosted Noyce Scholars for the last four academic years (2006-2010). The program is built upon seven years of an existing program in the UMass Boston Graduate College of Education: Teach Next Year (TNY). TNY is an accelerated teacher education program designed to prepare interns for urban teaching as they obtain their initial licensure. Noyce funding supports TNY interns who are dedicated to teaching math and science in urban schools. Graduates have all gone on to teaching positions in the Boston Public Schools.

Early Literacy Matters (ELM) is a partnership between the University of Massachusetts Boston, Lynn Public Schools, and the Gregg Neighborhood House. Our goal is to create 8 preschool centers of excellence impacting 300 children per year identified as “at risk for academic failure.” ELM has transformed 17 classrooms that serve children speaking over 41 languages. 75% of ELM children speak a first language other than English or have limited English proficiency, and 16% receive special education services. The mission of the program is to create a “pipeline” that starts at the middle and high schools in Boston, continues through the undergraduate programs at Tufts University and UMass Boston, and culminates in the graduate-level public health and/or medical programs at Tufts University School of Medicine or other medical schools.

The Elder Economic Security Standard Index measures the income that Massachusetts’ seniors need to maintain independence and meet basic living expenses. The sizable gap between Social Security benefits and living expenses occurs throughout Massachusetts, but is more substantial for singles than for couples. Those who are older, single, female or a member of a racial or ethnic minority group are exposed to especially high risk of falling short of Index values. Through this project, volunteers in communities throughout Massachusetts turn out in large numbers each September and October for COASTSWEEP, the statewide coastal cleanup program sponsored by the Massachusetts Office of Coastal Zone Management (CZM) and coordinated by the Urban Harbors Institute (UHI) of the University of Massachusetts Boston. COASTSWEEP is part of the International Coastal Cleanup (ICC) organized by Ocean Conservancy in Washington, DC. The Gastón Institute responds to requests from government and foundations for CBPR studies seeking to test theories and methods for addressing disparities in educational, health, and economic outcomes in U.S. Latino and Latin American communities. Our future research in this area seeks to answer questions about the factors that contribute to the production of disparities in health, educational, and economic outcomes. Our current CBPR projects seek to test the feasibility of a variety of intervention strategies.

Through this project, we have identified the following: 1. Students with disabilities and/or special health care needs are disproportionally under-represented secondary institutions; 2. Making the transition from secondary to post secondary institutions requires self determination and advocacy; 3. Although there are other programs with this transition, few focus on both educational and medical transitions for students with disabilities and ore special health care needs.

The Adult Literacy Resource Institute (part of UMass Boston since 1983, and 100% grant-funded) serves as the Greater Boston Regional Support Center for SABES, the state’s System for Adult Basic Education Support. We provide a variety of staff and program development services to adult basic education programs in the Boston area, especially the approximately 40 programs in the region (most located at community-based organizations) that are funded by the state’s Department of Elementary and Secondary Education and that offer ESOL, basic literacy and numeracy, GED preparation, and other classes (including family literacy, civics education, career pathways, and college transition).

Our work assists states with building a comprehensive understanding of the factors that influence employment outcomes at all levels: individuals and family, service providers, and state policy. The Framework for Employment, developed through extensive experience and research conducted within the state, is used to guide the much of the system improvement strategy work. The elements represent practices and outcomes known to be effective at enabling states to develop and sustain high-preforming integrated employment system.
