Glucose Phosphate Isomerase Deficiency: Unusual Acute Hemolytic Crisis In A Middle-Aged Woman, Koichi Maeda, Sheikh M. Saeed, Raymond W. Monto, Ernest Beutler

Hereditary hemolytic anemia associated with glucose phosphate isomerase (GPI) deficiency was first reported in 1967. Since then, about 30 cases have been reported in the literature; their ages ranged between 1 and 26 years. We present a case of glucose phosphate isomerase deficiency in a 56-year-old woman. Steroid therapy seemed to resolve our patient's acute stage. Since it has not been mentioned previously, further evaluation is necessary. Consideration of this deficiency may be helpful in investigating hemolytic anemia, regardless of the patient's age.

Richmond Smith As A Clinical Investigator: His Work On Adult Periosteal Bone Expansion And Nutritional And Endocrine Aspects Of Osteoporosis In Light Of Current Concepts, A. M. Parfitt

This paper discusses Richmond Smith's contributions to the field of metabolic bone disease from 1949 to 1969 in light of current concepts. He was the first to demonstrate continuing periosteal bone expansion in adults, a phenomenon of great biologic interest which is best explained if adult bone remodelling is qualitatively similar on all surfaces. He was the first to show that persons with less bone than normal have an increased risk of subsequent fracture, and so are more likely to benefit from preventive treatment. He found that blood vitamin D levels were lower in osteoporotic subjects than in controls, but …

Insulin-Resistant Diabetes Mellitus And Insulin Receptor Antibodies: Variable Association With Acanthosis Nigricans, José Goldman

Two patients with severe insulin-resistant diabetes mellitus and anti-insulin receptor autoantibodies are reported here. A marked decrease in the number of accessible insulin receptors was found in the monocytes of one patient. Otherwise, the Insulin receptors were of normal affinity compared to those of normal controls. Acanthosis nigricans was not present in one patient, and in the other patient it preceded the diagnosis of diabetes by ten years. We conclude that although acanthosis nigricans and insulin resistance are frequently associated, they probably do not bear a pathogenetic relationship to each other. The purpose of this study was to compare the proportion of women in the experimental group who had a successful breastfeeding experience following post-discharge nursing intervention with the proportion of women in the control group who had a successful breastfeeding experience without nursing intervention. Criteria defining success in breastfeeding were established before the study. Chi-square analysis was used to test for significance with the preset level of significance being α = 0.05. Feral pigeons (Columbia livia) are not harmless birds. Many potential infections of humans silently exist in pigeons which are not apparent. They have the potential for transmission of over 30 diseases to humans plus another ten to domestic animals. Environmental pollution from pigeon droppings is quite evident. Air pollution involves more than noxious gases from automobile exhausts and belching smokestacks. One serious air pollutant is air-borne fungi, which are agents for infectious diseases. There is much information written about the problem, but it remains primarily in the professional journals and technical references, neatly stacked away on library shelves. …

Rapid progress has been made in understanding the complexity of the immune system. The current status of lymphocyte subpopulations with regard to ontogenic development, their surface and intracellular markers, and functions in various Immunologic phenomena has been reviewed. Heterogeneity of T lymphocytes as defined by receptors for immunoglobulin isotypes is discussed in detail. Various cell types and their products involved in immunoregulatlon are also presented.

A Method For Evaluating The Platelet Surface Response By Using Electron Microscopy, Jeanne M. Riddle

Our method for evaluating the surface response of any given platelet population, which has evolved over the past two decades, provides a measure of both the adhesive and cohesive qualities of platelets. General morphologic features of the adherent, whole platelets can be assessed by transmission electron microscopy, and the results are reproducible when serial samples are collected on normal subjects and patient groups. With this technique, different metabolites and drugs can be combined with platelet populations that are either normoresponsive or hyperactive to determine if these substances trigger or inhibit the platelet response. This article provides an overview of the functions of human leukocytes in inflammation as elucidated by the Rebuck skin window technique. The migration sequence of various leukocytes into the field of inflammation is described, as well as cytologic, cytochemical, and transformational changes, and the interrelationships of responding leukocytes. Since it was introduced in 1955, the Rebuck skin window technique has provided an excellent means of studying in vivo inflammatory response to different phlogistic agents in normal individuals and in various disease states. This simple technique continues to prove fruitful to further study and to monitoring of many disease states.

We describe a rare case of a localized primary small intestinal lymphoma with features of both the Mediterranean and the Western type. This case demonstrates the need to regard intestinal lymphoma as a spectrum of diseases with the Mediterranean and the Western types at opposite ends and many variants in between. The role of the plasma cell infiltrate has been interpreted as reactive as well as neoplastic. In our case, both lymphocytic and plasmacytic proliferations spread widely in the intestinal submucosa, extended into muscularis propria and subserosal fat, and involved mesenteric lymph nodes. Bovine high molecular weight kininogen (bHMWK) partially corrects the aPTT of Fitzgerald-trait plasma, which is congenitally deficient in HMWK. The relationship between the structure and activity of HMWK was clarified by studying the effects of different fragments of bHMWK on the aPTT of Fitzgerald-trait plasma. The peptides studied, all in equimolar concentrations, were lys-bradykinin-free HMWK, bradyk In In-fragment 1-2-tree HMWK, heavy chain, fragment 1 -2-Hght chain, and light chain. Bradykinin- fragment 1-2-free HMWK, heavy chain, and light chain have little or no correcting activity upon Eitzgeraldtrait plasma aPTT. The anterior chamber fluid from the eye of this patient with sarcoidosis was found to contain microcolonies of a cellwall-deficient organism that was propagated and identified as acid-fast by the Intensified Acid-Fast stain. The colonies were inoculated into mice and retrieved from the dead or sacrificed animals. This report suggests that the acid-fast microbe in the aqueous humor of this case of uveitis-sarcoidosis may be the same organism as that found in the blood in sarcoidosis. Thus, it may be associated not only with the primary disease, but also with the complications of Boeck's sarcoidosis.

A Comparison Of Kidney Preservation Methods By Oxidative Phosphorylation Studies, Devprakash Samuel, Stanley G. Dienst

An oxygen consumption assay using adenosine diphosphonucleotide (ADP) was performed on canine kidneys to measure the efficiency of oxidative phosphorylation. The purpose of the assay was to establish a respiratory control index (RCI) that could be used to evaluate kidneys preserved by different methods. Of the four methods tested, perfusion yielded the best results after 24-hour preservation. Additionally, four pairs of human cadaver kidneys were evaluated with the same assay. Some correlation existed between the RCI values of the nontransplanted kidney in each pair and the clinical viability of its transplanted mate.
