/Among many measurable hormones, human chorionic gonadotropin (FICG) is commonly used for pregnancy testing because it is very sensitive and relatively specific. Pregnancy can be identified shortly after implantation. Because some reagents cross-react with luteinizing hormone (LH), the sensitivity of urine tests has been adjusted to maintain specificity. Radioreceptor assay s on serum are more sensitive than urine tests but similarly lack specificity. The problems of LH cross-reactivity are eliminated by antisera specific for the beta subunit. Quantitation of HCG provides additional information useful in diagnosing ectopic pregnancy, providing a prognosis in threatened abortion, and following neoplasms. Considerations of cost, …

Assessing the microbiological water quality of impoundments and the potential for waterborne disease outbreaks is a difficult task when using traditional sampling programs. Problems associated with using fecal coliform bacteria as indicators of human pathogen presence complicates assessments of future water quality in preimpoundment areas. Reliable determination of future and present microbiological water quality requires knowledge of how the chemical, physical, and biological characteristics of the watershed and impoundment interrelate to influence microbial indicator and pathogen densities. The porcine bioprosthesis has been the prosthetic valve of choice at Henry Ford Hospital since October 1971. By 1979, 23 cases of degeneration had been seen, and the rate seven years after implantation was 16%. Now, with a ten-year follow-up, there are 41 degenerated valves. After seven years, the percent free of degeneration is 88%, (SE of 2% [standard error]); at eight years, 82% (SE of 2.9%,); at nine years, 80% (SE of 3.4%); and at ten years, the percent free of degeneration is 69% (SE of 6.5%,). Immunological Involvement In Porcine Bioprosthetic Valve Degeneration: Preliminary Studies, Rollin H. Heinzerling, Paul D. Stein, Jeanne M. Riddle, Donald J. Magilligan Jr., John J. Jennings

Preliminary animal model studies have indicated that porcine bioprosthetic valves can be recognized by the immune system. This report presents the results of a preliminary investigation on the implanted valves of 27 patients. While none of the 27 demonstrated an immune response to porcine serum proteins, five patients developed antibodies against the implanted valve leaflets. Cells were enzymatically eluted from a degenerated bioprosthetic valve and were classified by means of monoclonal antibodies. Although these cells were not found to be of T-lymphocyte lineage, they did express la determinants and had the morphologic appearance of plasma cells. We therefore conclude that …

Spontaneous mammary carcinoma in C3H mice is preceded by premalignant hyperplastic alveolar nodules. When a primary tumor is evident, examination of the other mammae will reveal a number of microscopic precancerous lesions in various stages of transformation to overt malignant tumors. In an effort to inhibit the development of additional malignant tumors after surgical removal of the first apparent tumor, we treated female C3H/HeJ mice with subcutaneous injections of levamisole (0.6 mg per kg of body weight) on a twice weekly regimen. Acid phosphatase was the first tumor marker to be measured in blood. Although it is used as a marker of prostatic carcinoma, serum levels are also elevated in many diseases of prostate, bone, and hematologic tissue. While serum acid phosphatase concentration is elevated in patients with prostatic carcinoma or infarction, it may also be increased merely with benign hypertrophy and after prostate massage. Recent studies demonstrate that levels are not significantly increased after routine rectal examination. Although of limited use as a screening test, the assay may be useful in staging, prognostication during hormonal therapy, and in making clinical decisions. …

An Analysis Of A Sample Of Parents And University Freshmen In Regard To Their Attitudes Toward The Needs Of Sex Education In Community Schools, Linda C. Aleksa

This study attempted to investigate a sample of both parents' and college freshmens' attitudes toward sex education, and the content and timing of sex education in the Tidewater schools. Two Likert-type scales each containing ten statements were administered in order to survey attitudes. A statistically significant difference was found in scores on the attitude toward content and timing of sex education scale between parents and college freshmen. Parents demonstrated a more favorable attitude toward sex education as an integral component of the overall educational curriculum. Zoonoses are infections transmitted by animals to man. Of the more than 50 such infections that are clinically significant, several are discussed in the articles by McIntosh and Madhavan and by Meyer and Adar in this issue of the Journal (pp. 7-13). These include brucellosis, leptospirosis, relapsing fever, Colorado tick fever, toxoplasmosis, psittacosis, and echinococcosis. The McIntosh and Madhavan article reviews the experience at Henry Ford Hospital with 60 patients, while the article by Meyer and Adar reports on an unusual case of echinococcosis of the spine and retroperitoneum that occurred at Haim Sheba Medical Center in Israel.

Multi-individual cremation deposits found in the upper Santa Cruz River drainage were previously interpreted as the result of hypothesized cultural contact in the area. A review of 50 cremation deposits for which detailed analyses are available indicates that mulii-individual cremation deposits are the result of incomplete gleaning practices and have doubtful cultural significance. Hypotheses are generated from the present data which account for the apparent variation in cremation practices within the study area.

A clinical trial is currently in progress to determine the efficacy of combined fractions of hyperthermia and radiation. The protocol consists of two parts. First, four fractions of microwave-induced hyperthermia (45.0° ± 0.5°C) are applied for 1 1/2 hours to the volume encompassing the tumor, each separated by 72 hours. After a one-week rest, a second series of four fractions is administered again at 72- hour intervals. Each fraction consists of a 400 rad dose of radiation followed within 20 minutes by hyperthermia (42.5 ± 0.5°C) for 1 1/2 hours. Currently, we have treated 62 patients with 82 fields with …

In a series of animal tumor studies, heat was directly delivered into the tumor mass with an interstitial applicator. After hyperthermia (44°-45°C for 45 minutes) or radiation alone (3x10 Gy), only a slight delay in tumor growth was observed, while the combined treatment, either simultaneous or sequential (i.e., heat delivered four hours after irradiation), appeared to be the most effective. The cure rate in the two combined treatment groups was 100%, in contrast to the lack of cure in the first two groups. This report describes the history of hyperthermia and reviews current forms of treatment at both low (42°-43°C) and higher (<45°C) temperatures. Hyperthermia treatment at low temperatures includes fluid immersion and irrigation, regional perfusion with heated fluids, and electromagnetic radiofrequency waves. Low-temperature hyperthermia has also been combined with radiation therapy and with chemotherapy in recent clinical trials. At higher temperatures, we and other investigators have also had promising, preliminary results in treating tumors safely. With the specialized radiofrequency instrumentation we have developed to apply hyperthermia at any depth without preferential surface tissue heating, further clinical investigation of both superficial and deep internal solid tumors is now possible. Recent data suggest that hyperthermia may be an especially effective form of therapy for larger tumors that resist standard forms of treatment. Clinical trials are now underway to determine the most therapeutic dose/time regimen, to determine toxicity and therapeutic enhancement ratios of combined chemotherapy and x-irradiation with hyperthermia, and to evaluate any changes in the host immune system with such therapies.

It is known that above-normal temperatures (42°-42.5°C) provoke selective damage to neoplastic cells. We used heated circulating blood as a method for heat transfer on patients with limb tumors. From October 1964 to December 1979, we treated a total of 198 patients with hyperthermic perfusion for melanoma of the limbs (91), osteosarcoma (57), and soft tissue sarcoma (50). For melanoma patients, the five-year survival rate, excluding Stage IV, was 60%. For patients with soft tissue sarcoma, the five-year survival rates were 53% and 56% for hyperthermic perfusion and hyperthermic antiblastic perfusion. respectively. This report summarizes our preliminary experience at the Mallinckrodt Institute of Radiology treating 26 patients with metastatic tumors with hyperthermia and irradiation. Patients received 400 rad fractions given every 72 hours (twice weekly) for a total of 2400, 2800, 3200, or 4000 rad followed by hyperthermia. In addition, 18 lesions were treated according to a protocol designed by the Radiation Therapy Oncology Group (RTOG) for superficial measurable tumors to receive three doses of 500 rad followed by hyperthermia (six lesions) or irradiation alone (three doses of 600, 700, or 800 rad) every 72 hours.
