Criteria for the diagnosis of pituitary carcinoma are subject to controversy. Some authors do not accept the demonstration of cellular anaplasia alone as sufficient to establish the diagnosis but require evidence of metastatic disease in order to label a pituitary tumor as malignant. However, pituitary tumors can metastasize even in the absence of anaplasia. The present case of Cushing's disease and pituitary carcinoma had no clinical evidence of metastases when first reported. Partial surgical removal of the tumor followed by irradiation resulted in clinical remission of the Cushing's syndrome, but two years later progressive uncontrollable recurrence of the disease ended …

Homosexual and heterosexual patients with the acquired immunodeficiency syndrome were compared by risk group. Race; diagnoses; history of sexually transmitted diseases, sexual behavior, and drug use; and socioeconomic indicators differed considerably among the risk groups, suggesting different risk factors for acquisition of the syndrome. Patients in the homosexual, intravenous drug user, and Haitian risk groups differed in their serologic response to cytomegalovirus and syphilis testing, presumably due to lifestyle-related exposures. Differences in the rate of recovery of cytomegalovirus, serum levels of IgA and IgG, and antibody titers to Epstein-Barr virus were noted among patients with different diagnoses. We conclude that …

Too often when development planners or practitioners plan income-generating schemes for women they consider only handicrafts. While in some situations craft production may provide a secure source of income for women, in many cases it results in poor returns and proves more complicated an undertaking than expected. The Bangladesh Rural Advancement Committee (BRAC) is one agency that has developed a successful program of non-craft employment opportunities for women. Some 10,000 poor women have been engaged by BRAC in viable economic schemes: 9,000 in non-craft production. At the beginning of 1978, a group of families were awaiting access to low-cost housing in Mérida, a city on Mexico's southeastern coast. Some units were equipped with a new drainage system called SIRDO (Integrated System for Recycling Organic Wastes), and families interested in living in the experimental block where the SIRDO was to be installed could be given housing right away. Three years later, families in another community located in the crowded Valley of Mexico decided to try the system in their own neighborhood. Carotid Artery Disease, Carotid Endarterectomy, And Behavior: A Critical Appraisal, Mark P. Kelly

We reviewed the recent literature on the relationship between carotid artery disease, carotid endarterectomy, and behavior. The methodological adequacy of each study and the complexities involved in interpreting behavioral changes in patients with carotid disease are emphasized. Experimental design issues qualify conclusions in all studies. The weight of the evidence suggests that carotid artery disease may result in cognitive impairment, and that carotid endarterectomy is followed by psychometrically determined cognitive improvement in some patients. Although characteristics of the subgroup of patients most likely to improve have been identified in preliminary investigations, the clinical significance of such improvement has not been …

The zinc status of twelve lacto-ovo-vegetariah pregnant women and seventeen non-vegetarian pregnant women was evaluated. Zinc intakes were calculated from three-day dietary records and twenty-four hour dietary histories. Plasma zinc levels were drawn during the last trimester of pregnancy and approximately ten weeks postpartum. Gestational age at birth, birthweight, and APGAR scores of the infants born were obtained from the medical charts. Dietary zinc intakes for the vegetarians (10.5 ± 4.0 mg) did not differ significantly from the intakes of the non-vegetarians (10.9 ± 3.0 mg). We examined associations between physical fitness and risk factors for coronary heart disease in healthy women ages 18-65 years. Physical fitness was objectively determined by the duration of a maximal treadmill exercise test. Six physical fitness categories (very poor to superior), specific within 10-year age increments, were established. Mean risk factor levels varied across categories, but so did potential confounders such as age and weight. Multiple linear regression modeling was used to control for the effects of age, weight and year of exam on coronary risk factors. Studies showing an increase in plasma concentration of high density lipoprotein cholesterol (HDL-C) with moderate exercise have usually rejected the role of body weight change in the HDL-C raising process, ostensibly because the amount of weight lost has been negligible. To investigate HDL-C changes more thoroughly, we followed initially sedentary middle-aged men randomly assigned to either a moderate running (N=36) or a sedentary control (N=28) group for one year. Among runners, one-year changes in plasma HDL-C concentrations correlated strongly with their body weight changes (r = -0.53, P < 0.001). Curve-fitting procedures and regression analysis suggested that processes associated with weight change produce much of the plasma HDL-C changes induced by moderate exercise and that changes in HDL-C concentration predominantly reflect changes in the reputedly anti-atherogenic HDL2 sub-component. Otitis media continues to be a common and disabling problem which has become, in recent years, a fertile area for research. Based on studies in animals and humans, otitis media is classified into four groups: acute purulent otitis media (POM), serous otitis media (SOM), and mucoid or secretory otitis media (MOM). Together, these are referred to as otitis media with effusion (OME). The types can overlap and evolve into another type to become chronic otitis media and mastoiditis, characterized by the presence of granulation tissue or cholesteatoma. To identify risk factors for the occurrence of Kaposi's sarcoma and Pneumocystis carinii pneumonia in homosexual men, we conducted a case-control study in New York City, San Francisco, Los Angeles, and Atlanta. Fifty patients (cases) (39 with Kaposi's sarcoma, 8 with pneumocystis pneumonia, and 3 with both) and 120 matched homosexual male controls (from sexually transmitted disease clinics and private medical practices) participated in the study. The variable most strongly associated with illness was a larger number of male sex partners per year (median, 61 for patients; 27 and 25 for clinic and private practice controls, respectively). This issue of SEEDs tells the story of how the Working Women's Forum came into being in Madras, India and how it has brought together more than 13,000 poor, urban women around the issue of credit. It also describes how the Forum has not only provided its members with access to funds, but has expanded to include support services such as child care, education, health, and family planning and how the sense of strength and purpose that has grown up among the members is helping them to tackle the political and social problems that affect their lives.

Ce numéro de SEEDS retrace les différentes étapes qui ont mené à la création du Forum pour les Femmes à Madras, Inde et comment plus de 13,000 femmes pauvres ont pu, grâce à cette organisation, avoir accès au crédit. II décrit également comment le Forum a diversifié ses activités pour fournir à ses membres d'autres services, notamment dans le domaine de l'éducation, la santé, et la planification familiale et montre comment le fait de se sentir solidaires et reconnues permet à ses membres de mieux faire face aux problèmes politiques et sociaux qui affectent leur vie.
